    The effect of Hypericum Perforatum on wound healing of oral mucosa in diabetic rats

    Purpose This study aims to investigate the histopathological and biochemical effects of the topical application of Hypericum perforatum on the healing of surgical wounds created in the oral mucosa of rats with experimentally induced diabetes mellitus. Materials and methods The study was carried out on 48 adult male Wistar albino rats. The animals were divided into two main groups as control and study groups. Two main groups were divided into three subgroups according to the sacrification days. All rats were given streptozotocin 60 mg/kg, after 72 hours, and those having blood glucose levels above 200 mg/dL were included in the study. Mucosal defects were created in the palatal area of the rats. H. perforatum oil was applied topically twice a day to the wounds of the rats in the study group. Animal were sacrificed on the 3rd, 7th and 10th days and samples taken from the palatal wounds were examined histologically and biochemically. Results On the 7th day, ulceration, necrosis, epithelialization, polymorphonuclear leucocytes and hydroxyproline variables showed statistically significant differences (p<0.05). Ulceration, necrosis and polymorphonuclear leukocytes values were higher in the control group, whereas epithelialization and hydroxyproline values were found to be higher in the H. perforatum group. Among 10th-day groups, there was only a statistically significant difference between the values of hydroxyproline, whereas H. perforatum-treated group showed high hydroxyproline levels (p<0.05). Conclusion Topically applied H. perforatum did not create any difference on the 3rd day, but it has positively affected the wound healing on the 7th and 10th days in diabetic rats.

    Effects of laser irradiation at different wavelengths (660, 810, 980, and 1064 nm) on transient receptor potential melastatin channels in an animal model of wound healing

    The aim of the present study was to compare the effectiveness of four different laser wavelengths used for low-level laser therapy(LLLT) on healing of mucositis in an animal model of wound healing, by investigating expression of transient receptor potential melastatin(TRPM) ion channels. Forty-five rats were intraperitoneally injected with 100 mg/kg 5-fluorouracil on day 1 and 65 mg/kg on day 3. Superficial scratching on left cheek pouch mucosa was performed on days 3 and 5. After ulcerative mucositis was clinically detected, LLLT was started (660 nm, HELBO; 810 nm, Fotona-XD; 980 nm, ARC-Fox; and 1064 nm, Fidelis-Plus3) at 8 J/cm(2)/day from days 1 to 4. Oval excisional biopsy was performed at the wound site, and expression of TRPM2 to TRPM8 was evaluated. Student's t test was used for evaluation of significance of TRPM gene expression according to 0 value (alpha = 0.05). In 980-nm group, TRPM4, TRPM6, and TRPM7 were significantly higher than in the control group (p < 0.005). In 660, 810, and 1064 nm groups, only TRPM6 was significantly higher than in control group (p < 0.005). There were no significant differences between control and sham groups (p > 0.05). These findings suggest that expression of TRPM6 gene was significantly affected by irradiation with lasers at different wavelengths, whereas the TRPM4 and TRPM7 genes were only expressed in the 980-nm diode laser group. TRPM6 gene was highly expressed during LLLT, which may lead to accelerated wound healing and tissue repair. In contrast, there was some evidence that the 980-nm diode laser caused increased expression of TRPM4, TRPM6, and TRPM7 which are responsible for stimulation of Ca2+ and Mg2+ metabolism, as well as apoptotic pathways of controlled cell death.

    Effects of Low-Level Laser Therapy as an Adjunct to Standard Therapy in Acute Pericoronitis, and its Impact on Oral Health-Related Quality of Life

    Objective: The purpose of this study was to evaluate the effect of low-level laser therapy (LLLT) as an adjunct to standard therapy in acute pericoronitis. Methods: Eighty acute pericoronitis patients were randomly assigned to one of four LLLT groups: (neodymium: yttrium-aluminum garnet [Nd:YAG] 1064-nm: n = 20, 8 J/cm(2), 0.25W, 10 Hz, 10 sec; 808-nm diode: n = 20, 8 J/cm(2), 0.25W, continuous mode, 10 sec; 660-nm diode: n = 20, 8 J/cm(2), 0.04 W, continuous mode, 60 sec; or a placebo laser control group: n = 20). After standard treatment, LLLT or a placebo laser were applied to the treatment area at a distance of 1 cm from the buccal site. Interincisal opening, pain perception, and oral health-related quality of life (OHRQoL) were evaluated at baseline, 24 h, and 7 days after laser application. The data were analyzed by the one-way ANOVA test. Results: We found that the trismus and the OHRQoL in the Nd: YAG and the 808-nm diode groups were significantly improved when compared with the 660-nm diode and control groups at 24 h (p < 0.05). No statistically significant differences were detected on day 7 among the groups with regard to any of the parameters evaluated. Conclusions: The results demonstrate that both the 1064-nm Nd: YAG and the 808-nm diode lasers were effective in improving trismus and OHRQoL in acute pericoronitis. Taking into account the limitations of this study, we conclude that the 1064-nm Nd: YAG laser has biostimulatory effects and improves OHRQoL, making it suitable for LLLT.

    Histomorphometric analysis of the effects of grape seed extract (vitis vinifera) and low-level laser therapy (LLLT) on fracture healing

    Aim: In this study, we aim to investigate the effects of supplementary Grape Seed Extract (GSE) and Low-Level Laser Therapy (LLLT) on fracture healing, oxidant and anti-oxidant system in experimental mandible fractures.Material and Methods: 48 Wistar Albino rats (adult male, n=48) were used in our study. For all the subjects, a vertical fracture line through molar teeth in right mandibles was created and internally fixed using a four-hole microplate and four micro screws. Firstly, these subjects were randomly divided into 4 main groups (Control, GSE, LLLT, GSE+LLLLT) of 12 animals each, and then these were individually separated into two sub-groups of 7th and 21st days. The number of groups (n=6) was 8 in total. GSE of 300 mg/kg/day were provided to the subjects before they were sacrificed. LLLT of 23 J/cm2 was administered to two different points along with the fracture line at intervals of 48 hours for 7 days in the 7th day sub-groups and for 14 days in the 21st day sub-groups. After the procedure, while biochemical values such as TAS, TOS and OSI are measured; histopathologically it was examined in terms of capillary number and width, inflammatory cell, fibroblast count, collagen fibers, osteoblast count, ossification and mature bone formations.Results: For biochemical analyses, there was statistically significant difference only in TAS values on either the 7th or 21st day for the groups. Histological analyses showed that mandibular fracture healing were significantly better in the GSE and GSE+LLLT groups compared to the control group. The group of only LLLT had limited recovery while the combination of GSE+LLLT was the best for ossification.Conclusions: It is concluded that GSE may be one of the potential methods to accelerate fracture healings in mandibular fractures common in oral and maxillofacial surgery clinic and thus help patients recover in shorter time, and however LLLT can have positive effects on the process of ossification and recovery only when combined with the extract, which may result in better outcomes in clinical use.

    Histopathological Effects of Teriparatide in Medication-Related Osteonecrosis of the Jaw: An Animal Study

    Purpose: Osteonecrosis of the jaw after tooth extraction is a major complication in patients using bisphosphonates (BPs) for bone lesions, such as for the treatment of osteoporosis. The purpose of this study was to investigate the histopathologic effects of teriparatide (a synthetic parathyroid hormone) on rats developing osteonecrosis with BP use. Materials and Methods: Rats (n = 80) that had been injected intraperitoneally with zoledronic acid for 7 weeks were used. Maxillary first molar extractions and bone defects were established in the same region in the eighth week. Teriparatidewas administered subcutaneously to prevent osteonecrosis. Animals were sacrificed and histopathologic changes were examined. Osteoblasts, osteoclasts, inflammatory phase of bone healing, and osteonecrotic areas were evaluated. Results: The osteoclast numbers were larger in the experimental groups (teriparatide administered before and immediately after tooth extraction) than in the control group (administered zoledronic acid). The inflammatory phase of bone healing was more pronounced in the experimental group (teriparatide administered before tooth extraction) than in the control group. There were significant differences in osteoclast numbers and in the inflammatory phase of bone healing between the experimental and control groups (P < .05). The osteoblast numbers and osteonecrotic areas were similar in size between the experimental and control groups. There were no significant differences (P > .05). Conclusions: BPs have negative effects on osteoclasts and the inflammatory phase of bone healing, whereas teriparatide was found to be effective in eliminating the negative effects of BPs. Teriparatide had positive effects in preventing osteonecrosis; therefore, teriparatide could be an effective agent for MRONJ. (C) 2016 American Association of Oral and Maxillofacial Surgeons
