Amaç: İsoproterenol ile in vivo kalp yetmezliği geliştirilmiş ratlarda ilk non peptit yapıda ve spesifik AT2 reseptör agonisti olan compound 21' in kardiyak hasardan koruyucu etkilerini araştırmak ve AT1 reseptör antagonisti olan valsartan ile karşılaştırmak amaçlanmıştır. Gereç ve Yöntem: Bu çalışmada tahmini ağırlıkları 250-400 g olan 56 adet Wistar-Albino ırkı erkek rat kullanılmıştır. Her biri 7 adet olan 8 gruba ayrılmıştır. Grup 1 (SHAM grubu) e herhangi bir ilaç uygulanmadı. Grup 2 (ISO) 180 mg/kg dozunda isoproterenol s.c. 24 saat aralıklarla iki doz uygulandı. Grup 3, Grup 4 ve Grup 5 e sırasıyla valsartan(30 mg/kg per oral) ,Compound 21 (0.03 mg/kg intraperitoneal) ve Valsartan + Compound 21 kombinasyonu 30 gün boyunca verildi. Grup 6, Grup 7 ve Grup 8'e ise sırasıyla Valsartan, Compound 21 ve Valsartan + Compound 21 kombinasyonu aynı uygulama şekli, süresi ve dozunda uygulanıp deneyin 29. ve 30. gününde İsoproterenol (180 mg/kg sc) verildi. Deneyin başlangıcında ve sonunda ratlara transtorasik ekokardiyografi yapıldı. Deney sonlandırılmadan önce karotid artere takılan kanülle kan basıncı, kalp hızı, EKG değişiklikleri kaydedildi. Ratların kalp dokusundan histopatolojik ve biyokimyasal değerlendirmeler yapıldı. Bulgular: Çalışmamızda Compound 21'in kalp yetmezliğinden koruyucu etkisi valsartana üstün bulundu. Ekokardiyografik ve biyokimyasal değerlendirme sonuçlarına göre Compound 21 ve Valsartan kalp yetmezliğinden koruyucu etki gösterdi. Histopatolojik bulgularda ise Compound 21 ve Valsartan + Compound 21 kombine tedavisi sadece valsartan uygulamasına göre kalp yetmezliği gelişimini istatiksel olarak daha fazla azalttı. Sonuç: Bu sonuçlar, Ang II' nin AT1 ve AT2 reseptörleri üzerinden kalp yetmezliğinde önemli bir rol oynadığını desteklemektedir. Valsartanın AT1 reseptörlerine antagonist, compound 21'in AT2 reseptörlerine agonist etki mekanizmaları kalbi AII'nin zararlı etkilerinden korumuştur.
Aim: The study aimed to research the protective effects of Compound 21, the first specific non-peptide AT 2 receptor agonist, from cardiac injury in the rats with isoproterenol-induced heart failure in vivo and compare it with valsartan, AT 1 receptor antagonist. Materials and Method: In this study, 56 Wistar albino race male rats (estimated body weights250 to 400 g) were used. They were divided into 8 groups of 7. Group 1 (SHAM group) was not given any drug while Group 2 (ISO group) was injected with two doses of İsoproterenol (180 mg/kg s.c) at intervals of 24 hours. Group 3, Group 4, and Group 5 were administered with Valsartan (30 mg/kg per oral), C21 (0.03 mg/kg intraperitoneal), and the combination of Valsartan and C21 for 30 days respectively. Group 6, Group 7, and Group 8 were subjected to the same administration mode, duration and dose and in the 29th and 30th day of the experiment, İsoproterenol (180 mg/kg s.c) was given. Transthoracic echocardiography was performed in the rats at the beginning and end of the experiment. The blood pressure, heart rate, and EKG alterations were monitored via the carotid artery cannula before ending the experiment. The histopathological and biochemical measurements were made on the cardiac tissue of the rats. Findings: In our study, the protective effect of C21 from heart failure is found to be superior than that of Valsartan. According to the echocardiographic and biochemical measurement results, C21 and Valsartan showed the protective effect from heart failure. In the histopathological findings, the treatment with C21 and the combination of Valsartan and C21 statistically reduced the heart failure progression more than the administration of Valsartan alone did. Result: These results support the hypothesis that Ang II plays a key factor in heart failure via AT 1 and AT 2 receptors. The mechanisms of antagonist action of Valsartan at AT 1 receptors and of agonist action of C21 at AT 2 receptors protect the heart from the harmful effects of AII.
