Sivas Populasyonunda Faktör V Genin Leiden (G1691a) ve Hr2 (A4070g) Polimorfizmleri

Faktör V Leiden mutasyonu trombofilinin en sık görülen risk faktörlerinden biridir. Faktör V Leiden mutasyonun Avrupada sıklığının %7-10 olduğu belirlendi. Bu mutasyonu taşıyan populasyonlarda venöz tromboz, periferal vasküler hastalık, felç, açıklanamayan tekrarlayan gebelik kayıpları, pulmoner embolizm ve kalp krizi görülme riskinin arttığı ileri sürülmektedir. Bu mutasyondan sonra FV A4070G mutasyonu bulundu ve R2 olarak adlandırıldı. Bu polimorfizmin plazmadaki faktör V seviyelerini azalttığı, ayrıca iki fonksiyonel olarak farklı faktör V izoformu arasındaki dengeyi bozduğu ve koagulasyona eğilimi arttırdığı iddia edilmiştir. FV Leiden ve RH allelerinin prevalansı coğrafi bölgeler arasında önemli farklılıklar göstermektedir. Bundan dolayı bölgesel prevalansaların belirlenmesi, ilişkili hastalıklara yaklaşım için önemlidir. Bu amaçla, Sivas çevresinden rastgele seçilmiş 214 bireyde faktör V geninin Leiden ve R2 polimorfizmlerinin allel frekansı araştırıldı.
Materyal ve Metod: Hasta grubunu Sivas bölgesinde ikamet eden bireyler arasında rastgele seçtik. 103 erkek ve 111 kadın olmak üzere toplam 214 kişi çalışmamıza dahil edildi. Çalışma grubunun bireyleri, çalışmanın amacı hakkında bilgilendirildi ve yazılı onam belgesi imzalatıldı. Gen mutasyonlarını belirlemek için periferik kan örneklerinden DNA izolasyonu yapıldı. Mutasyon analizi revers hibridizasyon prensiplerine dayanan strip assay tekniğiyle yapıldı. Veriler SPSS 15.0 istatistik programı kullanılarak analiz edildi.
Sonuç: Şu ana kadar rapor edilmiş diğer populasyonlarla karşılaştırdığımızda, Sivas populasyonundaki FV Leiden ve HR2 taşıyıcılık frekanslarının bunlarla benzer olduğunu bulduk. Bu klinik açıdan takip edilmesi gereken bir bulgudur.; Factor V Leiden is one of the common known risk factors of thrombofhilic disease. Factor V Leiden mutation is determinated in Europe at a prevelance of 7-10%. It is suggested that the risk of venous thrombosis, peripheral vascular disease, stroke, unexplained fetal losses, pulmonary embolism and miyocardial infarctus at populations carry this mutation. After this mutation, Factor V A4070G polymorphism has been found and referred as R2 haplotype. This polymorphism has been claimed to decrease factor V levels in plasma and also alter the balance of two functionally different isoforms of Factor V and leads to thrombosis. The prevalance of FV Leiden and RH2 alleles shows significant differences between the geographic regions. Therefore, the regional prevalance is important in determining related diseases. In this regard, we investigated the allelic frequencies of Leiden and R2 polymorphisms of the Factor V gene in 214 randomly selected individuals around Sivas.
Materials and Methods: The study group was selected between individuals residing in Sivas. Total 214 subjects (103 men and 111 woman) were enrolled in the study. The subjects were informed of the purpose of the study and signed a written consent. The factor V gene polymorphisms were determined DNA isolation was performed from peripheral blood samples. The mutation analysis was performed by StripAssay technique which is based on the reverse-hybridization principle automatically. Data were analyzed by SPSS 15.0 package program.
Conclusions: Compared to all other populations reported so far, in our studys, Sivas population harbors generally the similar prevalance of the FV leiden and HR2 polymorphism. This is an finding to be followed in terms of clinical aspect.

Electrophysiological Changes In Iron Deficiency Anemia: Polyneuropathy

Demir eksikliği anemisi (DEA) en sık gıda eksikliği olarak bilinmektedir. Demirin beyin enerji metabolizması, nörotransmitter fonksiyonu ve miyelin formasyonu üzerindeki önemli rolüne bağlı olarak DEA davranışsal, gelişimsel ve kognitif disfonksiyonlara yol açabilmektedir.
Amaç: Çalışmamızda DEA’ nin periferik sinir sistemi üzerindeki etkilerini araştırmak ve olası elektrofizyolojik anormalliklerin uygun dozda demir tedavisi ile düzelip düzelmediğini incelemek amaçlanmıştır.
Materyal ve Metodlar: Yeni tanı almış 52 DEA hastası ve yaşları eşleştirilmiş 30 sağlıklı kişiye başlangıçta ve 3 aylık oral demir tedavisi sonrasında elektrofizyolojik inceleme yapılmıştır.
Bulgular: Dört (%7.69) hastada polinöropatiye ait elektrofizyolojik bulgular saptanmıştır. Üç aylık oral demir tedavisi sonrası polinöropatili 1 hasta dışında, tüm hastaların elektrofizyolojik olarak normale döndüğü görülmüştür.
Sonuç: DEA hastalarında demir eksikliğine bağlı nöropatik süreçlerin saptanması, DEA’ nin periferik sinir sistemi tutulumuna yol açabileceğini düşündürmektedir. Bu durum, nöropati hastalarında etyolojik açıdan demir eksikliğinin araştırılmasını vurgulamak açısından önemlidir. Nöropatiyle birlikte DEA’ nin varlığında, nöropatiye yönelik başka tedavi seçenekleri denenmeden önce demir replasmanı uygulaması faydalı olabilir.; Iron deficiency anemia (IDA) is known as the most common nutritional problem. Owing to the important role of iron in brain energy metabolism, neurotransmitter function and myelin formation, IDA may lead to behavioral, developmental and cognitive dysfunctions.
Objective: To investigate the electrophysiological effects of IDA on the peripheral nervous system, and to evaluate whether the possible electrophysiological abnormalities are reversible with appropriate doses of iron therapy, or not.
Materials and Methods: Electrophysiological evaluations were performed on 52 patients with newly diagnosed IDA and 30 age-matched healthy controls. The electrophysiological evaluations were repeated after 3 months of oral iron therapy.
Results: Four (7.69%) patients had polyneuropathy due to the electrophysiological evaluations. Except 1 patient with polyneuropathy, the electrophysiological findings of all patients were found to return to normal ranges after 3 months of oral iron therapy.
Conclusions: The detection of iron responsive neuropathic processes in IDA patients led us to think that IDA may cause peripheral nervous system involvement. It is important to emphasize the investigation of iron deficiency anemia as an etiological factor while planning a treatment for neuropathy patients. In cases where IDA is present, it would be benefical to treat IDA with iron replacement before applying other treatment options in neuropathy.

Serebral palsi hem term hem preterm doğmuş çocuklarda sık görülen nörolojik bir bozukluktur. Bozukluğun preterm ve term çocuklar arasındaki etiyolojik nedenleri, klinik ve radyolojik görüntüleme bulguları farklıdır. Bu çalışmada yaşları 6 ay-16 yaş arasında değişen 36 preterm ve 66 term Serebral Palsi’li (SP) 102 çocuk,etiyoloji, klinik ve manyetik rezonans görüntüleme (MRG) bulguları yönünden istatistiksel olarak araştırıldı.
Materyal ve Metot: Bu çalışmada bir yıllık bir sürede Malatya ili ve çevresindeki, yaşları 6 ay-16 yaş arasındadeğişen 36 preterm ve 66 term (SP)’li çocuk retrospektif olarak değerlendirildi.
Bulgular: Term serebral palsi (TSP)’lerin 26’sı (%39.4) ve preterm serebral palsi (PSP)’lerin 20’si (%55.5) ilk çocuk idi . En sık görülen SP nedeni, TSP’lerin %57.5’inde ve PSP’lerin %41.6’sında perinatal asfiksi idi. Spastik tetrapleji ve dipleji, TSP ve PSP’lerde sırasıyla %45, %19.6 ve %47, %38.8 oranında idi. Hem TSP hemde PSP’lerde sırayla %50’sinde mikrosefali tesbit edildi. TSP’lerde %31.8, PSP’lerde %47.2 şaşılık görüldü.
Kraniyal MRG’de, SP’lerin büyük çoğunluğunda patolojik değişiklik saptandı. Bu değişiklikler, TSP’lerin %44’ünde ve PSP’lerin %88’inde periventriküler lökomalazi (PVL), TSP’lerin %36.4’ünde, PSP’lerin %69.4’inde korpus kallozum (CC) patolojisi idi.
Sonuç: Çalışmamızda TSP ve PSP’lilerin etiyolojisinde en sık neden perinatal asfiksi, en fazla bulunan SP tipi de spastik tetrapleji olarak belirlendi. PSP’lilerde MRG’de, PVL ve korpus kallozum patolojisinin daha fazla olduğu saptandı.; Cerebral palsy (CP) is a frequent neurologic disease of term and preterm neonates. The disorder has various causes and the etiology, clinical manifestations and radiological screening findings are different in term and preterms. Thirty six preterm and 66 term CP patients aged between 6 months-16 years were evaluated in this study and were compared according to their etiology, clinical manifestation and cerebral Magnetic Resonance Imagıng (MRI) findings statistically.
Material and Method: In this study, 36 preterm and 66 term CP patients, aged between 6 months-16 years were evaluated retrospectively.These patients were living in Malatya city and its neighborhood.
Results: Twenty six of term (%39.4) patients and 20 (%55.5) of preterm patients were the first child. The most common CP etiology was perinatal asphyxia (%57.5) of term and (%41.6) of preterm. CP. The frequency of spastic tetraplegia and diplegia was respectively %45, %19.6 and %47, %38.8 in term and preterm CP. Fifty persent of term and %50 of preterm patients had microcephaly and %31.8 term and %47.2 preterm children had strabismus. Most of the CPs had cranial lesions on MRI. Periventricular leukomalacia (PVL) was seen in (%44) term and (%88) preterm patients. Corpus callosum (CC) pathology was seen in %36.4 of term and %69.4 preterm patients.
Conclusion: According to our findings, perinatal asphyxia was the most common CP reason and spastic tetraplegia was the most common CP type. Cranial MR images of preterm patients revealed that PVL and CC pathology were the most common central nervous system lesions.

In the Obstructive Sleep Apnea Syndrome (OSAS), the organism is exposed to hypoxic effects. Chronic hypoxia has thrombotic, vasoactive and inflammatory effects on cardiovascular system. This triggers the processes progressing to cardiovascular disease. While the pathological effects to which the patients are exposed are the same, marked differences are observed between patients in the progression of OSAS. Individual protective factors are thought to be effective in the development of these differences. In some studies, it was reported that the Val34Leu polymorphism of Factor XIII, which is also known as fibrin-stabilizing factor, might have a protective effect against cardiovascular diseases. In the light of these studies, we aimed to investigate whether this polymorphism has any effects on the development of the differences in the progression of the OSAS patients.
Materials and Methods: The patient group was selected from patients admitted to the our Sleep Laboratory. Total 119 of subjects, 44 OSAS patients, 32 OSAS patients with cardio vascular diseases (CVD) and 43 controls were enrolled in the study. The control group consisted of healthy volunteers who had no symptoms of OSAS and scored 0 on the Epworth sleepiness scale. In order to determine gene mutations, DNA isolation was performed from peripheral blood samples. Data were analyzed by SPSS 15.0 package program.
Results: While mean weight and BMI were significantly different, no significant differences were found between patient and control groups in terms of FXIII Val34Leu allele distributions.; Obstrüktif uyku apne sendromunda (OUAS) organizma hipoksik etkilere maruz kalmaktadır. Kronik hipoksi kardiyovasküler sistem üzerinde trombotik, vazoaktif ve inflamatuvar etkilere sahiptir. Bu da kardiyovasküler hastalık geşimine yol açansüreçleritetiklemektedir. Aynı patolojik etkilere maruz kalmalarına rağmen OUAS’nun progresyonunda hastalar arasında belirgin farklar gözlemlenmektedir. Bireysel koruyucu faktörlerin bu farklılıkların oluşmasında etkili olduğu sanılmaktadır.
Bazı çalışmalarda fibrin stabilize edici faktör olarak ta bilinen faktör XIII’ün Val34Leu polimorfizminin kardiyovasküler hastalıklara karşı koruyucu bir etki gösterdiği rapor edilmiştir. Bu çalışmalar ışığında bu polimorfizmin OUAS hastalarının progresyonundaki farklılıkların oluşmasında herhangi bir etkiye sahip olup olmadığını araştırmayı amaçladık.
Materyal ve Metod: Hasta grubunu uyku laboratuvarımıza başvuran hastalardan seçtik. 44 normal OUAS hastası, 32 kardiyovasküler hastalıklı (KVH) OUAS hastası ve 43 kontrol grubu olmak üzere toplam 119 kişi çalışmamıza dahil edildi. Kontrol grubu OUAS semptomları göstermeyen Epworth uykusuzluk skala değeri 0 olan sağlıklı gönüllülerden oluşturuldu. Gen mutasyonlarını belirlemek için periferik kan örneklerinden DNA izolasyonu yapıldı. Veriler SPSS 15.0 istatistik programı kullanılarak analiz edildi.
Sonuçlar: Hasta ve kontrol grupları arasında kilo ve BMI açısından önemli farklar bulunurken FXIII Val34Leu allel dağılımları açısından önemli bir farklılık bulunamamıştır.
