Abstract:
Öz:Amaç. Bu çalışmada kronik hepatit B ve C hastalarında otoantikor sıklığının retrospektif olarak araştırılması amaçlandı. Yöntem. Bu çalışmaya Nisan 2007-Ağustos 2008 tarihleri arasında Hepatoloji Polikliniğine başvuran, kronik hepatit C tanısı konulan 30 hasta ve kronik hepatit B tanısı konulan 33 olmak üzere toplam 63 hasta alındı. Kronik hepatit B grubuna HBsAg'si 6 aydan daha uzun süre pozitif olan, serum alanin amino transferaz (ALT) düzeyleri normalin 2 katından daha yüksek olan ve HBV DNA'sı pozitif olan hastalar alındı. Kronik hepatit C grubuna ise anti-HCV ve HCV RNA'sı pozitif olan hastalar alındı. Çalışmaya alınan hastaların hiçbiri daha önce antiviral ilaç ya da interferon-alfa (İNF-alfa) tedavisi almamıştı. Her iki hasta grubunda dosyalarından anti nükleer antikor (ANA), romatoid faktör (RF), anti mitokondrial antikor (AMA), karaciğer-böbrek mikrozomal antikoru (Anti LKM), düz kas antikoru (SMA) ve anti double stranded DNA (anti dsDNA) düzeyleri araştırıldı. Bulgular. Kronik hepatit C grubunda 9(%30) hastada RF pozitif saptanırken, ANA 3(%10) hastada, AMA 2(%6.7) hastada pozitif saptandı. LKM, ASMA, anti dsDNA pozitifliği ise hiçbir olguda saptanmadı. Kronik hepatit B grubunda 8(%24.2 ) hastada RF pozitif saptanırken, 1(%3) hastada ANA pozitif saptandı. AMA, LKM, SMA, anti dsDNA tüm hastalarda negatif saptandı. Sonuçlar. Kronik hepatit B ve C'de otoantikorların sıklığını araştıran tüm çalışmalarda hepatit B virüsü (HBV) ve hepatit C virüsunun (HCV) antikor oluşumunu indüklediği görülmüştür. Antiviral ilaçlar ve interferon kullanımının otoantikor oluşumunu artırdığı da yapılan çalışmalar ile gösterilmiştir. Bu nedenle yeni tanı alan kronik hepatit B ve C hastalarında tedavi öncesi otoantikor bakılması ortaya çıkabilecek otoimmün olaylar ve ekstrahepatik bulgular açısından yararlı olabilir.

Öz (İngilizce):Aim. The aim of this study is to investigate retrospectively the prevelance of autoantibody positivity in patients with chronic hepatitis B and C. Methods. 63 patients (30 patients with hepatitis C and 33 patients with hepatitis B) who applied to the hepatology clinic between April-August 2008 were included in the study. HbsAg positivity for more than 6 months, elevated alanine aminotransferase (ALT) levels of at least double fold and a positive HBV DNA serology were the inclusion criteria for hepatitis B patients. Anti HCV and HCV RNA positivity were the inclusion criteria for hepatitis C patients. None of the included patients had ever received antiviral or interferon-alpha (INF-alpha) therapy. Antinuclear antibodies (ANA), rheumatoid factor (RF), antimitochondrial antibodies (AMA), liver/kidney microsomal antibody (LKM), smooth muscle antibodies (SMA) and anti double stranded DNA (anti dsDNA) levels of the both groups were investigated from their reports. Results. RF was found to be positive in 9(30%) of chronic hepatitis C patients while these numbers were 3(%10) for ANA and 2(%6.7) for AMA. None of the cases had LKM, SMA or anti dsDNA positivity. RF was positive in 8(24.2%) and ANA was positive in 1(3%)of patients with chronic hepatitis B. AMA, LKM, SMA, anti dsDNA were negative in all patients with hepatitis B. Conclusions. It was observed in all studies regarding chronic hepatitis B and C that Hepatitis B (HBV) and Hepatitis C viruses (HCV) induce autoantibody production. It was also noted in all studies that antiviral drugs and interferon therapy also affect autoantibody production. For this reason autoantibodies should be investigated in chronic hepatitis B and C patients to prevent autoimmune events and extrahepatic manifestations.
