Topiramat kullanan epileptik pubertal çocuklarda serum leptin, ghrelin, nöropeptit y ve adiponektin düzeylerinin değerlendirilmesi

Abstract

Giriş ve Amaç: Topiramat hem jeneralize, hem de parsiyel nöbetlerde etkili yeni kuşak antiepileptik ilaçlardan biridir. Topiramat kullanımı ile kilo kaybı sık görülen bir yan etkidir ve mekanizması henüz aydınlatılamamıştır. Bu çalışmada topiramatın pubertal epileptik çocuklarda serum leptin, ghrelin, nöropeptit y adiponektin düzeylerine etkisini ve kilo kaybının mekanizmasını araştırmak hedeflenmiştir.Gereç ve Yöntem: Bu çalışmaya epilepsi tanısı konularak topiramat tedavisi başlanan yirmiyedi pubertal çocuk dahil edildi. Olguların tedavi öncesinde ve tedavinin altıncı ayında serum leptin, ghrelin, nöropeptit y ve adiponektin düzeyleri ölçüldü; vücut kitle indeksi (VKİ) ve L/A oranlarları bakılarak birbirleriyle ilişkisi değerlendirildi.Sonuç ve Yorum: Olguların tedavi öncesine göre altıncı ayda ortalama serum leptin düzeylerinde ve L/A oranılarında anlamlı azalma gözlendi. Serum ghrelin, NPY, düzeylerinde ve VKİ'de anlamlı değişiklik gözlenmedi. Topiramata bağlı kilo kaybında özellikle L/A oranının önemli olduğu düşünüldü.Anahtar Kelimeler: topiramat, leptin, ghrelin, nöropeptit y, adiponektin

Background and aim: Topiramate is one of the new generation drugs which are used in both generalized and partial epilepsy. Weight loss is an commonly seen side-effect of topiramate with an undescribed mechanism. We aim to study the effects of topiramate, on serum leptin, ghrelin, neuropeptide- Y, adiponectine levels in pubertal children with epilepsy and obtain the mechanism of weight loss.Material and Method: Twenty seven pubertal children with epilepsy who are on topiramate are included in this study. Before and at the sixth month of medication, serum leptin, ghrelin, neuropeptide Y, adiponectine levels, body mass index and leptin/adiponectine ratio were recorded to evaluate the relationship with each other.Results and conclusion: There was a significant decrease in serum leptin levels and leptin/adiponectine levels in sixth month of medication compared to premedicational values. There was no significant change in the level of serum ghrelin, adiponectine, neuropeptide y levels and body mass index. We concluded that the weight loss due to topiramate has a significant relationship with serum leptin/adiponectine ratio.Key words: topiramate, leptin, ghrelin, neuropeptide Y, adiponectine
