Down sendromlu hastalarda keratokonus nedeni ile uygulanan penetran keratoplasti

Abstract

Öz:AMAÇ: S.B. Kartal Eğitim ve Araştırma Hastanesi Göz Kliniğinde, Down sendromu ve keratokonusu bulunan 6 hastaya uygulanan penetran keratoplasti sonuçları incelendi. YÖNTEM: Üç hastada ameliyat nedeni akut korneal hidrops sonrası üç aylık tedaviyi takiben gelişen korneal skar, 2 hastada santral korneal skar, 1 hastada ileri keratokonus ile birlikte katarakt idi. Kataraktı olan bu hastaya kombine olarak keratoplasti, katarakt ekstraksiyonu ve göz içi lens yerleştirilmesi uygulanmıştır. BULGULAR VE SONUÇ: Bir hastada keratoplastiyi takiben birinci yılda gelişen katarakt nedeniyle katarakt ekstraksiyonu yapılmış ve göz içi lensi yerleştirilmiştir. 1 hastada gelişen grefon reddi medikal tedavi ile düzeltilmiştir. 2 hastada grefon hastalığı nedeni ile grefon bulanmış (%33.3) ikinci kez uygulanan keratoplastiler sonrasında grefon yine bulanık izlenmiştir. Greft bulanıklık oranı yüksek gibi görünse de, koruyucu tedbirler, dikkatli bakım ve erken tanı ile keratoplasti, Down sendromlu hastalarda bazen görme kalitesini yükseltmede tek umuttur ve başarılı bir yöntemdir.

Abstract:PURPOSE: Six patients with Down syndrome underwent penetrating keratoplasty for keratoconus. METHODS: In three patients, the indication for surgery was acute cornea) hidrops which had not resolved in three months before surgery. Two patients had central corneal scars. The other one patient had keratoconus with cataract. This patient underwent combined penetrating keratoplasty, cataract extraction and intraocular lens insertion. RESULT: Four of the six patients maintained clear grafts at their most recent follow up examination. In one patient; one year after keratoplasty, cataract was developed and he underwent cataract surgery and intraocular lens implantation. One of six patient had graft rejection and was treated succesfully. Two grafts were lost because of rejection. Rekeratoplasty was unsuccesful in these patients. Good results can be obtained with penetrating keratoplasty for keratoconus in patients with Down sydrome with protective measures and carefuly treatment. Keratoplasty can be a choise for Down sydrome with keratoconus.
