Klopidogrele bağlı kounis sendromu: nadir bir olgu sunumu

Abstract

Klopidogrel akut koroner sendromların tedavisinde ve stent trombozundan korunmada standart tedavinin önemli bir bölümünü oluşturmaktadır. Klopidogrel kullanan hastalarda istenmeyen majör kardiyovasküler olaylar üzerine olumlu etki yanında allerjik reaksiyonlar da görülebilmektedir. Bunların içinde ise anjiyoödem, sistemik inflamatuvar yanıt sendromu gibi hayatı tehdit eden durumlar oldukça nadir görülmektedir. İlaçlara bağlı gelişen alerjik reaksiyonlar alışılmışın dışında bulgu ve belirtilerle ortaya çıkabilmekte ve hatta akut koroner sendromlara neden olabilmektedir. Patogenezinde vazoaktif aminler ve enflamatuvar mediyatörlere bağlı hipersensitivite reaksiyonunun suçlandığı akut koroner sendromlar Kounis sendromu olarak tanımlanır. Vakamızda klopidogrel yükleme dozu sonrasında anjiyoödem ve solunum sıkıntısı ile birlikte sağ koroner arterde vazospazm gelişen 56 yaşında erkek hastayı sunduk.

Clopidogrel forms an important part of standard drug in the treatment of acute coronary syndromes and in the prevention of the stent thrombosis. Allergic reactions may also be seen as well as positive impacts on the major adverse cardiovascular events in patients taking clopidogrel. Among them life-threatening conditions such as angioedema, systemic inflammatory response syndrome has extremely rare seen. Allergic reactions caused by drugs emerge with unusual signs and symptoms and even may lead to acute coronary syndromes. Acute coronary syndromes that the hypersensitivity reactions due to vasoactive amines and inflammatory mediators are considered to be in the pathogenesis is defined as Kounis Syndrome. In our report we presented a 56-year-old male patient who had angioedema and respiratory distress together with vasospasm in the right coronary artery after the loading dose of clopidogrel.
