BÖLÜM IV ÖZET Deri lçishmaniosis'i halen bölgemizde önemli bir sağlık sorunu olarak devam etmektedir. Bu hastalığın tanısında yalnızca klinik veya yalnızca direkt mikroskopi kullanılması hasta ve hekim açısından bazı problemler oluşturmaktadır. Güvenilir tanı ve kemoterapiye cevabın takibi için daha sensitif ve özgül tanı yöntemlerine ihtiyaç vardır. Çalışmamıza 1996-1998 yılları arasında deri leishmaniosis'i klinik şüphesi olan 117si kadın, 83'ü erkek olmak üzere toplam 200 hasta dahil edilmiştir. Ayrıca çapraz reaksiyonları elimine etmek amacıyla sıtma'lı (10), toksoplazma'lı (10), amoebiosis'li (10), giardiosis'Ii (10), tüberküloz'lu (10) hastalardan alınan serumlar da çalışılmıştır. NNN besiyeri, İFA ve KUSA antijenleri laboratuvarımızda hazırlanmıştır. Tanı yöntemleri olarak direkt mikroskopi, kültür, ELISA ve İFA testi kullanılmıştır. Leishmaniosis için pozitiflik sırasıyla % 41.5, % 49, % 53, % 71 şeklinde bulunmuştur.. Kullanılan serolojik tanı yöntemlerinin sensitivite ve spesifıtesi ELISA'da % 53 ve % 80, IFA'da % 71 ve % 96 şeklinde tespit edilmiştir. ELISA testi ile tüberkülozlu bir hasta ve sıtma'lı bir hastada düşük titrasyonda (1/64) oranında seropozitiflik saptanırken İFA testi ile çapraz reaksiyon tespit edilmemiştir. Leishmaniosis tanısı için direkt mikroskopi ve kültürün kesin olarak yapılması gerekliliği, diğer serolojik testlerin birlikte kullanılmasının güvenirliliği artırdığı ve tedavi takibini kolaylaştırdığı gösterilmiştir. 76
SUMMARY Cutaneous leishmaniasis is still an important health problem in our region. In the diagnosis of this disease, if physician merely use clinic status or direct microscopy, there may be some problems. For reliable diagnosis and to follow the response to chemoterapy more sensitive and specific methods are needed. We included 200 patients (117 women and 83 men) in our study between 1996 to 1998 years. For elimination of cross reactions, sera with malaria (10), toxoplasmosis (10), amoebiasis (10), giardiasis (10), and tuberculosis (10) were studied. NNN medium, IFA, and ELISA antigens were prepared in our laboratory. As methods of diagnosis, direct microscopy, culture, ELISA, and IFA were used and the ratios of positivity are respectively 41.5 %, 49 %, 53 %, and 71 %. ELISA sensitivity and spesificity were calculated as 51 % and 80 %. IFA sensitivity and spesificity were calculated as 71 % and 96 %. Seropositivity of ELISA in a patient with tuberculosis and in a patient with malariasis patient was found in 1/64 titration (low). IFA test did not give cross reaction. In the diagnosis of Leishmaniosis, direct microscopy and culture are absolutely necessary; nevertheless other serological tests induce the realiability of diagnosis and facilitate the effectiveness of therapy. 77
