Amaç Parsiyel üreteral obstrüksiyon oluşturulmuş ratlarda böbrekte meydana gelen hasarın önlenmesinde antioksidan ve antiinflamatuar etkinliği olan Ghrelin'in koruyucu ve tedavi edici etkilerinin araştırılması amaçlanmıştır. Gereç ve Yöntem Çalışmaya Wistar Albino cinsi erişkin 28 rat dahil edildi ve ratlar 4 gruba ayrıldı. Sham grubuna laparotomi yapıldıktan sonra retroperitoneal alanda üreter bulundu ve usülüne uygun olarak tekrar kapatıldı (n=7). Ghrelin grubuna yedi gün boyunca intraperitoneal Ghrelin uygulandı ve on beşinci gün nefrektomi uygulandıktan sonra sakrifiye edildi (n=7). Parsiyel üreteral obstrüksiyon grubuna laparotomi sonrası parsiyel üreteral obstrüksiyon yapıldı. Bu işlemi takiben yedi gün beklendi ve sonraki yedi gün intraperitoneal serum fizyolojik verildi. On beşinci gün nefrektomi uygulandıktan sonra ratlar sakrifiye edildi (n=7). Parsiyel üreteral obstrüksiyon+Ghrelin Grubuna laparotomi sonrası parsiyel üreteral obstrüksiyon yapılıp yedi gün beklendi. Bunu takip eden yedi gün boyunca intraperitoneal 10ng/kg/gün dozunda Ghrelin uygulandı ve on beşinci gün nefrektomi uygulandıktan sonra ratlar sakrifiye edildi (n=7). Tüm gruplar histolojik hasar yönünden değerlendirildi ve aynı dokularda Katalaz, Süperoksit Dismutaz, Total Glutatyon, Malon Dialdehit ve Myeloperoksidaz ölçümü yapıldı. Bulgular Histolojik değerlendirmede Parsiyel üreteral obstrüksiyon grubu ile sham grubu karşılaştırıldığında parsiyel üreteral obstrüksiyon grubunda hasarın istatistiksel olarak anlamlı şekilde arttığı gözlendi (p=0.001). Parsiyel üreteral obstrüksiyon+Ghrelin grubu ile parsiyel üreteral obstrüksiyon grubu karşılaştırıldığında ise bu grupta histopatolojik değişikliklerin istatistiksel olarak anlamlı derecede azaldığı tespit edildi (p=0.001). Biyokimyasal analiz yapıldığında parsiyel üreteral obstrüksiyon grubu ile sham grubu karşılaştırıldığında biyokimyasal değişikliklerin Katalaz, Total Glutatyon ve Myeloperoksdaz düzeylerinde istatistiksel olarak anlamlılığı(p<0.05) tespit edildirken, parsiyel üreteral obstrüksiyon grubu ile parsiyel üreteral obstrüksiyon+Ghrelin grubunun karşılaştırılmasında Süperoksit Dismutaz ve Total Glutatyon düzeylerinde istatistiksel anlamlılık saptandı (p<0.05). Sonuç Deneysel parsiyel üreteral obstrüksiyon oluşturulmuş ratlara Ghrelin verilmesi antiinflamatuar ve antioksidan etkilerinden dolayı doku hasarını azaltmaktadır. Ostrüktif üropati gelişen durumlarda Ghrelin verilmesi biyokimyasal ve histopatolojik olarak doku hasarını önleyebilir. Ghrelin'in klinik kullanıma girebilmesi için bu konuda daha geniş serili ve çok merkezli çalışmalara ihtiyaç vardır.
Object Protective and therapeutic effects of Ghrelin which has the antioxidant and anti-inflammatory activity on preventing kidney damage that was occurred by induced partial ureteral obstruction in rats was aimed to investigate. Material and Method 28 adult male rats are breed of Wistar albino were included to the study and rats were divided into 4 groups. After the laparotomy operation to Sham group ureter was identified at the retroperitoneal area and was duly sutured (n = 7), Ghrelin was performed for seven days intraperitonealy and after the nephrectomy at the fifteenth day rats were sacrificed ( n=7). Partial ureteral obstruction was performed after the laparotomy to the PUO. We waited for seven days after this procedure and salin infusion was performed intrapetionealy for next seven days. Rats were sacrifiated after the nephrectomy operation at fifteenth day (n=7). Partial ureteral obstruction was performed after the laparotomy and waited for seven days in the PUO+Ghrelin group. Ghrelin was given at the 10ng/kg/day dose intraperitonealy for next seven days and rats were sacrificed after the Nephrectomy operation at fifteenth day (n=7). All groups were evaluated for histological damage and Catalase, Superoxide Dismutase, Total Glutathione, Malondialdehyde and Myeloperoxidase levels were measured in the same tissues. Results When second group and the group with sham were compared histologically, it was observed that damage had increased in statistically significant in partitial ureteral obstruction group (p=0.001). When ghrelin-treated after partial ureteral obstruction group was compared with the group of just partial ureteral obstruction, histopathological changes were found to be significantly reduced in this group (p=0.001). While Statistical significance on CAT, GSH and MPO enzymes levels were detected on biochemical changes in the second group when compared with the sham group (p <0.01), the third group showed a statistically significant difference in the levels of SOD and GSH enzymes compared with the fourth group (p<0.05). Conclusion Ghrelin administration to the rats after experimental partial unilateral ureteral obstruction forming, ghrelin administration reduces tissue damage due to ghrelin's anti-inflammatory and antioxidant effects. Ghrelin administration may prevent tissue damage by biochemically and histopathologically in cases of obstructive uropathy events. In order to get in to the clinical use of Ghrelin, larger series and multicenter studies are needed in this regard.
