Öz:Modern tanı, tedavi ve kontrol yöntemlerine rağmen tüberküloz halen önemli halk sağlığı sorunu olmaya devam etmektedir. Dünya nüfûsunun üçte biri tüberküloz basilleriyle infektedir. Yılda yaklaşık iki milyon kişi tüberkülozdan ölmektedir. Türkiye'de tüberküloz insidansı yıllar içinde düşüş göstermekle birlikte, ülkemiz tüberküloz insidansmın orta düzeyde olduğu ülkeler arasında yer almaktadır (insidans: 26/100,000 kişi, 2003). Yüksek tüberküloz insidansmın en önemli nedenlerinden biri anti-tüberküloz ilaçlara direnç gelişmesidir. Türkiye genelinde direnç profilini ortaya koyacak bir çalışma olmamakla birlikte, lokal çalışmalardan primer çoğul direnç oranının % 1-5, sekonder çoğul direnç oranının ise % 4-17 arasında olduğu görülmüştür. En az bir major ilaca karşı primer ve sekonder direnç oranlarının sırasıyla % 18-27 ve % 28-53 arasında değiştiği bildirilmiştir. Üçüncü "WHO/IUATID Global Project on Anti-Tuberculosis Drug " raporuna göre, dünya genelinde en az bir ilaca karşı primer ve sekonder direnç oranlarının ortalaması sırasıyla % 10.2 ve % 18.4 olarak saptanmıştır. Ortalama çoğul direnç oranları ise yeni hastalarda % 1.1, tedavi görmüş hastalarda % 7 kadardır. Bu yazıda Türkiye ve dünyada major anti-tüberküloz ilaçlara direncin sıklığı ve direncin nedenleri hakkında bilgi verilmiştir.
Abstract:İn spite of modern diagnostic, treatment and control methods tuberculosis is still continuing to be an important public health problem. One third of the world's population has been infected with tubercle bacilli and annually about 2 million persons die due to tuberculosis. Although tuberculosis incidence in Turkey is in the declining trend (incidence: 26/100,000 inhabitants, 2003), Turkey is among the countries having moderate incidence. One of the most important reasons for the high tuberculosis incidence is development of resistance to anti-tuberculosis drugs. Although a surveillance of the drug-resistance has not been performed at the national level, in local studies, initial multi-drug resistances (MDR) are in a range of 1 % to 5 % and acquired MDR in a range of 4 % to 17 %, have been reported. According to published reports, the frequencies of primary and secondary resistance to at least one drug vary from 18 % to 27 % and 28 % to 53 %, respectively. In the world, according to the third report of the WHO/IUATLD Global Project on Anti-Tuberculosis Drug, the median prevalences of primary and secondary resistance to at least one antituberculosis drug were 10.2 % and 18.4 %, respectively. The median prevalences of MDR was 1.1 % in the new patients and 7.0 % in the previously treated patients. This paper has provided information about the reasons and frequency of the resistance to major antituberculosis drugs in Turkey and the world.
