Öz:Amaç: Sezaryen operasyonlarında hem anne hem de fetusun güvenliğini sağlamak gerektiğinden anestezik yönetim çok önemlidir. Bu çalışmada İnönü Üniversitesi Tıp Fakültesi Anesteziyoloji ve Reanimasyon Anabilim Dalında 2009-2011 yılları arasında gerçekleştirilen sezaryen operasyonlarındaki anestezi uygulamaları geriye dönük olarak değerlendirildi. Gereç ve Yöntem: Sezaryenin acil ya da elektif oluşu, uygulanan anestezi yöntemleri, rejyonal anestezi tekniğinin başarılı olup olmadığı, hipotansiyon olasılığı, toplam efedrin tüketimleri, kullanılan ön yükleme sıvılarının niteliği kaydedildi. Genel anestezi altında operasyona alınan olgularda kullanılan kas gevşeticinin türü ve entübasyon güçlüğü, operasyonun sonunda entübe olarak yoğun bakıma çıkıp çıkmadığı ve mevcut ek hastalıkları kaydedildi. Bulgular: Hastaların %30.8’i acil, %69.2’si elektifti. %262’si genel anestezi, %71.6’sı spinal anestezi, %2,4’üne kombine spinal epidural anestezi uygulanmıştı. Acil alınan 782 olgunun 276 (%35)’ine spinal anestezi uygulanmıştı. Spinal anestezi için kullanılan hiperbarik bupivakain miktarı 9.1±1.1 mg idi. Spinal anestezi 66(%3) olguda başarısız olduğu için genel anesteziye geçildi. Sistolik kan basıncı ortalamaları 115±18 mmHg, diastolik kan basıncı ortalamaları 83.5±12 mmHg idi. Nabız ortalaması 81.7±10 atım/dk, hipotansiyon insidansı %40, toplam tüketilen efedrin miktarı 8.4± 2 mg idi. Ön yükleme sıvısı olarak %71.9 kristalloid, %17.6 kolloid, %10.1 kristaloid ve kolloid kombine kullanılmıştı. Kas gevşetici olarak %23.2 süksinilkolin, %2.8’inde rokuronyum kullanılmıştı. Olguların %4’ün de entübasyon güçtü, 34 (%1.3) olgu entübe olarak yoğun bakıma çıktı. Hastaların %83.4’ünde ek hastalık yokken, preeklamptik hasta oranı %8.3, HELLP sendromlu olgu sayısı %1.1 idi. Sonuç: Kliniğimizde sezaryenlerde rejyonal anestezi tekniklerinin kullanımı yüksek bulunmuş olup bu oran yurt dışındaki oranlara benzerdir. Tercih edilen spinal anestezinin başarısızlık ve komplikasyon oranı düşük bir teknik olduğu kanısındayız.
Abstract:Aim: Choosing the anesthesia technique should be carefully made because it would have an effect on both mother and fetus in cesarean section.The purpose of this study was to evaluate the anesthesia methods which was employed in Inonu University Medical Faculty, Anesthesiology and Reanimation Department between 2009-2011 in retrospective manner. Material-Methods: Techniques of anaesthesia, drugs used in regional anesthesia, ephedrine consumption of the patients have been recorded. Being elective or emergent, methods of anesthesia, regional anesthesia technique that was successful, the possibility of hypotension, total consumption of ephedrine, pre-loading solutions were recorded. In general anesthesia; type of muscle relaxant used in surgery, intubation difficulty of the patients, the rate of intubated patients admitted to ICU. Results: The mean presenting age of patients were 30.8 percent were urgent, 69.2 were elective. Twenty six percent of patients were undegoing to procedure under general anesthesia.7 1.6 percent were spinal anesthesia, and 2.4 percent were combined spinal epidural anesthesia. The amount of hyperbaric bupivacine for spinal anesthesia was 9.11.1 mg. In 66 of patients spinal anesthesia was failed and general anesthesia were administered. The mean systolic blood pressure was 11518 mmHg, mean diastolic blood pressure was 83.512 mmHg. The average puls rate was 81.710. The incidence of hypotension was 40%. The total amount of ephedrine consumption was 8.42 mg. Fluid preloading was obtained in 71.9 percent of patients with crystalloid, 17.6 percent with colloid and 10.1 percent with colloid and crystalloid solutions. Muscle relaxation was obtained in 23.2 percent of patients with succinylcholine, 2.8 percent with rocuronium. The rate of diffucult intubation was 4%. 34 of patients intuated in intensive care unit due to inadequate respiratory function. Patients 83.4% with no additional disease, while 8.3% of patients with preeclampsia, HELLP syndrome developed in 1.1% of patients. Conclusions: The use of regional anesthesia techniques in cesarean section in our clinic was high and similar to those abroad. Spinal anesthesia is the preferred of the opinion that a technical failure and a low complication rate.
