Hepatit C virüs enfeksiyonunda ledipasvir/sofosbuvir tedavisinin etkinliği

Abstract

Giriş: Bu çalışmada tedavi deneyimsiz ve tedavi deneyimli [Pegile-İnterferon±Ribavirin (PEG-IFN±RBV)] olup sonrasında nüks gelişmiş Hepatit C Virüsü (HCV) ile enfekte hasta grubunda, yeni geliştirilmiş bir Direkt Etkili Antiviral (DEA) ajan olan Ledipasvir/Sofosbuvir (LED/SOF) kombinasyon teavisinin etkinliği çalışılmıştır. Materyal ve Metod: İnönü Üniversitesi Turgut Özal Tıp Merkezi Gastroenteroloji Kliniğine 06.06.2016-23.05.2018 tarihleri arasında başvuran, 3-6 ay boyunca LED/SOF tedavisi alan, tedavi deneyimsiz (daha önce tedavi almamış) ve tedavi deneyimli (PEG-IFN±RBV) olup sonrasında nüks gelişmiş Hepatit C ile enfekte hastaların dosyaları incelenmiştir. Bulgular: Çalışmamıza 20 kadın (%44.4) 25 erkek, (%55.6) olmak üzere 45 hasta dahil edildi. Bu hastalardan 22'si daha önce PEG-IFN±RBV tedavisi alıp nüks etmiş olup, 23'ü tedavi deneyimsiz hasta idi. Hastaların yaşı, cinsiyeti, tedavi başlangıcındaki ve 6. aydaki aspartat aminotransferaz (AST), alanin aminotransferaz (ALT), alkalen fosfataz (ALP), gama-glutamil transferaz (GGT), total bilurubin, albümin, Uluslararası Düzeltme Oranı (INR), hemoglobin (HGB) ve trombosit (PLT) değerleri, tedavi başlangıcındaki, 1. ay, 2. ay ve 6. aydaki HCV-RNA düzeyleri incelendi. Tedavi öncesi HCV-RNA düzeyleri ile tedavi verildikten sonraki 1.ay, 2.ay ve 6.ay HCV-RNA düzeyleri birbirleri ile karşılaştırıldığında değerlerdeki azalma istatistiksel olarak anlamlı bulundu (p<0.05, p<0.05, p<0.05). Başlangıç ve 6. ay AST, ALT, GGT, albumin ve PLT sonuçları kendi aralarında incelendiğinde istatistiksel olarak anlamlı bulundu (p<0.05, p<0.05, p<0.05, p<0.05, p<0.05). Sonuç: HCV enfeksiyonunda tedavi deneyimli veya tedavi deneyimsiz hastalarda LED/SOF tedavisi güvenilir ve başarılı bulunmuştur. Anahtar Kelimeler: RBV, PEG-IFN, HCV-RNA, LED/SOF.

Introduction: In this study, the efficacy of Ledipasvir/Sofosbuvir combination therapy, a newly developed RIA, was studied in pre-treated (PEG-IFN±RBV) and treatment-naïve Hepatitis C patient populations. Material and Method: The files of patients infected with Hepatitis C who were admitted to the İnönü University Turgut Özal Medical Center Gastroenterology Clinic between 06.06.2016 and 23.05.2018, who were either pre-treated (PEG-IFN±RBV) or treatment-naïve (untreated) and had received LED/SOF treatment for 3-6 months following relapse, were evaluated. Results: Forty-five patients 20 females (44.4%) and 25 males (55.6%) were included in the study. Twenty-two of these patients had relapsed with PEG-IFN±RBV treatment and 23 were treatment-naïve. The age, sex, aspartat aminotransferaz (AST), alanin aminotransferaz (ALT), alkalen fosfataz (ALP), gama-glutamil transferaz (GGT), total bilurubin, albümin, International Normalized Ratio (INR), hemoglobin (HGB) ve platelet (PLT) 0values of the patients at the beginning of the treatment and at the 6th month of the treatment and HCV-RNA levels at the beginning of the treatment, 1st month, 2nd months and 6th months were evaluated. When pre-treatment HCV-RNA levels and post-treatment 1st month, 2nd month and 6th month HCV-RNA levels were compared with each other, the decrease in values was statistically significant (p <0.05, p <0.05, p <0.05) AST, ALT, GGT, albumin and PLT results were found to be statistically significant between the baseline and 6th month (p <0.05, p <0.05, p <0.05, p <0.05, p <0.05). Conclusion: LED/SOF treatment was found to be reliable and successful in pre-treated (RBV±PEG-IFN) and treatment-naïve Hepatitis C patients. Keywords: RBV, PEG-IFN, HCV-RNA, LED/SOF.
