Öğe ABO and Rh blood groups frequency in men, women and neonates in Diyarbakir province(2019) Arac, Esref; Solmaz, Ihsan; Samanci, SerhatAim: In this study, our objective was to determine the distribution of the blood groups among females, males, and neonates along with the overall analysis of Rh and ABO blood group systems in Diyarbakır region and contribute particularly to the blood group database for neonates, as there is no other study focused on the blood groups in neonates in particular. Material and Methods: The data of 127,091 patients, who had applied for the determination of their blood groups or were hospitalized and needed the determination of their blood groups between January 2016 and July 2019, were analyzed in this retrospective study. Results: In our study, the distribution of subgroups of the ABO system without considering the Rh system was as follows: 39.69% A, 33.62% O, 18.63% B, and 8.06% AB. The same rates were 36.79% A, 34.6% O, 20.77% B, and 7.84% AB among neonates. The comparison of the general population (females and males) with the neonates showed that group A was 3 percent lower and group B was 2 percent higher among neonates. We believe our study is important, as it is the only single-center study focused on three groups (females, males, and neonates) and conducted in the Research and Training Hospital in Diyarbakır.Conclusion: We believe that the differentiation in the distribution of ABO blood subgroups among neonates will also contribute to the literature as an important finding, which may depend on the social events like the increased number of Syrian refugees and domestic migrations.Öğe Acute renal injury due to rhabdomyolysis: A tertiary hospital experience(2021) Arac, Esref; Solmaz, Ihsan; Yuksel, Enver; Ekin, Nazim; Arac, SongulAim: Rhabdomyolysis (RML) is a clinical and biochemical syndrome caused by acute necrosis of striated muscle fibers and the subsequent transportation of potentially toxic cellular contents into the systemic circulation. In this study, we present data from rhabdomyolysis patients that developed acute renal failure who were hospitalized and treated in our clinic in 2019. Materials and Methods: We reviewed all cases of RML treated in the tertiary hospital within a 12-month period, which was defined as serum creatinine kinase (CK) levels > five times the upper normal limit. The inclusion criteria were as follow: (a) being aged ≥ 18 years, (b) being treated for RML within the designated time period, (c) AKI development. The exclusion criteria are as follows: (a) being aged 5 time upper normal limit (UNL), 14 conformed to the inclusion criteria and were selected as subjects Results: A total of 14 patients were included in the study, 12 of which were male. The mean age of the patients was 48.1 (18–80). The etiologies were as follows: hypothyroidism, 3; prolonged exposure to sun, 2; electrolyte imbalance due to severe diarrhea, 1; viral upper respiratory tract infection (URTI), 3; intramuscular injection, 1; heavy exercise or falls.Conclusion: Rhabdomyolysis is an interdisciplinary clinical condition that can lead to life-threatening outcomes including AKI. Rapid diagnosis and treatment can be life-saving. AKI is a significant potential complication of RML and renal function should be evaluated irrespective of CK levels or the presence of myoglobinuria.Öğe A case of fascioliasis mimicking malignancy(2019) Oruc, Idris; Oruc, Zeynep; Arac, Esref; Kaplan, Muhammet AliFascioliasis, initially considered as malignancy, can lead to unnecessary invasive procedures. A 69-year-old man presented with hepatic masses. Positron emission tomography (PET/CT), upper gastrointestinal endoscopy and colonoscopy performed to detect the primary tumour showed no lesion, except in the liver. The patient had eosinophilia and a history of raising livestock in an endemic area. The result of the indirect hemagglutination test for Fasciolia hepatica was 1/320. Serologic evaluation and dynamic computed tomography confirmed the diagnosis. Occassionally, it may be difficult to differentiate fascioliasis from malignancy. Fascioliasis should be considered in the differential diagnosis of patients with suspicious masses in the liver, especially in those living in endemic areas,prior to conducting invasive procedures.br / Fasciola hepatica, fascioliasis, malignancy.Öğe Impact of vitamin D supplementation on the ocular surface in patients with vitamin d deficiency(2020) Alakus, Mehmet Fuat; Oncul, Hasan; Yilmaz Oncul, Fatma; Dag, Umut; Arac, EsrefAim: The aim of this study was to determine the effect of vitamin D (vit D) supplementation on the ocular surface parameters in patients with vit D deficiency.Material and Methods: In total, 63 patients with serum vit D deficiency were included. Patients were treated with an oral dose of 300.000 IU cholecalciferol. The ocular surface disease index (OSDI) questionnaire, fluorescein tears breakup time (FBUT), Oxford score, and Schirmer’s 1 test were performed for all patients. The data was obtained initially and at 4, 8, and 12 weeks after vit D supplementation.Results: Forty-nine of the patients were female and 14 were male. The mean age was 30.8±9.7 (18–58) years. All 63 patients came to the first follow-up, 57 patients came to the second follow-up, and 48 patients came to the third follow-up. The OSDI and Oxford scores yielded statistically significant decreases, while Schirmer’s 1 test and FBUT values showed a statistically significant increase after treatment with vit D. A positive correlation was observed between vit D values and FBUT and Schirmer’s 1 values; there was a negative correlation between OSDI values (r=0.286 and p0.001, r=0.219 and p=0.032, r = -0.357 and p0.001, respectively). The dry eye findings improved significantly after vit D supplementation in all cases. Conclusion: During clinical follow-up, it is important to evaluate serum vit D levels in patients with dry eye syndrome (DES). Increasing their vit D levels will help clinicians to treat DES quickly and effectively.Öğe Is diabetic dermopathy related with microangiopathic complications?(2019) Ayhan, Erhan; Arac, EsrefAim: Diabetic dermopathy is a skin disease, seen in patients with diabetes mellitus, characterized by atrophic scar and hyper-pigmented lesions. Recently, it was proposed that this disease might be associated with other microangiopathic complications of diabetes such as retinopathy, nephropathy, and polyneuropathy. In this study, patients with diabetic dermopathy were compared with two control groups to test the validity of these associations.Material and Methods: Twenty-three patients, who admitted to Dermatology Outpatient Clinic Health Sciences University Gazi Yaşargil Education and Research Hospital and diagnosed as diabetic dermopathy were included in the study. Twenty-three patients with well controlled diabetes mellitus (Control Group A) who treated as outpatients and 23 patients with diabetes mellitus who had poor general status and hospitalized by internal medicine department (Control Group B) were included as two control groups. Patients were evaluated in terms of retinopathy, nephropathy, neuropathy, heart attack history and diabetic foot ulcer.Results: In the study 82.6% (n:19) of patients were male and 17.4% (n:4) were female (female: male ratio 4.75:1). There was no significant relationship between fasting blood glucose and HbA1c elevation with retinopathy, neuropathy, polyneuropathy and diabetic foot. However, there was a significant relationship between fasting blood glucose levels and heart attack. Although heart attack history and diabetic foot was more common in the patient group and retinopathy, nephropathy and polyneuropathy was more frequent in the Control Group B, there was no significant difference between both groups.Conclusions: In conclusion, microangiopathic complications, heart attack and diabetic foot are not only common in the patient group with diabetic dermopathy but also in the Control Group B. Even though these complications were more common in these two groups, there was no significant difference between each and the other diabetic (Control Group A).Keywords: Diabetes mellitus; diabetic dermopathy; skin lesions; cutaneous manifestations.Öğe Measuring the knowledge and attitudes of physicians towards patients with HIV/AIDS: study of Anatolian group(Emerald Group Publishing Ltd, 2024) Kaya, Safak; Arac, Esref; Akgul, Fethiye; Comoglu, Senol; Kaya, Sehmuz; Arac, Songul; Yildiz, YesimPurpose This paper aims to determine the knowledge and attitudes of the physicians regarding human immunodeficiency virus (HIV)/acquired immune deficiency syndrome (AIDS), to emphasize that these patients exist and they will exist in the future and to raise awareness so as to prevent that their rights to treatment are revoked. Design/methodology/approach The survey was conducted via a link sent through an online system. Random physicians from 81 cities of the country were invited to the survey. The survey has 41 questions regarding knowledge and attitudes in total, including epidemiological information such as age, gender and title. Findings A total of 3,107 physicians has voluntarily participated in the study. In total, 2,195 (70.7%) are internal physicians and 912 (29.3%) are surgical physicians among the participant physicians. In total, 1,452 (46.7%) of the participants are specialist physicians, 608 (19.6%) of the participants are practising physician and the rest of it is physician assistants, academicians and dentists, respectively. Originality/value In this study, it has been found out that the physicians have a lack of knowledge on HIV/AIDS and they adopt a discriminatory attitude towards HIV-positive persons. HIV-positive patients who are exposed to discrimination and scared of being uncovered refrain from applying to hospitals for treatment, which puts public health into jeopardy due to the high viral load and these patients are faced with difficulties in coping with both medical and emotional load of the disease.
