Amaç ve metod: Sitokinler natriüretik peptit salgısı üzerine direkt stimülatör etkisine sahiptir. NT-proBNP diyaliz hastalarında mortalite açısından değerli bir biyomarkırdır. Bu çalışmanın amacı periton diyalizi ve hemodiyaliz hastalarında Tiyoredoksin redüktaz, NT-proBNP, IL-6 ve hsCRP düzeylerinin karşılaştırılması ve aralarındaki ilişkinin incelenmesidir. Aynı yaş ve cinsiyetteki 30 HD hastasında, 30 PD hastasında ve 20 kişilik sağlıklı kontrol grubunda hsCRP, Tiyoredoksin redüktaz, serum IL-6 ve NT-proBNP düzeyleri ölçüldü. Sonuçlar: hsCRP, IL-6 ve NT-proBNP düzeyleri HD ve PD hastalarında kontrol grubuna göre anlamlı yüksekti (p<0.05). hsCRP, IL-6 ve NT-proBNP düzeyleri açısından HD ve PD grupları arasında fark yoktu (p>0.05). ). Tiyoredoksin redüktaz düzeyleri açısından 3 grup arasında fark yoktu (p>0.05). Tiyoredoksin redüktaz, çalıştığımız herhangi bir parametre ile korele değildi. BMI ,hsCRP, IL-6 ve NT-proBNP ile pozitif korelasyona sahipken, serum albumini ile negatif korele idi. Serum IL-6 düzeyleri, hsCRP ve BMI ile pozitif, albumin ile negatif korelasyona sahipti. Serum NT-proBNP seviyeleri serum albumini ile negatif korele iken, yaş, hsCRP, sistolik ve ortalama kan basıncı ile pozitif korele idi. Çoklu regresyon analizleri sonucu serum hsCRP'nin belirleyicisi IL-6 (β:0.677, p:0,000) iken, IL-6'nın belirleyicisi hsCRP (β:0.677, p: 0,000) idi. NT-proBNP'nin belirleyicisi serum albumini (β:-0.416, p: 0,000) ve yüksek sistolik kan basıncı (β:0.414, p:0,000) idi. Sonuç olarak HD ve PD hasta grubunda sağlıklı kontrollere göre serum NT-ProBNP, hsCRP ve IL-6 düzeyleri anlamlı olarak yüksekti. Tiyoredoksin redüktaz düzeyleri açısından 3 grup arasında fark yoktu. Diyaliz hastalarında NT-proBNP düzeyleri , IL-6 ve Tiyoredoksin redüktaz düzeyleri arasında ise ilişki yoktu.
Cytokines have a direct stimulating effect on natriuretic peptide secretion. N-terminal probrain type natriuretic peptide (NT-proBNP) is a valuable biomarker for mortality in dialysis patients. The aim of this study was to compare thioredoxin reductase, NT-proBNP, IL6 and High Sensitivity C reactive protein (hsCRP) levels in peritoneal dialysis (PD), and hemodialysis (HD) patients and investigate relationship among these parameters. Material and methods: In age and sex matched 30 HD patients, in 30 PD patients and 20 healthy controls, hsCRP, thioredoxin reductase, serum IL6, and NT-proBNP levels were measured Results. hsCRP, IL6 and NT-proBNP levels in HD and PD patients were significantly higher than in controls (p<0.05). There was no difference according to hsCRP, IL6, and NT-proBNP between HD and PD patients (p>0.05). Thioredoxin reductase was not different in 3 groups (p>0.05). Thioredoxin reductase was not correlated with any parameters studied. hsCRP were positively correlated with BMI (r:0.275,p:0.034), IL6 (r:0.633,p:0.000), and NT-proBNP (r:0.277,p:0.032), and negatively correlated with serum albumin (r:-0.425,p:0.001). Serum IL6 levels were positively correlated with HsCRP (r:0.633,p:0.000), BMI (r:0.775,p:0.034) and negatively correlated with albumine (r:-0.342,p:0.007). Serum NT-proBNP levels were negatively correlated with serum albumin (r:-0.385,p:0.002), and positively correlated with age (r:0.315,p:0.002), HsCRP (r:0.277,p:0.032), systolic (r:0.421,p:0.001) and mean blood pressure (r:0.311,p:0.015) . In multiple regression analyses, the predictor of hsCRP was IL6 (β:0.677,p:0.000), the predictor of serum IL6 was HsCRP (β:0.677,p:0.000). The predictors of serum NT-proBNP levels were serum albumin (β:-0.416,p:0.000) and mean blood pressure (β:0.414,p:0.000). As a conclusion, serum NT-proBNP, HsCRP, IL6 levels are significantly higher in HD and PD dialysis patients. Thioredoxin reductase was not different in 3 groups. NT-proBNP levels in dialysis patients are not related with IL6 levels and thioredoxin reductase.
