Turgut Özal Tıp Merkezi Kalp ve Damar Cerrahisi Kliniğinde Hemodializ İçin Yapılan Arteriovenöz Fistüllerin Erken ve Geç Dönem Komplikasyonları

Abstract

Kronik böbrek yetmezliği olan hastalar hemodiyaliz için uzun süre açık kalabilen kalıcı arteriovenöz (AV) fistüllere ihtiyaç duyarlar. Bu çalışmanın amacı, AV fistül operasyonları sonrası erken ve geç dönemde meydana gelen komplikasyonları belirlemektir.

Yöntem: Çalışmaya Kalp ve Damar Cerrahisi kliniğinde Ekim 2009 ve Şubat 2011 yılları arasında arteriyovenöz fistül operasyonu uygulanan 72 hastanın erken dönem (ilk 48 saat)komplikasyon gelişen (Grup 1) ile geç dönem (48 saat sonra) komplikasyon gelişen hastalar (Grup 2) retrospektif olarak değerlendirildi.

Bulgular: Erken dönemde 11 hastada tromboz, 4 hastada kanama, 3 hastada hematom gelişti. Grup 2’de ise 8 hastada anevrizma, 11 hastada stenoz veya tromboz, 5 hastada yüksek debili AV fistül, 2 hastada periferik iskemi gelişti.

Sonuçlar: Kronik böbrek yetmezlikli hastalarda, fistül açıklığını artıracak ve komplikasyonları azaltacak yöntemler geliştirilerek mortalite ve morbidite azaltılabilir.

The patients with chronic renal failure need Arteriovenous (AV) fistulas that are long term patency for hemodialysis. The aim of this study is to identify at the early and late stages of the complications after AV fistulas operations.

Materials and Methods: This study was performed retrospectively at the clinic of cardiovascular surgery between October 2009 and February 2011 and contains 72 patients who had undergone AV fistula operation. Group 1 consisted of reintervention at the early stage (first 48 hours), and group 2 consisted of reintervention at the late stage (term following 48 hours).

Results: Thrombosis (n=11), bleeding (n=4) and hematoma (n=3) at the early stage developed in the group 1. At the late stage aneurysm (n=8), stenosis or thrombosis (n=11), overflow AV fistulas (n=5), peripheric ischemia (n=2) were developed in the group 2.

Conclusions: The morbidity and mortality of patients can be reduced by developing methods that improve the patency of AV fistula procedure and decrease the complications in patients with chronic renal failure.
