Bu araştırmanın amacı, daha önce doğum yapmamış gebelerin doğum şekli tercihleri üzerine sağlık eğitiminin etkisini belirlemekti. Bu araştırma, tanımlayıcı ve tek grup son test deneme öncesi modeli olarak yapılmıştır. Araştırma, 14 Eylül 2011-11 Şubat 2014 tarihleri arasında yürütüldü. Araştırmanın evrenini Malatya ilinde 8 ASM'ye kayıtlı daha önce hiç doğum yapmamış ve gebeliğinin 7. ayı ve üzerinde olan 200 gebenin taranması sonucu sezaryen doğum isteyen veya doğum şekli konusunda kararsız olarak belirlenen 58 gebe oluşturdu. Evrenin tamamı örnekleme alındığı için herhangi bir örnekleme yöntemi kullanılmamıştır. Veriler Anket Formu kullanılarak toplandı. Sezaryen isteyen veya doğum şekli konusunda kararsız olan her kadına, Eğitim Materyali kullanılarak 15 dakikalık normal doğumun faydaları ve sezaryenin riskleri konusunda bire bir eğitim evlerinde verildi. Kadınlar doğumlarını yaptıktan sonra Anket Formunun doğum sonrası kısmı dolduruldu. Elde edilen verilerin istatistik değerlendirilmesinde; yüzdelik dağılım ve Fisher Exact testi kullanıldı. Tanımlatıcı gruptaki 200 gebenin planladıkları doğum şekli tercihleri incelendiğinde %71.0'inin vajinal doğum, %22.0'sinin sezaryen doğum istediği ve %7.0'sinin doğum şekli konusunda kararsız olduğu belirlenmiştir. Çalışma grubundaki 58 gebenin %84.5'i gebeliğin özel hastanede ve tamamı doktor tarafından takip edildiğini belirtmiştir. Gebelerin %75.9'u sezaryen doğum istediğini, %24.1'i ise kararsız olduğunu belirtmişti. Gebelerin %93.1'i sezaryen doğum yapmış ve %77.8'i sezaryen doğuma hekimin karar verdiğini belirtmiştir. Gerçekleşen doğum şekli ile kadının eşinin çalışması, gebeliğin takip edildiği yer, doğumun yapıldığı yer ve planlanan doğum şekli arasında istatistiksel olarak anlamlı fark saptanmıştır (p<.05). Gerçekleşen doğum şekli ile kadının yaşı, kadın ve eşinin eğitimi, kadının çalışması ve aylık gelir durumu arasında anlamlı fark saptanmamıştır (p>.05). Bu araştırmanın sonuçları, sezaryen doğum oranının yüksek olduğunu ve verilen bireysel eğitimin sezaryen oranını azaltmadığını gösterdi. Vajinal doğum oranlarını arttırmak için; birinci basamak sağlık kurumlarında çalışan hemşireler-ebeler tarafından kadınların antenatal dönemde vajinal doğum yapmaları konusunda cesaretlendirilmesi, eğitim verilmesi ve eğitimin sürekliliği önerilebilir.
The aim of this study was to determine the effect of health education on pregnant women's birth preferences who have never given a birth. This study was carried out as a model of defining and single group and last test of pre-trial. The research was carried out between September 14 th and February 11 th 2014. The universe of the study consisted of 58 pregnant women who wanted to give birth with cesarean and those who didn't make up their mind about the form of giving birth, all of whom were chosen among 200 pregnant women who were registered to the Center of Family health in the city of Malatya and who had never given any birth and who were in the 7th month of their pregnancy or more. All the universe was regarded as the sample, that is why no sampling method was used. The data were collected by using questionnaire. Each woman who to give birth by caesarean or indecisive about how to give birth was given a 15-minute training in their houses about the risks of caesarian section and benefits of vajinal birth by using educational materials. After the women gave birth tho their babies, they were asked to fill the part of the questionnaire which was allocated to post-natal period. Percentage distribution and the Fiscer Exact test were used for statistical analysis of the data obtained. When the preferences of 200 pregnant women were examined, 71.0 % pregnant women want vajinal birth and 22.0 % of them want caesarean and 7.0 % of them stated that they were indecisive about how to give birth. 84.5 % of 58 pregnant women in the group said that they were all controlled at private hospitals by a doctor regularly. 75.9 % of the pregnant women had stated that they wanted normal birth and 24.1 % of them were indecisive. 93.1 % of the pregnant women gave birth by caesarean and 77.8 % of them said that their doctor decided how to give birth. It has been determined that there is statistically a significant relation between the type of birth and the job of wife's husband; between the place where pregnancy is checked and how the birth is planned to happen (p<.05). No significant difference has been observed between the form of birth and the age of woman, the educational state of wife and her husband, the job of the woman and monthly income (p>.05). The results of these study showed that the rates of caesarean were high and giving individual training was not an effective way to diminish the rates of caesarean. To increase the rates of vajinal birth, it can be suggested that women in the antenatal period should be encouraged and trained for vajinal birth by nurses and midwives who are working at health care institution and that the training should be maintained.
