Abstract:
Öz:Gebeliğe bağlı fizyolojik ve kolumna vertebralisteki anatomik değişiklikler nedeniyle spinal anestezide lokal anesteziklere yanıt gebe olmayanlara göre farklı olabilmektedir. Bu çalışma gebelerde mekanik faktörler ile hipotansiyon arasındaki ilişkiyi analiz etmek için planlandı. Olgular; gebe olup spinal anestezi ile sezaryen yapılacaklar (GS, n=14) ve gebe olmayıp spinal anestezi ile jinekolojik girişim (GJ, n=13) uygulanacaklar olmak üzere iki gruba ayrıldı. 2,5 ml % 0.5'lik hiperbarik bupivakainin L3-4 aralığından intratekal uygulanmasından hemen sonra her iki gruptaki tüm olgulara sol lateral-tilt pozisyonu verildi. Gruplardaki en yüksek duyu bloğu seviyesi ortalaması sırasıyla GS için; T3.5 (T1-T6) ve GJ için T6 (T3-T8) saptandı ve iki grup arasında fark istatistiksel olarak anlamlıydı (p < 0.05). Duyusal blok en üst seviyeye ulaşana kadar geçen zaman GS 'te 5.1±1.6 dakika iken GJ'de 6.4±1.3 dakika idi. Jinekolojik girişim yapılan olgularda en yüksek duyu bloğu seviyesi T5-6 olan 9 olgu saptandı ve bu olguların hiçbirinde kan basıncı değişikliği gözlenmezken gebelerde bu seviyede kalan 5 olgunun 3’ünde hipotansiyon belirgindi (p < 0.05). Sonuç olarak gebeliğe bağlı hipotansiyonda mekanik faktörlerin dışındaki diğer nedenlerin araştırılması gerektiğini düşünmekteyiz.

Öz (İngilizce):Reaction to local anesthetic related to pregnancy is different from non-pregnant women because of physiological and mechanical factors in spinal column. This study was undertaken to analyse the possible relationship between mechanical factors and hypotension. Patients were allocated into two groups as pregnants undergoing caesarean section (GS) and nonpregnants undergoing gynecological surgery (GG) under spinal anesthesia. In two groups, 2.5 ml of 0.5% bupivacaine was administered at the level of L3-4 . Immediately after the drug injection, all women were turned to the supine horizontal position with left tilt. The mean height of block was to T3.5 (T1-T6) and T6 (T3-T8) in the groups GS and GG, respectively and the difference was significant between the groups (p< 0.05). The onset time to maximum level of sensory block height were 5.1±1.6 min and 6.4±1.3 min in GS and GG groups, respectively in the GG . In the cases which had gynecological operation, there were 9 patients with maximum level of sensory block level T5-6 . While there were not blood pressure changes in these patients, the 3 of the 5 caesarean section patients who had same sensory block height developed hypotension (p < 0.05). We conclude that, the other reason except for mechanical factors should be researched for the pregnancy induced hypotension.
