Öz:Amaç: Dermatoşalazis klinikte sık karşılaşılan bir patolojidir. Ameliyat endikasyonlarının başında kozmetik nedenler ve görme alanı üzerindeki olumsuz etkileri gelmektedir. Bu kişilerdeki kuru göz semptom sıklığı dikkat çekicidir. Çalışmamızda dermatoşalazisin göz yaşı ve oküler yüzeyde oluşturduğu değişiklikleri incelemeyi amaçladık. Gereç ve Yöntem: Oküler yüzeyi etkileyebilecek bir hastalığı olmayan dermatoşalazisi mevcut olan (grup I) ve olmayan (grup II) hastalar çalışmaya dahil edildi. Kuru göz semptom varlığı sorgulandı. Rutin oftalmolojik muayeneyi takiben blefarit varlığı kaydedildi, Schirmer I testi, gözyaşı kırılma zamanı (GKZ) ve impresyon sitoloji tekniğiyle oküler yüzey hücre incelemesi yapıldı. Tüm hastaların levator fonksiyonları ölçüldü. Veriler istatistiksel olarak karşılaştırıldı. Bulgular: Grup I'de %53.8, grup II'de ise %38.7 olguda kuru göz semptomları mevcuttu. Yine grup I'de 28 gözde blefarit saptanırken, Grup II'de 10 gözde mevcuttu. Schirmer I testi sonuçları ortalama grup I ve II'de sırasıyla 10.37±5.46mm ve 8.87±5.22mm iken GKZ değerleri 6.32±3.09 sn ve 6.42±5.55 sn idi. İmpresyon sitolojisinde grup I'de üst kadranda alt kadranlara göre skuamöz metaplazi derecesinde belirgin artış izlendi (p=0.00). Dermatoşalazisin derecesi arttıkça, alt kadran oküler yüzeyde de skuamöz metaplazinin anlamlı bir şekilde arttığı görüldü. Sonuç: Dermatoşalazis oküler yüzey değişikliklerine ve kuru göz semptomlarının artmasına neden olabilmektedir. Sadece basit bir kozmetik problem olmayıp, tedavi gerektiren bir patolojidir.
Abstract:Purpose: Dermatochalasis is a frequently encountered clinical pathology. The main surgical indications are cosmetic problems and the detrimental effects on the visual field. The frequency of dry eye symptoms in these patients is worthy of attention. In our study we aimed to investigate the effects of dermatochalasis on tear formation and the ocular surface. Material and Method: Patients having no disease which may affect ocular surface were separated into group I with, and group II without dermatochalasis. They were asked about symptoms of dry eye. Following routine examination the presence of blepharitis was recorded. The Schirmer I test, tear break up time (BUT) and cytologic evaluation of the ocular surface by the impression cytology technique were performed. The levator function was measured in all patients. The data obtained was compared statistically. Results: Dry eye symptoms were present in 53.8% of patients in group I and in 38.7% in group II. Blepharitis was found in 28 eyes in group I and in 10 in group II. The average Schirmer test results in groups I and II were 10.37±5.46mm and 8.87±5.22mm, while the BUT values were 6.32±3.09sec and 6.42±5.55 sec, respectively. Comparision of the impression cytology results revealed a striking increase in the degree of squamous metaplasia in the upper quadrant of group I than the lower quadrant (p=0.00). It was demonstrated that squamous metaplasia of the lower quadrant of the ocular surface increased significantly as the degree of dermatochalasia increased. Conclusion: Dermatochalasia may lead to an increase in ocular surface changes and dry eye symptoms. It is therefore not simply a cosmetic problem but a pathology which must be treated.
