ÖZET BAZAL HÜCRELİ KARSİNOM OLGULARINDA HİSTOMORFOLOJİK ÖZELLİKLERİN PROFİLİ ve PROGNOSTİK ANLAMI Bazal hücreli karsinomlar (BHK), tüm dünyada en yaygın cilt tümörlerinden biri olmasına rağmen, tümörün klinik ve tedavi seçenekleri ile korele histopatolojik sınıflaması hala pratik kullanıma girmemiştir. Bu çalışmada BHK' lann histopatolojik tip özellikleri ile Ki-67, bcl-2, p53, CD31 immunoreaktiviteleri arasındaki ilişki araştınlmıştır. Çalışma İnönü Üniversitesi Tıbbi Patoloji Anabilim Dalı' ında, 1997-2001 yılları arasında BHK tanısı almış 198 olguyu içermektedir. Hematoksilen eosin (H&E) ile boyalı kesitlerde tümörün gelişim paterni, histopatolojik tipi, tümör adalarının çevresindeki palizatlanma, mitoz sayısı, skuamöz metaplazi, nekroz, ülser, kalsifikasyon, stromal desmoplazi, tümöre karşı oluşmuş inflamasyonun derecesi, anaplazi, multisentrisite ve Demodeks follikülaris varlığı değerlendirildi. Stromadaki müsini göstermek amacı ile Alcian blue (pH 2,5), amiloid varlığını göstermek amacı ile Kristal viyole uygulandı. İmmunobistokimyasal olarak Ki-67, p53, bcl-2, CD31 belirleyicileri kullanıldı. Yüz doksan sekiz tümörün 98' inde mikst histpatolojik tip kaydedildi. En sık nodüler gelişim paterni (%40,4) olmak üzere infiltratif (%36,4), nodüler baskın infiltratif ( %21,7) ve superfisiyal (%1) gerişim patenleri izlendi. İnfiltratif gelişim paterni ile ülser (p<0,05), anaplazi (p=0,034), mitoz sayısı (p=0,014), stromal desmoplazi (p=0,015), palizatik dizilim kaybı (p=0,033) arasında ilişki saptandı. Damarlanma artışı ile nodüler gelişim paterni arasında negatif korelasyon (p=0,023) saptandı. Ülserasyon ile p53 boyanma derecesi arasında (p=0,03) ilişki saptandı, bcl-2 %25,9, Ki-67 %58,8, p53 % 82,6 oranmda değişen derecelerde pozitif bulundu. Demodeks follikülaris %29,4 oranmda saptandı. Prognoz hakkında fikir vermek ve tedaviyi yönlendirmek için BHK' un patoloji raporlarında histopatolojik tip, gelişim paterni, ülser, anaplazi, mitoz sayısı, nekroz, stromal desmoplazi varlığı belirtilmelidir. SUMMARY THE PROFILE AND PROGNOSTIC VALUE OF THE HISTOMORPHOLOGIC FEATURES IN CASES WITH BASAL CELL CARCINOMA Although basal cell carcinoma (BCC) is the most common tumour of the skin in the world, practically the standardization of histopathological classification of the tumour is not in use yet. In this study the correlation between the immunohistochemical staining of BCC by Ki-67, bcl-2, p53, CD31 and its histopathologic features were investigated. This study includes 198 cases which took diagnosis of BCC between 1997-2001 at the Department of Pathology of Inonu University Faculty of Medicine. The histopathological type, growth pattern, palisadic arrangement, mitosis, squamous metaplasia, necrosis, ulceration, calcification, sclerosis of the stroma, infl amatory response, anaplasia, and demodex follicularis were investigated with hematoxylen and eosin (H&E). Histochemically Alcian blue (pH2,5) was used to show mucin in the stroma and Crystal violet was used to show amiloid. Immunohistochemically Ki-67, p53, bcl-2, CD31 markers were used. Ninety eight of 198 cases of BCC were of mixed histopathologic type. The most common growth pattern was the nodular pattern (40,4%), and the other patterns were infiltrative (36,4%), superficial (1,5%) and nodular predominant infiltrative (21,7%). There was correlation between infiltrative growth pattern with ulceration (p<0.05), anaplasia (p=0,34), mitosis (p=0,014), sclerosis of the stroma (p=0,015) and poor palisading border (p=0,033). There was also correlation between ulceration and p53 immunostaining. There was negative correlation between augmentation of vascularization and nodular growth pattern. Demodex follicularis was seen in 29.4%. In pathologic reporting of BCC, especially the histopathologic type, growth pattern, necrosis, ulceration, stromal desmoplasia, anaplasia, number of mitosis must be noted. This will give an idea about the prognosis and will help for the decision of different treatment choices.
