    Anti-inflammatory Montelukast prevents toxic effects of 2,3,7,8-tetrachlorodibenzo-p-dioxin: Oxidative stress, histological alterations in liver, and serum cytokine levels

    This study aimed to investigate the potential beneficial effects of the montelukast (ML) on oxidative stress and histological alterations in liver tissues and cytokine levels in rats intoxicated with 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD). Rats were divided randomly into four equal groups (control, TCDD, ML, TCDD + ML). TCDD were administered by gavages dissolved in corn oil at the doses of 2 mu g/kg/week, and ML was given intraperitoneally at the dose of 10 mg/kg/day. Oxidative status, histological alterations, and cytokine levels were analyzed on day 60. The results showed that although TCDD induced oxidative stress via significant increase in formation of thiobarbituric acid reactive substance, it caused a significant decline in glutathione (GSH), catalase (CAT), and superoxide dismutase (SOD) levels in liver. Besides, TCDD led to significant histopathological damage in liver and serum cytokine levels alterations (increase in tumor necrosis factor alpha and interleukin 1 beta levels). In contrast, ML treatment reversed oxidative effects of TCDD by increasing the levels of GSH, CAT, and SOD and decreasing the formation of TBARS. Also, it can normalize the levels of histological and cytokine alterations induced by TCDD. In conclusion, it was determined that TCDD exposure caused adverse effects on cytokine levels, histological alterations, and oxidative stress in rats. However, ML treatment partially eliminated toxic effects of TCDD. Thus, it was judged that coadministration of ML with TCDD may be useful to attenuate the negative effects of TCDD.

    Applicability of the ROX index in decision-making for hospitalization in COVID-19 patients

    Aim: Data on the outpatient follow-up of COVID-19 cases is still scarce. Also, the significance of the ROX index in decision-making for hospitalization in the ambulatory COVID-19 cases remains unknown. The aim of this study is to determine the general characteristics of COVID-19 patients treated as outpatients and to investigate whether the ROX index is applicable in hospitalization decisions.Material and Methods: This retrospective cohort study was conducted in confirmed adult COVID-19 cases between 15 October 2020 and 01 March 2021. A total of 5240 confirmed COVID-19 patients were included in the present study. Factors affecting hospitalization were investigated.Results: The study population was divided into two groups as those who require hospitalization (n=672) and those who did not (n=4568). The number of male patients and the mean age of the patients were significantly higher in hospitalized patients group (p=0.046, p<0.001). ROX index that was calculated at the home visit on the third day of disease was found significantly lower in the group of hospitalized patients (p<0.001). There was a significant correlation between ROX index and inflammatory biomarkers in the present study (p<0.001). The ROX index was found the most accurate parameter for decision-making for hospitalization in ambulatory COVID-19 patients (AUC=0.794 CI=0.773-0.814, p<0.001).Discussion: The ROX index can be a useful and objective clinical tool for decision making for hospitalization in the ambulatory COVID-19 cases.

    Attitudes and behaviors of individuals aged 90 and over towards rational use of drugs: cross-sectional research

    Introduction: The current study was aimed to evaluate the attitudes and behaviors of individuals 90 years and older living in Malatya regarding rational drug use. Methods: This cross-sectional, descriptive study was carried out with people aged 90 and over (n=1022) living in Malatya province and its districts. Face-to-face interview method was used to collect study data. In the first part of the form used for data collection, the descriptive characteristics of the participants were questioned, and in the second part, the attitudes and behaviors towards rational drug use were examined. Rational drug use features are evaluated by World Health Organization (WHO) prescribing indicators. Results: The frequency of rational drug use was 84.1%, and the rate of over-the-counter drug use was 19.2% among individuals 90 years and older. 52.3% of the participants had over-the-counter medication at home for use, when necessary, without doctor's recommendation. The most commonly available over-the-counter group of drugs at home were painkillers. Conclusion: The frequency of rational drug use in individuals aged 90 and older has been determined higher than in the literature on individuals aged 65 and older. Education, guidance and counseling programs can be created to increase rational drug use within elderly people communities.

    Circulating Soluble Lectin-Like Oxidized Low-Density Lipoprotein Receptor-1 Levels Are Associated With Erectile Dysfunction in Patients Without Known Coronary Artery Disease

    Aim. Endothelial dysfunction and microvascular damage are involved in the pathogenesis of erectile dysfunction (ED). Soluble lectin-like oxidized low-density lipoprotein receptor-1 (sLOX-1) is identified endothelial receptor for oxidized low-density lipoprotein (ox-LDL) that plays a pivotal role in ox-LDL-induced endothelial dysfunction. The purpose of the current study was to determine the association between sLOX-1 and ED in patients without known coronary artery disease (CAD). Main Outcome Measures. Diagnosis of ED was based on the International Index of Erectile Function Score-5. Levels of sLOX-1 were measured in serum by enzyme-linked immunosorbent assay. Methods. One hundred thirty-eight subjects with ED patients without known CAD (ED group) and 75 age-matched subjects without ED and known CAD (Non-ED Group) were included in this study. Results. Plasma levels of sLOX-1 were significantly higher in ED than in Non-ED group (9587 and 49 +/- 30pg/mL, respectively, P<0.001). The levels of sLOX-1 highly negative correlated with score of ED (r=-0.618, P<0.001). The sLOX-1 levels >75pg/mL predicts ED with 26.8% sensitivity and 96.0% specificity on receiver operator characteristic analysis. Conclusions. Our study demonstrated that serum sLOX-1 levels were associated with endothelial dysfunction that predicts ED. Moreover, the current study revealed that there was strong negative correlation between the levels of circulating sLOX-1 and score of ED. This study suggested sLOX-1 may be involved in the pathogenesis of ED in patients without known CAD. Kobat MA, Frdolas F, Balin M, Celik A, Bentli R, and Baydas A.     Clinical features and prognostic factors affecting survival of ambulatory followed COVID-19 patients aged over 65 years

    Aim: The majority of the patients with COVID-19 are followed ambulatory. Determination of prognostic factors of mortality in risky groups is essential to improve patient management. The aim of this study is to describe the clinical presentation in patients over 65 years of age with COVID-19 who are followed up at home by a physician and provide insights into the initial prognostic factors in this distinctive population. Material and Methods: This is a retrospective and observational study. Clinical records of the patients aged over 65 years who were visited by the filiation team, including a physician at home, due to the diagnosis of COVID-19 disease within 2 months were reviewed. Factors affecting mortality were examined. Results: Our study included 51 deceased (mean age: 75,1 +/- 9,0 years, 40,2% males), and 102 patients with COVID-19 who survived (mean age: 73,0 +/- 6,9 years, 68,6% male). Platelet count ( < 150.000, OR 7,26, p=0,001), CRP level ( < 4, OR 4,55, p=0,02), albumin level ( OR 3,24, p=0,02), and Troponin I level (OR 0,03, p=0,02) were the strongest predictors for death. When propensity score matching was applied, gender (male, OR 7,14, p=0,02) and platelet count (< 150.000, OR 5,34, p=0,02) were the strongest predictor Discussion: Elderly COVID-19 patients have a high mortality rate. An easily measurable and accessible platelet count may be a predictor of a bad outcome. Close follow-up and timely treatment may significantly reduce mortality in high- risk elderly patients under.

    Clinical Properties of COVID-19 Developed in the Patients with Tuberculosis

    There are limited publications about the Coronavirus disease 19 (COVID-19) clinic developing in the patients with active tuberculosis (TB). In this study, it was aimed to determine some clinical features of patients diagnosed with TB who also had COVID-19. In this retrospective cross-sectional study, 71 pa- tients with COVID-19 were evaluated out of a total of 595 patients diagnosed with TB in our province between 2015 and 2021. After contracting COVID-19, a total of nine (12.6%) TB patients were hospital- ized, five (7%) patients were admitted to the intensive care unit, three (4.2%) were intubated, and one (1.4%) died due to severe COVID-19. The frequency of such health problems was found to be higher than the normal population living in the same province. None of these complications were observed in a total of 40 female TB patients, and the hospital and intensive care unit admission rates for men were significantly higher than for women. The results of this study showed that men with active TB had more health problems due to COVID-19 than the normal population. Comprehensive studies are needed to detail the resilience of female TB patients against COVID-19.

    Cyberchondria, Covid-19 phobia, and well-being: a relational study on teachers

    Background: This study aims at investigating level and contributor factors of Cyberchondria, COVID-19-related Phobia, and Well-Being in a sample of teachers in Turkey. Methods: The study was conducted on teachers (n=1000) working in a province in eastern Turkey. Data for the study were collected using a form that included particitest, and Kruskal Wallis analysis of variance were used to analyze the data. Results: As participant's cyberchondria levels rose, C19P-SE scores increased (r=0.271, p<0.001), and WHO-5 scores decreased (r=-0.224, p<0.05). Corona-phobia was higher in those who used social media than in those who did not (p<0.05). Cyberchondria scale scores were higher among those who had taken medications without a physician's recommendation during the pandemic. Participants who had a disabled person or a person in need of care in their household had higher scores for distrust of the physician and C19P-SE than for the cyberchondria severity scale sub-dimension, and the WHO-5 mean scores were lower (p<0.001, P=0.016, and P=0.020, respectively).Conclusions: The study results show that increasing levels of cyberchondria trigger COVID-19 phobias in teachers during the COVID-19 pandemic and negatively affect their well-being. This descriptive study can help understand the risk group for cyberchondria, the influencing factors, and the health and economic consequences, and identify strategies for effective combating with cyberchondria.

    Diabetic muscle infraction: An unusual cause of muscle pain in a diabetic patient on hemodialysis

    Diabetic muscle infarction (DMI) is a rare, painful and potentially serious complication in patients with poorly controlled diabetes mellitus and frequently misdiagnosed clinically as abscess, neoplasm, or myositis. A 36-year-old diabetic woman referred to our clinic with severe pain in the left anteromedial thigh. She had a 15-year history of Type 2 diabetes mellitus (DM). She was complicated by diabetic nephropathy and requiring hemodialysis. She had first noticed pain and swelling in her left thigh after a minimal trauma for 2 days prior to presentation. Clinical and laboratory evaluation, and muscle biopsy revealed the diagnosis of muscle infarctions. She did no respond to the conservative therapy. Pain and swelling in her thigh worsened progressively. She underwent surgical debridment and then, her clinical status improved. We describe the characteristic clinical and pathologic findings and the course of the illness with emphasis on the importance of recognition of the syndrome so that unnecessary investigation and overzealous therapy can be avoided.

    THE EFFECT OF CHRONIC CARBON MONOXIDE EXPOSURE ON BRAIN IN BARBECUE WORKERS

    The aim of our study is to investigate whether chronic carbon monoxide (CO) exposure has effects on the brains of the people who work as barbecue workers (BWs) for a long time. Twenty males who had been working in the indoor environments of various restaurants (Group I) and 20 healthy males who were matched by age with the other group (Group 2) were included in the study. Laboratory tests were performed in both groups of people. Magnetic Resonance Spectroscopy (MRS) was applied to the people whose blood samples were taken. In Group I, significant correlations were not detected between carboxyhemoglobin (COHb) values and the variables of age, working hours, hemoglobin (Hb), white blood cell (WBC), hematocrit (HCT), platelet (PLT), mean platelet volume (MPV), pH, oxygen saturation (Sat O2) (p>0.05). In terms of Hb, WBC, HCT, PLT, MPV, pH, Sat O2, COHb, N-acetylaspartate/creatine in basal ganglion, choline/creatine in basal ganglion, N-acetylaspartate/creatine in frontal lobe periventricular white matter, choline/creatine in frontal lobe periventricular white matter variables, significant differences between Group I and Group 2 were not determined. (p>0.05). As a result of this study; it is detected that chronic exposure to CO in BWs does not have any effects on brain with respect to MRS imaging method.

    EFFECTS OF SLEEP QUALITY, INCOME LEVEL AND COMORBID CONDITIONS ON QUALITY OF LIFE IN A TURKISH ELDERLY POPULATION: A MULTICENTRE STUDY

    Introduction: Our aim is to investigate the effects of sleep quality, income level and comorbid conditions on Quality of Life in a Turkish elderly population. Materials and Method: This multicentric study was performed in seven districts. A total of 1030 patients older than 65 years of age who applied to the internal medicine and geriatrics outpatient clinics of study centres between January and December 2014 were included. All patients underwent the Pittsburgh Sleep Quality Index and Quality of Life Assessment (Short Form 36) tests viaface to face interview method. The demographic properties of patients were also recorded during this interview. Results: The mean Quality of Life subscale scores, except for vitality, were significantly lower (p < 0.001) in patients with poor sleep quality. When analysing the relationship between Quality of Life and multiple chronic diseases, it was observed that patients having multiple chronic diseases had significantly lowered scores in three subscales (physical functioning, mental health and bodily pain; p=0.04, p=0.04, p=0.01, respectively). There were significant differences between patients with high versus low income level with respect to the mean physical functioning, mental health and bodily pain subscales (p=0.01). Conclusion: Our study demonstrated an adverse effect of low sleep quality, multiple chronic disorders and low income level on Quality of Life.

    Endothelial Dysfunction in Patients with Acromegaly

    It has been shown that Growth hormone (GH) and insulin like growth factor 1 (IGF-1) has important effects on heart and vascular system. However, little is known about endothelial function in acromegaly. The aim of this study was to determine endothelial function in patients with acromegaly. Endothelial function was evaluated in 10 patients with active acromegaly (eight men, two women) and in 11 age, sex and weight matched control (eight men, two women) using a flow mediated dilatation (FMD) technique in the brachial artery. In acromegalic patients mean FMD was 8.3 +/- 3.5 % and significantly lower than that of control group (p=.023). However, there were no significant correlations between GH, IGF-1 levels and percentage of FMD. These findings suggest that endothelial function impaired in patients with acromegaly.

    Etiologic Factors of Nonalcoholic Hepatosteathosis in Malatya

    Aim: Etiology of non-alcoholic fatty liver disease is not exactly understood. The aim of this study is to investigate the etiologic factors in patients with non-alcoholic fatty liver disease in and around Malatya, Turkey. Material and Methods: In patients who applied to the Department of Gastroenterology, Inonu University, fatty liver was detected through hepatobiliary ultrasonography; these patients were evaluated retrospectively. Patients having over 20 gram alcohol/day in females and 30 gram alcohol/day in males were ruled out to exclude alcohol dependent fatty liver. Patients with normal liver enzyme levels and those with a two fold or higher increase were accepted as hepatosteatosis and steatohepatitis patients, respectively. Results: A total of 112 patients 58 (51.8%) women, 54 (48.2%) men with the 43.8±11.3 years mean age were included in the study. The mean age of men was 41.8±11.3 years and the mean age of women was 45.4±11.3 years. In a total of 112 patients, we have detected hyperlipidemia in 97 (86.6%), obesity in 53 (47.3%), insulin resistance in 47 (42%), hyperlipidemia with insulin resistance in 16 (14.2%), hyperlipidemia with obesity in 27 (24.1%), obesity with insulin resistance in 35 (31.3%), and latent diabetes mellitus in 11 (31.3%). Conclusion: Our results suggest that it is important to evaluate patients with the risk factors such as hyperlipidemia, obesity, insulin resistance regarding non-alcoholic fatty liver disease, which itself may lead to cirrhosis. Besides patients with non-alcoholic fatty liver disease should be evaluated in terms of latent diabetes.

    Geriatrik hemodiyaliz hastalarında diyabet varlığının trombosit indekslerine etkisi

    Çalışmamızda trombosit indekslerinin diyabetik ve diyabetik olmayan geriatrik hemodiyaliz hastalarında karşılaştırılması amaçlandı. Çalışmaya 42 diyabetik (20 erkek, 22 kadın; ortalama yaş: 71,2±5,5) 55 diyabetik olmayan (30 erkek, 25 kadın ortalama yaş:72,1±4,6) geriatrik hemodiyaliz hastası alındı. Tam kan sayımı için EDTA'lı tüplere 2ml venöz kan örneği alındı ve trombosit indeksleri [Ortalama Trombosit Hacmi (OTH), Trombosit Dağılım Genişliği (TDG)] otomatize kan sayım cihazında çalışıldı. OTH ve TDG anlamlı olarak diyabetik hastalarda diyabetik olmayanlara kıyasla yüksek bulundu (P<0.05). Trombosit indekslerinden OTH ve özellikle TDG diyabetik geriatrik hemodiyaliz hastalarında yüksek olarak saptandı. Daha ileri çalışmalarla trombosit indeksleri ile artmış kardiyovasküler hastalık riski arsındaki ilişki değerlendirilmeli ve tedavi planında bu indeksler göz önünde bulundurulmalıdır.

    Haploidentical Transplantation Experience in Turgut Ozal Medical Center: Case Report and a Review of Literature

    Haploidentik kök hücre nakli HLA uyumlu donörü olmayan hastalar için başka bir alternatif nakil modelidir. Hala, allojenik nakil gereksinimi olan hastaların ancak üçte biri HLA uyumlu vericiye sahiptir. Acil kök hücre nakli gereksiniminde haploidentik nakil bir çok hasta için önemli bir çözüm yoludur. Daha önceleri haploidentik hematopoetik kök hücre nakli yüksek oranda graft rejeksiyonu, graft versus host hastalığı, graft başarısızlığı ve nakil ile ilgili mortalite ile karakterize idi, fakat T hücre deplesyonundaki yeni gelişmeler ve mega doz kök hücre nakli yönetimi haploidentik kök hücre naklinde bazı problemlere çözüm oldu. Biz bir vakaya uygulanan Turgut Özal Tıp Merkezi' ndeki haploidentik kök hücre nakli deneyimimizi sunduk.

    Haploidentical Transplantation Experience in Turgut Ozal Medical Center: Case Report and a Review of Literature

    Haploidentical hematopoietic stem-cell transplantation is an optional transplant modality for patients without a Human Leukocyte Antigensmatched donor. Still, one-third of patients who might benefit from allogeneic transplantation are able to find a Human Leukocyte Antigensmatched related donor. In need of immediate stem-cell transplantation, haploidentical donor is an important solution for many patients. Previously, haploidentical hematopoietic stem-cell transplantation was characterised by a high rejection rate, graft-versus-host disease, graft failure, and transplant-related mortality, but there have since been significant improvements in some areas. Development of new extensive T-cell depletion methods with megadose stem cell administration have overcome problems of haploidentical hematopoietic stem-cell transplantation. We presented an applied case haploidentical hematopoietic stem-cell transplantation as an experience in Turgut Ozal Medical Center.

    Hepatic Arginase - Nitric oxide imbalance: Impact of carcinogenesis and therapeutic effect of sodium channel blockage in an in vivo rat model

    Objective: Nitric oxide synthase and arginase are frequently antagonistic and interactive, although both use L-arginine as common substrate. Their balance is of potential functional importance. How the balance changes in cancer is unknown. Increasing evidence suggests that progression of carcinomas involves functional voltage-gated sodium channel (VGSC) activity. Methods: The present study extended this study to liver and aimed to determine whether (i) DMBA carcinogenesis would affect the activities of arginase and NOS and (ii) treatment with Na-channel blocker RS100642 would ameliorate the impact of the carcinogen on the arginase-NOS balance. Results: DMBA application significantly increased arginase activity and, correspondingly, the level of L-ornithine by 25-33%. In contrast, NOS activity decreased by 11%. Importantly, RS100642 treatment completely suppressed the effect on arginase. Conclusion: It is concluded (i) that DMBA carcinogenesis changes the hepatic arginase-NOS balance, increasing the overall dominance of arginase and (ii) that VGSC inhibition has a protective effect on liver.

    INVESTIGATION OF KNOWLEDGE, ATTITUDE AND BEHAVIOURS OF PARENTS REFUSING CHILDHOOD VACCINES IN MALATYA, AN EASTERN CITY OF TURKEY

    Objectives: The modem rise of vaccine rejection in society can alter the current progress that has been made towards the control and prevention of certain diseases, possibly even resulting in epidemics involving these preventable diseases. The aim of this study is to analyse the knowledge, attitude and behaviours of parents in Malatya city who rejected childhood vaccines. Methods: This descriptive, cross-sectional study was conducted between September-November 2019. The study includes parents who rejected vaccines and are registered in the family medicine clinics of Malatya city - total of 453 participants. The objective was to include all parents and avoid a sampling procedure. However, only 151 (33%) parents agreed to participate. These parents who rejected vaccines were individually contacted by phone. Descriptive data was represented by number (n) and percentage (%). The chi-square test was utilized in the statistical analysis of data and p <0.05 was considered significant in all evaluations. Results: Mothers in the study group had a mean age of 26.07 +/- 3.64, while the fathers were on average 30.03 +/- 4.59 years of age; 98% of parents were aware of the health risks that vaccine rejection presented; 93% of parents were not satisfied with the explanation, insight, and advice that the healthcare personnel provided regarding vaccines. All parents of the study group stated the following: vaccines should not be administered because other children in their close environment acquired a disease as a result of vaccination, vaccines can harm the immune system of children - not yet fully developed, vaccines are unsafe and endanger the health of children. Conclusions: It can be said that parents who have obtained a lot of false information possess altered decisions and views on vaccinations, to the point where they accept the risks presented by preventable diseases. In addition, individuals lose trust following negative experiences with vaccination.

    Malatya İli Örneğinde Nüfus YoğunluğununCOVID-19 İnsidansı ile İlişkisi

    Giriş: Koronavirüs Hastalığı-2019 (COVID-19) pandemisi ile tüm dünyada sosyal ortamlardaki insan yoğunluğunun azaltılması ve sos yal mesafe kuralının uygulanması ile virüsün yayılımı azaltılmaya çalışılmıştır. Ancak yerleşim bölgelerinin nüfus yoğunluğu ile hastalığınyaygınlığı hakkında az sayıda araştırma vardır. Bu çalışmada, Malatya ili örneğinde nüfus yoğunluğu ile COVID-19 insidansı arasındakiilişkinin saptanması amaçlanmıştır.Materyal ve Metod: Korelasyonel (Ekolojik) bir araştırma olarak yapılan bu çalışmada Malatya ili ve ilçelerinin nüfus yoğunluğu km2 başına düşen kişi sayısı olarak hesaplandı. Yerleşim bölgelerinin COVID-19 insidansları ile hesaplanan nüfus yoğunluğu arasındaki ilişkiPearson korelasyon analizi kullanılarak araştırıldı. Bulgular: Malatya ili genel COVID-19 insidansı binde 77 olarak hesaplandı. İlçeler içinde Arguvan ile Kale’nin COVID-19 insidansının sırasıylabinde 11 ile 16 olarak en düşük, Yeşilyurt ile Battalgazi ilçelerinin COVID-19 insidansının ise sırasıyla 105 ile 69 olarak en yüksek düzeydeolduğu görüldü. Malatya ili ilçelerinin verileri karşılaştırıldığında insidans ve nüfus yoğunluğunun (r= 0.82, p= 0.001) çok yüksek düzeydekorelasyona sahip olduğu belirlenmiştir. Sonuç: Bu çalışmada, ülkemizin orta büyüklükte bir şehri olan Malatya’da nüfus yoğunluğu ile COVID-19 insidansı arasında önemli birkorelasyon bulunduğu saptanmıştır. COVID-19 pandemi yayılımı pek çok dinamik faktörle ilişkili olmakla birlikte; şehir planlamalarındanüfusun geniş alanlara yayılımı sağlanarak ileride oluşabilecek benzer salgınlara karşı direnç oluşturulabilir.
