Öz:Amaç: Bu prospektif çalışmada, yenidoğan sepsislerinin tanısında laboratuvar testlerinin etkinliğini araştırmak ve ideal bir kombinasyon meydana getirmek amaçlandı. Yöntem; Yenidoğan sepsis bulguları gösteren 7, şüpheli sepsis kabul edilen 7 ve kontrol grubu olarak 9 yenidoğanın eritrosit sedimentasyon hızı (ESH), immatür/total nötrofil (İ/T) oranı, lökosit ve trombosit sayısı, C-reaktifprotein (CRP), alfa-1 asit glikoprotein (AAGP), interlökin (IL)-1beta, IL-6 ve tümör nekroz faktör-alfa (TNF-alfa) değerleri araştırıldı. Klinik skorlama olarak Tollner puanlaması esas alındı. Bulgular: Lökosit sayısı ve IL-1beta dışındaki tüm göstergelerde anlamlı farklılıklar saptandı. Laboratuvar göstergeleri ile bebeklerin klinik durumları arasındaki korelasyon incelendiğinde IL-6'nın en iyi korelasyonu gösterdiği (r= 0.745) ve bunu sırasıyla İ/T oranının (r= 0.677), CRP'nin (r= 0.637), TNF-alfa'nın (r=0.623), trombosit sayısının (r= -0.571), AAGP'nin (r= 0.550) ve IL-1beta'nın (r= 0.499) izlediği belirlendi. Yorum: Yenidoğan sepsislerinde İ/T oranı ve CRP'nin bakılmasının önemini bu çalışmada bir kez daha vurguluyor ve bu parametrelere IL-6'nın da dahil edilmesiyle klinisyenlerin sepsisi daha kolay tanıyabileceğini düşünüyoruz.
Abstract:Objective: In this prospective study, we aimed to investigate the efficacy of laboratory tests in the diagnosis neonatal sepsis, and to synthesis the optimal combination. Method: We studied erithrocyte sedimentation rate (ESR), immature to total (I/T) neutrophil ratio, neutrophil and thrombocyte counts, C-reactive protein (CRP), alfa-1 acid glycoprotein (AAGP), interleukin (IL)-1beta, IL-6 and tumor necrosis factor-alfa (TNF-alfa) in 7 infants with sepsis, 7 infants with suspected sepsis, and 9 infants as control. The clinical status of the infants was evaluated by Tollner's scoring sistem. Results: All tests, except for neutrophil count and IL-1beta, showed significant differences. With regand to correlation of the tests with clinical status, IL-6 showed the best correlation (r= 0.745) followed by I/T ratio (r= 0.677), CEP 0= 0.637), TNF-alfa (r= 0.623), trombocyte count (r= -0.571), AAGP (r= 0.550), and IL-1beta (r= 0.499). Conclusion: We emphasize the importance of I/T ratio and CRP, neonatal sepsis more easily when IL-6 was added into these tests.
