6- ÖZET Bu çalışmada mide kanseri (n=43), Hp( + ) gastrit (n=32), Hp(-) gastrit (n=32), Hp(+) ülser (n=34) ve Hp(-) ülserli (n=34) hastaların mide sularında malondialdehit (MDA) seviyesi, süperoksit dismutaz (SOD), katalaz (CAT) ve glutatyon peroksidaz (GSH-PX) aktiviteleri ölçüldü. Kontrol grubunu tamamen sağlıklı 33 kişiden oluşturulmuştur. Mide kanseri, Hp(+) gastrit, Hp(-) gastrit, Hp(+) ülser ve Hp(-) ülser olan hastalar kontrol grubu ile karşılaştırıldığında, mide suyu MDA düzeylerinde anlamlı bir artış görüldü (sırasıyla p Hp(+) gastrit, Hp(-) gastrit, Hp(+) ülser ve Hp(- ) ülser olan hastalar SOD ve GSH-PX aktiviteleri açısından kontrol grubu ile karşılaştırıldığında anlamlı bir artış görüldü. Kanserli hastaların SOD ve GSH-PX aktivitesinde istatistiksel olarak anlamlı bir fark bulunmadı. Çok az sayıda mide suyunda katalaz aktivitesi tespit edilebilmiştir. Kanser grubunun %7' sinde, Hp(+) gastritlerin %19'unda, Hp(-) gastritlerin %15'inde Hp(+) ülserlerin %21' inde Hp(-) ülserlerin %15'inde ve kontrol grubunun %19'unda aktivite tespit edilebilmiştir. Kanser grubu hariç tüm hasta grupları, kontrol grubu ile karşılaştırıldığında CAT aktivitesinin yüksek olduğu gözlenmiştir. Hücrelerde oksidatif strese bağlı hasar olması ve bu hücrelerden adı geçen enzimlerin mide suyuna geçmesi beklenen bir durumdur. Ayrıca lipid peroksidasyonu sonucu mide mukoza hücrelerinden mide lümenine salıverilen MDA da hastalıkların varlığı ve derecesi hakkında bize fikir verebilecek bir parametredir. Sonuç olarak; MDA ve diğer antioksidan enzimler bu tip mide hastalıklarında çalışıldığında bize oksidatif hasar ve antioksidan savunma sistemi hakkıda bir ön fikir verebilir. 67
7. SUMMARY In this study, ma londi aldehyde (MDA) levels and antioxidant enzymes activities (superoxide dimutase 'SOD', glutathione peroxidase ' GSH-PX', and catalase) were determined in gastric fluids of controls (n=33) and patients (n=175) suffering from different gastric diseases. The patient groups comprised of those: gastric cancers (n=43), Helicobacter pylori (+) gastritis (n=32), Helicobacter pylori (-) gastristis (n=32), Helicobacter pylori (+) gastric ulcers (n=34), and Helicobacter pylori (-) gastric ulcers (n=34). MDA level was found to be significantly increased in all patient groups when compared with controls (P<0.001,P<0.025,P<0.05,P<0.01,P respectively). In comparison with controls, both SOD and GSH-PX activities were significantly increased in gastric fluids of all patients groups. The exception was gastric cancer group in which no significant differences from controls were seen (P>0.05). In regard to catalase, its activity was detected in much lesser degree; 7% in gastric cancers, 19% in Helicobacter pylori (+) gastritis, 15% in Helicobacter pylori (-) gastritis, 21% in Helicobacter pylori (+) ulcers, 15% in Helicobacter pylori ( + ) ulcers, and 19% in control). It was also found that mean values of this enzyme in gastric fluids were higher in patients groups than controls. Again, the exception was cancers group. It is known that oxidative stress causes damage to gastric cells, thereby leading to passage and accumulation of antioxidant enzymes in gastric fluids. Additionally, MDA was secreted to the gastric fluid as consequence of lipid peroxidation. The amount of MDA may provide us prelimiary knowledge and prediction on the degree and the severity of gastric diseases. It can be concluded that MDA level may be a marker for the degree and the severity of gastric diseases.
