Öğe Arthroscopic limited intercarpal fusion without bone graft in patients with Kienbock's disease(Turkish Joint Diseases Foundation, 2016) Ertem, Kadir; Gormeli, Gokay; Karakaplan, Mustafa; Aslanturk, Okan; Karakoc, YunusObjectives: This study aims to present our clinical results obtained in arthroscopic limited intercarpal fusion performed without using bone graft in patients with Kienbock's disease. Patients and methods: The study included 11 patients with Kienbock's disease (6 males, 5 females; mean age 28.9 years; range 14 to 51 years) who were performed arthroscopic lunate excision and scaphocapitate fusion between November 2012 and December 2013. Bain and Begg Arthroscopic Classification was used for the staging of Kienbock's disease. Quick Disabilities of Arm, Shoulder and Hand and Mayo Wrist scorings were used for clinical evaluation. Results: Intercarpal fusion was achieved in approximately 7.2 weeks. There was a statistically significant difference in Mayo Wrist scores of postoperative third and sixth months and pre- and postoperative Quick Disabilities of Arm, Shoulder and Hand scores. There was no postoperative complication. Conclusion: According to our study findings, arthroscopic limited intercarpal fusion without using bone graft may be performed in patients with Kienbock's disease. Satisfactory clinical and functional results were obtained as a result of treatment with this method in stage 3 and 4 Kienbock's disease.Öğe Bilateral thigh abscess due to rectal fistula: A rare case report with silent clinical presentation(2023) Koroglu, Muhammed; Dalda, Yasin; Ozdes, Huseyin Utku; Ozdemir, Zeynep Maras; Aslanturk, OkanA 53-year-old male patient who underwent a rectal cancer operation nine years ago was evaluated with complaints of pain and swelling in the lateral thigh. Radiological imaging revealed abscess foci in the right thigh muscles. Surgical drainage and debridement were performed but developed recurrence. Contrast-enhanced abdominal tomography was performed due to a positive medical history and it was seen that an enterocutaneous fistula was formed from the anastomosis site of previous cancer surgery to both sides of the thigh. Bilateral thigh abscesses were drained and radical debridement was performed in the same session with surgery for the fistula. Pathological examination of the surgical specimens showed no evidence of cancer and the patient was discharged on the 14th postoperative day with complete resolution of the infection. Hip abscesses are rare, delays in the diagnosis and treatment of the underlying cause in cases of abscess secondary to abdominal or pelvic pathologies lead to an increase in mortality. Therefore, it should be kept in mind that thigh abscesses may be another side of the coin besides being seen as an isolated infection.Öğe Cerebrotendinous Xanthomatosis patients with late diagnosed in single orthopedic clinic: two novel variants in the CYP27A1 gene(Bmc, 2024) Koroglu, Muhammed; Karakaplan, Mustafa; Gunduz, Enes; Kesriklioglu, Betul; Ergen, Emre; Aslanturk, Okan; Ozdemir, Zeynep MarasBackground Cerebrotendinous Xanthomatosis (CTX) is a rare autosomal recessive lipid storage disorder caused by loss of function variants in the CYP27A1 gene which encodes sterol 27-hydroxylase, on chromosome 2q35. Although the symptoms begin commonly in infancy, CTX diagnosis is often delayed. The aim of this study is to review the orthopedic findings of the disease by providing an overview of the clinical features of the disease. It is to raise awareness of this condition for which early diagnosis and treatment are important. Methods We retrospectively evaluated the clinical, laboratory, radiological, and genetic findings of eight patients from four families who were admitted to our Orthopedics and Traumatology Department between 2017 and 2022 due to bilateral Achilles tendon xanthomas, were found to have high cholestanol and CYP27A1 gene mutations. Results The mean age of patients was 37, and five of them were male. The mean age at the onset of symptoms was 9.25 years. The mean age of initial diagnosis was 33.75 years. Between symptom onset and clinical diagnosis, an average delay of 24.5 years was observed. All patients had bilateral Achilles tendon xanthoma. Notably, a novel variant (c.670_671delAA) in CYP27A1 gene was identified in three patients who also presented with peripheral neuropathy and bilateral pes cavus. One patient had osteoporosis and four patients had osteopenia. Five patients had a history of bilateral cataracts. Furthermore, three of the patients had early-onset chronic diarrhea and three of the patients had ataxia. Two of the patients had epilepsy and seven of the patients had behavior-personality disorder. All patients had low intelligence, but none of them had cardiac disease. Conclusion We present the diagnostic process and clinical features which the largest CTX case series ever reported from single orthopedic clinic. We suggest that patients with normal cholesterol levels presenting with xanthoma being genetically analyzed by testing at their serum cholestanol level, and that all siblings of patients diagnosed with CTX be examined.Öğe Clinical results of anterior cruciate ligament reconstruction with bone-patellar tendon-bone autograft: A prospective study(2018) Aslanturk, Okan; Ergen, Emre; Gormeli, Ayse Cemile; Gormeli, Gokay; Yilmaz, OzgurAim: Anterior cruciate ligament(ACL) injury is the most common ligament injury of knee. Gold standard treatment of ACL rupture is reconstruction of ligament with autograft or allograft. There is still controversy in graft choice in literature. In this study, we aimed to present prospective follow-up results of patients who were reconstructed with bone-patellar tendon-bone autograft for ACL rupture. Material and Methods: Twenty-seven patients were operated for ACL rupture with bone-patellar tendon-bone(B-PT-B) autograft ACL reconstruction. Patients were followed for at least 2 years. Patients were evaluated preoperatively and postoperatively with Lysholm knee function score, IKDC objective score and Tegner activity level scale. Development of arthritis were evaluated radiologically. Results: All of operated patients were male. The mean age of the patients was 25.4 (range 19-35 years). The mean follow-up time was 27.9 (range 24-30 months). There was a statistically significant increase in all postoperative knee scores. Hematoma, requiring surgical drainage, developed in two patients. Six patients had anterior knee pain while kneeling at final follow up. Conclusion: ACL rupture causes knee instability that can lead early knee arthritis and meniscal injuries. Patients with ACL rupture showed increased knee stability and functional scores after reconstruction. Anterior cruciate ligament reconstruction with B-PT-B autograft incorporates and heals faster with low rerupture and complication rate.Öğe Clinical results of surgical neurectomy of Morton’s neuroma(2024) Koroglu, Muhammed; Karakaplan, Mustafa; Turkmen, Ersen; Ozdes, Huseyin Utku; Acet, Omer; Ergen, Emre; Aslanturk, OkanAim: Morton’s neuroma (MN), specifically with its forefoot placement, sometimes significantly impacting patients’ quality of life. The aim of this study is to present the outcomes of neurectomy surgeries performed for MN disease. Investigating the impact of this surgery, which has a complication such as hypoesthesia, on postoperative patient satisfaction, is the targeted objective. Materials and Methods: Seventeen masses of 16 patients were studied. In patients diagnosed with MN mass through physical examination and imaging techniques, a 3 cm incision was made to excise the nerve mass either dorsally or plantar under spinal anesthesia. During routine outpatient follow-ups, the American Orthopedic Foot and Ankle Society (AOFAS) score, Visual Analog Scale (VAS) score, and Coughlin’s criteria were assessed both preoperatively and postoperatively. The effects of conservative treatments such as preoperative orthoses and steroid injections on pain scores were recorded and evaluated. Recurrence of mass development and wound issues were investigated to evaluate the outcomes. Results: The mean age at the time of surgery was 46.75 years (26-58 years). The mean follow-up period was 23.12 (6-75 weeks) weeks. After surgery, excellent results were achieved in 9 patients (64.3%), and good results were obtained in 4 patients (28.6%). Recurrent mass development occurred in two patients (12.5%). Two patients (12.5%) were operated on with a plantar approach, and one patient with recurrent massbelonged to this group. Hypoesthesia was described in 15 out of 16 patients. There were significant improvements between patients’ preoperative VAS and AOFAS scores and postoperative VAS and AOFAS scores, which were statistically significant (p <0.001). Conclusion: Numerous studies have been conducted on the diagnosis and treatment of MN disease, yet a common procedure has not been developed. Among existing treatments, excision of the mass with some healthy nerve tissue and a dorsal approach to the foot are commonly employed methods. Dorsal neurectomy surgeries demonstrate a success rate exceeding 80%. Informing patients about the possibility hypoesthesia before surgery and establishing realistic expectations can make this procedure safely applicable.Öğe Comparison of medium-term revision rates after autograft and allograft anterior cruciate ligament reconstruction(Edizioni Luigi Pozzi, 2020) Sevimli, Resit; Gormeli, Gokay; Polat, Haci; Kilinc, Oner; Turkmen, Ersen; Aslanturk, OkanAim: The present study investigates time until revision and revision rates, and their relationship with gender and age, among 267 patients with anterior cruciate ligament tears treated with autograft and allograft reconstructions. MATERIALS AND METHODS: A retrospective evaluation was carried out based on data collected on 269 knees (bone-patellar tendon-bone autograft in 25, gracilis-semitendinosus autograft in 136, quadriceps autograft in two, allograft in 106) belonging to 267 patients who underwent an anterior cruciate ligament reconstruction between 2009 and 2018. RESULTS: Of the 269 knees of the 267 patients (22 women and 247 men) operated on for an anterior cruciate ligament rupture, an autograft was used in 163, and nine of those required revision, while an allograft was used in 106 knees, and seven required revision. Revision surgery was necessary for six out of the 22 female patients and for only 10 out of the 247 male patients (p<0.001). CONCLUSION: Each type of graft used for treatment is associated with certain advantages and disadvantages. Hamstring autografts and allografts were the most commonly used grafts during the anterior cruciate ligament reconstruction surgeries carried out at our clinics. The rate of re-rupture was quite low with use of both graft types, leading us to believe that the type of graft preferred for anterior cruciate ligament reconstruction surgery should be based on a common decision of the surgeon and patient.Öğe Complications after the percutaneous release of trigger thumb in adults(2020) Aslanturk, Okan; Yilmaz, Ozgur; Ari, Bunyamin; Canbay, Ali; Kavak, HüseyinAim: Trigger finger is a common orthopedic problem causing pain and could restrsict daily activities. Surgical intervention can be done via open or percutaneously. There is not much data about minor complications in literature. In this study, we reported our results and complications of percutaneous release (PR) of trigger thumb.Material and Methods: Retrospective data of patients treated for trigger thumb with PR between September 2017 and January 2019 were reviewed. Age, gender, affected side, preoperative Quinnell grade of triggering; previous history of steroid injection and history of medical diseases (e.g. diabetes mellitus, rheumatoid arthritis) were recorded.Results: Thirty thumb of 28 patients who met the inclusion criteria treated with PR for trigger thumb included in the study. Mean age of patients was 53.7 ±9.95 months (range 36-73). Twenty (71%) of patients were female, 8 (29%) were male. Right hand was dominant side in all patients. Right thumb was affected in 18 (64%) patients, left thumb in 8 (29%) and 2 (7%) were bilateral. Due to Quinnell grading system 9 (32%) patients were grade 2 and 19 (68%) were grade 3. Twenty (71%) of patients had previous history of steroid injection. There were 5 complications in our patients. One recurrence of triggering and 4 ecchymosis and edema have been reported.Conclusion: Percutaneous release of trigger thumb is safe and reliable technique with low complication rate. To prevent complication surgeon should be careful during the procedure and should well inform patient what to do after procedure.Öğe Does the COVID-19 pandemic cause late diagnosis and delay in treatment in developmental dysplasia of hip patients?(Wiley, 2021) Mert Dogan, Gulec; Aslanturk, OkanBackground The aim of the current study was to search the effect of COVID-19 restriction on developmental dysplasia of hip (DDH) screening. Material and Methods We retrospectively reviewed the patients who brought to DDH screening in April-May and June-July 2020 and compared with the same period of 2019. We recorded age, gender, DDH type and risk factors of the patients. Results The number of patients taken for DDH screening was 430 and 400 in April-May 2019 and June-July 2019, respectively. In 2020, the number of patients taken for DDH screening was 159 and 776 in the same period, respectively. Thirteen patients were diagnosed with DDH older than 3 months age in June-July 2020. There were only two patients in same period in 2019. In 2020, 6 of 13 patients who applied to the hospital late for the routine US and had pathologic hips were not taken to orthopaedics or follow-up by their families. Conclusion In the era of COVID-19, the number of late diagnosis and lost follow-up for DDH are increased. Carpal height ratio was statistically significantly reduced compared with preoperative values. All patients declared their satisfaction with the results due to their preoperative status. Fractures are usually missed initially. Mechanism of injury is usually reported as hyperextension and less frequently as direct compression of metacarpophalangeal joint. Fracture of the hand sesamoid bones has been usually reported in the thumb. We report a case of fifth-digit sesamoid fracture, treated conservatively. In 6 months of followup, the patient had a full range of motion without pain.Öğe Hair tourniquet syndrome of toe(Elsevier, 2023) Koroglu, Muhammed; Ozdes, Huseyin Utku; Ozbey, Rafet; Yilmaz, Ozgur; Ergen, Emre; Oklu, Yunus; Aslanturk, OkanIntroduction: Hair tourniquet syndrome (HTS) is a rare surgical emergency caused by a hair or thread wrapping around an appendage. We aimed to present our clinical experience with HTS of toes and attract physicians' attention to this rare entity.Methods: Between January 2012 and September 2022; 26 patients (25 pediatric and one adult case) were treated for HTS. All pediatric cases were treated surgically under loop magnification. The adult patient was treated nonsurgically. The patient's age, gender, affected appendage and side, duration of symptoms, and postoperative complications were recorded.Results: Thirty-six toes of 25 patients (13 boys, 11 girls, and a male adult) were included in the study. The mean age of pediatric patients was 126.6 days. The third toe was the most affected (n:16), followed by the fourth (n:8). In seven patients more than one was affected.Conclusion: HTS should be treated as soon as possible when diagnosed to prevent further complications including appendage loss.& COPY; 2023 European Foot and Ankle Society. All rights reserved.Öğe Hair tourniquet syndrome of toes and fingers in infants(Turkish Assoc Orthopaedics Traumatology, 2019) Aslanturk, Okan; Ozbey, Rafet; Yilmaz, Ozgur; Ergen, EmreObjective: Hair tourniquet syndrome is an uncommon condition characterized by strangulation appendages by a hair or thread. The aim of this study was to evaluate the results of hair removal and antibiotic therapy in our patients with hair tourniquet syndrome. Methods: Between January 2012 and August 2018, 16 patients (8 boys, 8 girls; mean age: 118.5 [range: 20 to 380] days) were treated surgically for hair tourniquet syndrome. All patients were treated surgically under local or general anesthesia in the pediatric emergency department or in the operating theater using magnifying loupes. The age, gender, affected fingers or toes and the affected sides of the patients and the duration of symptoms until presentation were recorded. Results: A total of 24 toes and fingers were treated for hair tourniquet syndrome. The right side was affected in 12 patients, the left side was affected in three, and both sides in one. The second toe was affected in three patients, the third toe in eleven, the fourth in six, and the fifth in two patients. Both the thumb and the second finger were affected in one patient. The average duration of the symptoms (excessive crying, swelling, redness) was 1.5 (range: 1 to 2) days. All patients healed without any complications. Conclusion: Hair tourniquet syndrome should be kept in mind as an etiology in infants with toe and finger strangulation. These patients should be examined undressed. Immediate removal of hair is an effective treatment method to save appendage. (C) 2019 Turkish Association of Orthopaedics and Traumatology. Although musculoskeletal tuberculosis generally affects spine and large joints (hip and knee), hand involvement of the tuberculosis is seen in 10% of the patients with musculoskeletal involvement and also isolated tuberculosis of hand or wrist is much rarer. In the following report, we discuss the case of a 23-year-old male patient who was diagnosed with isolated tuberculosis of the capitate and triquetrum bone. The patient presented with a nonhealing sinus on the left wrist joint. Imaging revealed lytic lesions of the capitate and triquetrum. The diagnosis of tuberculosis was confirmed by histopathological examination on the bone specimen obtained from the debridement and curettage. Antituberculosis therapy was started postoperatively and 1month later, healing of the sinus was observed. Methods A total of 22 patients (six females, 16 males; mean age: 36 years, range: 18 to 56 years) who underwent an arthroscopic intervention for traumatic or non-traumatic stiff elbow and arthrosis between January 2005 and November 2015 were included in the study. All patients started elbow movement after the first day following surgery. Pre- and postoperative radiological evaluations of patients were carried out, in addition to functional evaluation to measure the range of motion of the elbow joint and the Disabilities of the Arm, Shoulder and Hand (DASH) scores before and after surgery. Results The mean follow-up was 28.4 (range: 21 to 118) months. The mean preoperative flexion-extension arc of the patients was 89 degrees (range: 0 degrees to 115 degrees), and the mean flexion-extension arc increased to 103.5 degrees (range: 52 degrees to 128 degrees) at the final follow-up visit (p < 0.05). The mean preoperative DASH score was 42.17 (range: 33 to 81), decreasing to 30.35 (range: 9.7 to 41.3) postoperatively (p<0.05). In the final visit, none of the patients were found to require additional surgical interventions for the elbow.
