Psikotik belirtilerden sonra MRI ile chiari 1 malformasyonu saptanan ve cerrahi dekompresyon uygulanan bir olgu

Abstract

Sunulan olgu ilk olarak yedi y ı l, ikinci olarak da bir yı l önce psikiyatri kliniğinde şizofreni tan ısıyla yatı rı lmıştı r. Üçüncü yatışını n ikinci ay ı nda, psikoz tablosunda büyük oranda iyileşme görülmüş, ancak ataksi, hipotoni, düşme nöbetleri, disfaji gibi belirtiler ortaya çı km ıştı r. EEG ve BBT gibi incelemelerin sonuçlar ı , normal olarak değerlendirilmesine karşın, yeniden yap ı lan nörolojik bakı da ataksi, bilateral horizontal nistagmus, dismetri, dizartri ve kas zaafı saptandığı için multipl skleroz dü şünülmüştür. Bunun üzerine yapı lan kranial MRI incelemesinde Chiari-1 malformasyonuna uyan bulgular saptanmıştı r. Nöroşirürji kliniğince yapı lan cerrahi dekompresyon ile hastan ı n Chiari-1 malformasyonuna bağlı semptomları nda iyileşme sağlanmıştı r.

The case which is presented here has been hospitalized twice previously, with a diagnosis of schizophrenia; the first to be seven years and the second to be one year ago. Although satisfactory improvement in the symptoms of psychosis was obtained in the second month of third hospitalization, symptoms like ataxia, hypotonia, drop attacks and dysphagia occurred. As EEG and cerebral and cerebellar CT seans were reported as demonstrating no anomaly. multipl sclerosis became a probable diagnosis to explain the emerging sympoms. But signs found on cranial MRI were consistent with Chiari-1 malformation. Symptoms due to Chiari malformaton were absent after the surgical decompression.
