    Comparison of the Effects of Local and Systemic Zoledronic Acid Application on Mandibular Distraction Osteogenesis

    Bisphosphonates are antibone resorptive drugs that are used to prevent bone tissue resorption in several skeletal diseases. The aim of this study was to examine the effects of systemic and local applications of zoledronic acid (ZA) on newly regenerated bone in a model of experimental distraction osteogenesis (DO). To do this mandibular DO was applied to 30 adult female Sprague Dawley rats, which were randomly divided into 3 groups: control, DO only, systemic zoledronic acid (SZA), and local zoledronic acid (LZA). In the LZA group, the gap between the bone fragments was filled with a gelatin sponge soaked in 2mg of ZA and 0.1 mL of sterile saline. In the SZA group, a single dose of 0.1 mg/kg ZA was administered systemically. After the surgery, there was a 5-day latent waiting period and 10-day distraction phase. Following a 28-day consolidation period, the rats were euthanized and theirmandibles were collected. The distracted bone area was seen to be filled with newly regenerated bone tissue in all 3 groups, both histologically and histomorphometrically. In addition, amounts of new bone formation, osteoblast cella, osteoclast (OC) cells, osteopontin, and vascular endothelial growth factor in the SZA and LZA groups were found to be higher when compared with the controls. Furthermore, in the SZA group, new bone formation, osteoblast, OC, osteopontin, and vascular endothelial growth factor were detected in significant amounts compared with the LZA group. Osteoclast numbers did not differ in a statistically significant manner in the SZA group with respect to the LZA group. Based on the results of this study, systemic and local applications of ZA could increase the formation of new bone in patients of DO, and systemic application is a more effective method compared with local application.

    DENTAL STEM CELL SOURCES AND THEIR POTENTIALS FOR BONE TISSUE ENGINEERING

    Tissue engineering arouses excitement in all medical fields that deal with bone healing. The ultimate aim of these approaches are to shorten the healing process, obtain highly differentiated functional tissues and eliminate the need for a second surgical site required for autogenous bone grafts. Mesenchymal stem cells have been increasingly used in the experiments which were conducted in these fields and the results are promising. Dental stem cells have come to the forefront both because of their relative ease of access and also their superior characteristics. This article investigates the importance of dental stem cells for bone tissue engineering and their regeneration potentials.

Mezenşimal kök hücreler bu alandaki çalışmalarda giderek artarak kullanılmakta ve sonuçlar umut vermektedir. Dental kök hücreler de hem kolay temin edilebilmesi hem de üstün özellikleri sebebiyle ön plana çıkmaktadırlar. Bu çalışmada dental kök hücrelerin kemik doku mühendisliği yaklaşımları için önemi ve rejenerasyon potansiyelleri incelenmiştir

    The effects of dental pulp stem cells on bone regeneration in rat calvarial defect model: Micro-computed tomography and histomorphometric analysis

    Objective: Stem cell therapies may be applicable to all fields of medicine, including craniomaxillofacial surgery. Dental pulp stem cells also have significant osteogenic properties. This study aimed to evaluate the influence of dental pulp stem cells on bone regeneration and to ascertain whether or not there was any superiority over traditional methods. Design: In this study, 15 non-immunodeficient Wistar albino rats were used. The rats were divided into three groups: (1) untreated control group; (2) hydroxyapatite tri-calcium-phosphate (HA/TCP) paste; (3) human dental pulp derived stem cells (DPSC) mixed with HA/TCP paste (HA/TCP+DSPC group, n = 10). Two symmetrical full-thickness cranial defects were created on each parietal region (10 defects for each group). The animals were sacrificed 8 weeks post-surgery and samples were analyzed by microcomputer tomography (mu-CT) and histomorphometry. Results: The calcification rate and bone mineral density (BMD) values in Group 3 were found to be significantly higher than in the other two groups. Radiographically, bone regeneration was greater in Group 2 compared with the control group. However, there was no significant difference between Groups 2 and 1 in respect of histological analysis. Conclusions: According to the results of the present study, DPSCs may be a suitable factor for bone tissue engineering because they can be easily obtained and differentiate into bone cells.     The effects of high-fat diet on implant osseointegration: an experimental study

    Objectives: In this study, we investigated whether a high-fat diet (HFD) affected the bone implant connection (BIC) in peri-implant bone. Materials and Methods: Four male rabbits were used in this study. Dental implant surgery was introduced into each tibia, and four implants were integrated into each animal. In both the normal diet (ND) group (n=2) and HFD group (n=2), 8 implants were integrated, for a total of 16 integrated implants. The animals continued with their respective diets for 12 weeks post-surgery. Afterward, the rabbits were sacrificed, and the BIC was assessed histomorphometrically. Results: Histologic and histomorphometric analyses demonstrated that BIC was not impaired in the HFD group compared to the ND group. Conclusion: Within the limitations of this study, we found that HFD did not decrease the BIC in rabbit tibias.

    Effects of low-level laser therapy following surgical extraction of the lower third molar with objective measurement of swelling using a three-dimensional system

    The aim of the present study was to evaluate and compare the effects of single- and two-dose low-level laser therapy (LLLT) on the postoperative swelling, trismus and pain of patients undergoing extraction of impacted mandibular third molars. In addition, edema was volumetrically measured with a 3dMD face system. A total of 45 patients were randomly divided into three treatment groups (15 patients in each group) as follows: Group 1, receiving routine management with ice application and serving as the control group; Group 2, receiving a single dose of LLLT immediately following surgery; and Group 3, receiving two doses of LLLT, immediately following surgery and on day 2 after surgery. In the present study, a gallium-aluminum-arsenide (0.3 W, 40 sec, 4 J/cm(2)) diode laser device was applied extraorally at the insertion point of the masseter muscle. The trismus, pain level and facial swelling of the patients were evaluated. The visual analog scale (VAS) was used to examine the pain degree, while a 3dMD face photogrammetric system was used to evaluate the volumetric alterations of the swelling. The results indicated no statistically significant differences in the mean swelling or trismus among the three groups. The mean VAS measurements did not differ significantly among the groups at postoperative day 2; however, significantly reduced VAS values were observed in Group 2 compared with Group 1 at postoperative day 7 (P<0.05). The present study demonstrated that, although single-dose or two-dose LLLT had beneficial effects on the swelling, trismus and pain level, a significant reduction was only observed in the pain level at postoperative day 7.

Bu sebeple, dikkatli tedavi edilmesi ve agresif doğasından dolayı uzun dönemli takibi gerekmektedir. Nadir görülen bu kistin teşhis ve tedavi yaklaşımları ile bir olgu raporu sunulmuştur.

    The investigation of effects of proton pump inhibitors

    Aim: Proton pump inhibitors (PPI) have been widely used throughout the world. The effects of the PPIs on bone mineral density (BMD) is important for oral surgeons. The aim of this study was evaluate alterations in bone mineral density in patients using PPI. Material and Methods: A total of 92 patients were enrolled in the study and were separated into 4 groups (24 in control group, 22 in Lansoprazole group, 24 in Pantoprazole group and 22 in Esomeprazole group). Hounsfield Unit scores were measuredseparately of the incisor, canine, premolar, molar and tubule regions in the jaws with computed tomography. The measurement area in the dental regions was defined as an area of 2mm2 at least 2mm from the apex of the teeth. All data were collected and statistically analysed. Results: There was no significant difference between the groups in respect of mean age. There were no significant differences between the control and the study groups. Also no significant difference was found among the study groups. Conclusion: These data revealed that there were no correlations between decreased maxillary bone mineral density and proton pump inhibitor use. Further studies with more extensive samples should take into consideration drug doses and the period of use.

    Is Conservative Surgical Treatment Sufficient to Treat Unicystic Mural Ameloblastoma in Infant?

    Ameloblastoma, a benign neoplasm derived from odontogenic epithelium, is an aggressive and locally invasive tumor. It represents 11% of all odontogenic tumors and 1% of all oral odontogenic epithelial tumors. In this case report, a 20-month-old boy was referred to our clinic with complaint of collapse in his symphysis region of the mandible. Radiographic examination revealed unilocular radiolucency in this region. The lesion was enucleated with 1 tooth germ under general anesthesia and diagnosed as mural unicystic ameloblastoma by histopathologic examination. After the surgery, complete healing was obtained clinically and radiographically. No sign of recurrence has been seen during the follow-up period of 4.5 years. To our knowledge, this was the second youngest case of ameloblastoma in the English literature. However, it is the youngest case of ameloblastoma that occurred in an infant boy.

    Is More Cortical Bone Decortication Effective on Guided Bone Augmentation?

    This study aims to evaluate the possible effect of more cortical bone decortication (CBD) on guided bone augmentation. A total of 16 New Zealand rabbits and 32 titanium domes were used. No cortical bone decortication was applied to the control group and in the study groups, the cortical bones were decorticated with a round burr (Group A: 1 hole with bleeding, Group B: 5 holes with bleeding, Group C: a thin layer of compact bone was completely removed with no bleeding). Then 2 titanium domes were placed on the calvarium of each rabbit with hydroxyapatite/beta-tricalcium phosphate. After 3 months, the animals were sacrificed and specimens were sent for histological and histomorphometric analysis. Histological and histomorphometric analysis showed that bone decortication with burr significantly increased new bone regeneration in all the experimental groups compared with the control group (P<0.05). No statistically significant difference was determined between the study groups. In conclusion, CBD, which has no negative impact on surgery, has a positive effect on guided bone augmentation. However, a greater amount of CBD does not have a greater effect.

    Is Ultrasonography Useful in the Diagnosis of Nasolabial Cyst?

    Nasolabial cysts are nonodontogenic cysts that occur beneath the ala nasi. Its pathogenesis is uncertain. Because the nasolabial cyst is a soft tissue lesion, plain radiographs are useless. CT and MRI should be evaluated. In this report, a nasolabial cyst is described including its features on ultrasonography (USG) and CT exams.

    Pulpal dental kök hücre kaynağının kritik boyuttaki rat kalvaryal defekt modeli üzerindeki kemik rejenerasyon etkinliğinin incelenmesi

    Kök hücre tedavileri tıbbın her alanında umut verici sonuçlar vermektedir. Maksillofasiyal cerrahi, kemik doku mühendisliğinin çalışıldığı alanlardan birisidir. Bu çalışma ile pulpa kaynaklı dental kök hücrelerin kemik rejenerasyonuna etkisi ve geleneksel yöntemlere göre üstün olup olmadığı araştırıldı. Wistar albino cinsi 15 sıçan 3 gruba ayrıldı. Her deneğin kalvaryal kemiğine standart bilateral defektler açıldı. 1.gruptaki defektler (10 adet) boş bırakıldı. 2. gruptaki defektler (10 adet) Hidroksi-apatit-tri-kalsiyum-fosfat (HA-TCP) ile dolduruldu. 3.gruptaki defektlere (10 adet) ise insan dental pulpasından elde edilen mezenkimal kök hücrelerin HA-TCP greft malzemesine ekilmesi ile elde edilen materyal yerleştirildi. 8 hafta sonra denekler sakrifiye edilip mikro-BT ve histomorfometri incelemeleri yapıldı. Deneklerden elde edilen 30 numunenin istatiksel analizleri yapıldı. Buna göre; hem mikro-BT hem de histomorfometri sonuçları, kök hücre grubunun kemik yoğunluğu ve kalsifiye alanları diğer gruplara göre istatistiksel olarak anlamlı ölçüde daha fazla artırdığını gösterdi. Greft ve kontrol grupları arasında histolojik olarak bir fark bulunmamasına rağmen mikro-BT sonuçlarına göre greft grubu lehine anlamlı bir fark vardı. Bu durum greft materyalinin densiteye etkisinin bir sonucu olarak yorumlandı. Elde edilen sonuçlara göre pulpal kaynaklı dental kök hücrelerin, geleneksel yöntemlere göre daha etkili olduğu ve kemik doku mühendisliği çalışmaları için uygun bir kaynak olabileceği söylenebilir.
