6. ÖZET Hipertansiyon özellikle preeklampsi ile komplike olduğu zaman anne ve fetusda en büyük hastalık ve ölüm sebebidir. Bu çalışma hipertansiyonla komplike gebelerde serum lipid değişimleri ve lipit fraksiyonlarının oranlarım değerlendirmek için yapıldı. Çalışma grubunda proteinürü (> 5g / 24 saat) ve yüksek kan basıncı değerlerine (> 1 60 / 110) göre pre- eklampsili (grup 1) 35 hasta, kontrol grubu' olarak 35 hipertansiyonu olmayan gebe kadın (grup 2) alınarak karşılaştırıldı. Her iki grup anne yaşları (grup 1 : 28,9 ± 5,2; grup 2: 27,9 ± 2,7), hamilelik süreçleri (38-40 hafta) ve B1M (vücut kitle indeksi) (grup 1: 27,5 kg/m ; grup 2: 24,8 kg/m ) yönünden karşılaştırıldı. Hipertansiyonlu ve hipertansiyonu olmayan gebelerin aile öykülerinde hiperlipidemi ve diabetes mellitus'lu hasta yoktu. Ayrıca gebelerin tümü hamilelik sırasında sigara içmedi.. Tam 12 saatlik açlık periyodundan sonra 5 ml venöz kan alındı. Toplanan bu örneklerden total kolesterol, trigliserit, apolipoprotein A-l (apo A-I), apo lipoprotein B (apo B), lipoprotein (a) Lp (a) ve LDL kolesterol (LDL-C) ile HDL kolesterol (HDL-C) analizleri yapıldı. Gruplar arasındaki farklılıkların istatistiksel analizleri çoklu kıyaslamayı içeren tek ya da iki yönlü varyans analizi kullanılarak yapıldı. Grup 1 ve 2'deki lipoprotein profillerine göre preeklampside serum trigliserit seviyeleri (258,8±66), hipertansiyonu olmayan gebe kadınlardaki değerlerle (198,4 ± 49) karşılaştırıldığında anlamlı şekilde yüksek bulundu. LDL-C / HDL-C ve total kolesterol (TC) / HDL-C oranlan preeklampside anlamlı olarak yüksekti. Kontrol grubuyla karşılaştırıldığında yüksek riskli hamile kadınlarda TC / HDL-C oranlan 1 kat ve LDL-C / HDL-C oranlan 1,5 kat daha yüksekti. Daha yüksek oranların sebebi yüksek total kolesterol değerinden çok, düşük HDL-C sonucu olarak yorumlandı. Bu çalışmadaki en belirgin bulgular hipertansif hastalıklarda trigliserit seviyesinin yükselmesiydi. TC / HDL-C oram aynı grupta anlamlı olarak yüksekti. Bu bulgular istatistiki olarak önemli olabilir. Çünkü preeklampside rastlanan lipoprotein profilleri, uterus spiral arteriolleri gibi predispose damarlarda patolojik lipid depolanmasının artması ile ilişkili olabilir. 43
7. SUMMARY Hypertension, especially when complicated by pre-eclampsia is a major cause of both maternal and fetal morbidity and mortality. This study was undertaken in order to evaluate the serum lipid changes and lipid fraction ratios in pregnancies complicated by hypertensive disorders (e.i. pre eclampsia). Study population comprised 35 patients with pre-eclampsia (group 1) according to clinical findings of proteinuria (> 5g/ 24 hi) and high blood pressure (>160 / 110) values which compared to 35 non hypertensive pregnant women as a control (group 2). Both groups were matched by maternal age (group 1 : 28,9 ± 5,2; group 2: 27,9 ± 2,7) and gestational period (38 to 40 weeks) as well as body mass index (group 1: 27,5 kg/m2; group 2: 24,8 kg/m2). None of the hypertensive and normotensive patients had a family history of hyperlipidemia and no patients had evidence of diabetes mellitus. Furthermore none of the patients smoked cigarettes during the present pregnancy. 5 ml venous blood was drawn after fasting period of at least 12 his. It was studied total cholesterol (TC), triglyceride (TG), apo A-l, apo B, Lp (a), LDL-C and HDL-C from these samples. Statistical analysis for differences between groups was undertaken using the one way or two way analysis of variance including multiple comparison. The lipoprotein profiles of the group 1 and group 2 show that serum triglyceride levels were significantly elevated in pre-eclampsia (258,8 ± 66) compared to non hypertensive pregnant woman (198,4 ± 49). The ratios LDL-C (134 ± 39,6) to HDL-C (58,3 ± 10,2) and of the total cholesterol (253,6 ± 47) to HDL-C were significantly elevated in pre-eclamsia. The TC / HDL levels were 1 times and LDL-C / HDL-C ratios were 1,5 times higher in the high risk pregnant patient group compared to controls. The higher ratios were appeared to be result of low HDL-C values rather than elevated TC levels. 44 The most prominent findings of this study was the elevated triglyceride levels in the hypertensive patients. The ratio of TC / HDL-C was significantly elevated in the same group. These findings may be significant importance since the lipoprotein profiles in the pre-eclampsia could be associated with enhancement of pathological lipid deposition in predisposed vessels such as the uterine spiral arteries. 45
