Pas III katkılı ve pas III katkısız donör trombosit süspansiyonlarının trombosit sayısı PH ve alerjik transfüzyon reaksiyonlarının üzerine etkilerinin karşılaştırılması

Abstract

Amaç: Güncel kan bankası pratiğinde donörlerden elde edilen aferez trombosit konsantreleri donörün plazması içinde süspanse edilmektedir. Hastalarda oluşan reaksiyonları azaltmak için donör plazmasının daha az kullanıldığı trombosit katkı solüsyonlu (platelet additive solutions, PAS) trombosit toplama yöntemleri giderek yaygınlaşmaktadır. Çalışmamızda PAS-III ile toplanan aferez trombosit süspansiyonlarında (I) trombosit kalitesini değerlendirmek ve trombosit transfüzyonuna bağlı (II) başta allerjik reaksiyonlar olmak üzere diğer transfüzyon reaksiyonlarının sıklıklarını incelemeyi amaçladık. Materyal ve Metot: Çalışmamızda Temmuz 2019 - Mart 2021 yılları arasında aferez trombosit setlerinin rutin kullanımı sonuçları retrospektif olarak değerlendirildi. Bulgular: Merkezimizde toplanan PAS-III ilaveli aferez trombosit konsantrelerinden farklı zaman dilimlerinde kalite kontrolü yapılanlar tümü (100 adet trombosit konsantresi) dahil edildi. Kan bankamızda 20-24 C0'de ajitasyon ile saklanan bu aferez trombosit konsantrelerinden 1. ve 5.günlerde trombosit sayıları ve pH değerleri incelendi. Çalişmamızda %65 PAS-III/%35 plazma aferez trombosit süspansiyonlarında 1. ve 5. günlerde ölçülen ortalama trombosit sayıları 3.44 x1011 ve 3.0 x 1011 olarak bulunurken, pH değerleri 1. ve 5.günlerde sırası ile ortalama 6.83 ve 6.62 olarak tespit edildi. Ayrıca allerjik ya da diğer herhangi bir transfüzyon reaksiyonu tespit edilemedi. Sonuç: Sonuç olarak, çalışmamızda % 65 PAS-III/%35 donör plazması içerisinde süspanse edilen aferez trombosit konsantrelerinde donör plazmasının daha az kullanımı ile ilişkili olarak (I) daha iyi donör konforu ve (II) donör plazmasından kaynaklanan transfüzyon reaksiyonlarının sıklığında ve/veya şiddetinde azalma olabileceği görülmektedir. Aim: In current blood bank practice, apheresis platelet concentrates obtained from donors are suspended in the donor's plasma. Platelet additive solutions (PAS) platelet collection methods, in which less donor plasma is used, are becoming more common in order to reduce the reactions occurring in patients. In our study, we aimed to evaluate the platelet quality in apheresis platelet suspensions (I) collected with PAS-III and to examine the frequencies of other transfusion reactions, especially allergic reactions related to platelet transfusion (II). Material and Method: In our study, the results of routine use of apheresis platelet sets between July 2019 and March 2021 were evaluated retrospectively. Results: All of the PAS-III added apheresis platelet concentrates collected in our center, whose quality control was performed in different time periods (100 platelet concentrates) were included. Platelet counts and pH values were examined on the 1st and 5th days of these apheresis platelet concentrates, which were stored in our blood bank at 20-24°C with agitation. In our study, mean platelet counts measured on days 1 and 5 in 65% PAS-III/35% plasma apheresis platelet suspensions were found to be 3.44 x 1011 and 3.0 x 1011, while pH values were determined as 6.83 and 6.62 on days 1 and 5, respectively. Also, no allergic or any other transfusion reaction was detected. Conclusion: In conclusion, in our study, apheresis platelet concentrates suspended in 65% PAS-III/35% donor plasma were associated with (I) better donor comfort and (II) reduction in the frequency and/or severity of transfusion reactions originating from donor plasma. appears to be possible.
