Öğe Investigation of hepatitis B virus prevalence and reactivation frequency in malignancy patients administering chemotherapy(2021) Cengiz, Muhammet; Seckin, Yuksel; Cagin, Yasir; Nuransoy Cengiz, AyseAim: This study aimed to investigate the frequency at which physicians test hepatitis B virus (HBV) serology before treatment and the rate of hepatitis B virus reactivation (HBVr) after treatment in patients undergoing chemotherapy (CT) due to a solid organ malignancy (SOM) or hematological malignancy (HM). Materials and Methods: Baseline clinical feature, HBV serology, and liver function test data for 1275 patients ≥ 18 years of age who underwent CT for the first time due to various SOMs and HMs from 2015-2017 were obtained from the database and retrospectively analyzed. HBV serology was studied in 296 (201 HM, 95 SOM) of the 1275 patients retrieved from the database. Results: The prevalence of HBV was 9.5% (121 patients). Only 49 (40.4%) HBV-positive patients had HBV DNA. Of those, 72 (59.5%) underwent prophylactic antiviral therapy. HBVr was detected in only 3 patients (1.5%), and all of those patients had HMs and were recommended oral antiviral therapy but did not. Conclusion: In patients with SOMs, the screening rates of HBV serology, as well as prophylaxis and follow-up HMs were extremely low. The frequency of new-onset diabetes after transplant (NODAT) is 5-30% in liver transplant recipients. We aimed to analyze the frequency and predictors of NODAT in the patients undergoing liver transplantation due to acute liver failure. Methods Adult patients undergoing liver transplantation due to acute liver failure were analyzed retrospectively. The patients with chronic liver failure or diabetes were excluded. We measured pretransplant random blood glucose and posttransplant fasting blood glucose. NODAT was diagnosed according to principally 1st month fasting blood glucose (group 1 < 100, group 2 100-125, group 3 > 125 mg/dL). The participants were subgrouped according to age, gender, body mass index, etiology, antiviral medication, thyroid function, pretransplant random blood glucose, donor type, immunosuppressive drug, common infection, and surgical complication. Results Mean age of total 91 patients was 33.48 (+/- 13.35), and 52.7% (n = 48) of them was female. The ratio of NODAT was 26.98% on the 1st month. NODAT group had a higher pretransplant random blood glucose than the others. Pretransplant hyperglycemia increased the risk of NODAT by 4.065 times (p = 0.018). Conclusion We showed that pretransplant hyperglycemia increased NODAT risk by 4 times, but hypoglycemia did not affect. So, pretransplant hyperglycemia should be controlled also in the patients with acute liver failure as in the patients with chronic liver failure.Öğe Postpartum Dönemde Homozigot Faktör-V Leiden Mutasyonuna Bağlı Akut Budd-Chiari Sendromu(Fırat Tıp Dergisi, 2017) Bilgiç, Yılmaz; Cengiz, Muhammet; Cengiz, Ayşe Nuransoy; Yılmaz, Cengiz; Çağın, Yasir Furkan; Sancaktar, Fatih; Yıldırım, OğuzhanÖz: Budd--Chiari Sendromuküçük hepatik verilerden sağ atriuma kadar olan damarsal yapılarda ortaya çıkan darlık veya obstrüksiyonlar sonucunda karaciğer venöz dönüşünün bozulmuş olduğunadir görülen bir durumdur. Klinik spektrumu, kronik asemptomatik hast alıktarı akut fulminan karaci-- ğer yetmezliği arasında değişkenlik gösterir ve karaciğer sirozu ile neticelenebilir. Clinical spectrum changes in a range between chronic asymptomatic patients to fulminant liver failure, it may also re sults with cirrhosis. T here are many etiologies of the syndrome like inherited and secondary thrombophilias and external compression of veins. Pregnancy and postpartum period increase the tendency to thrombosis. In this paper, we presented a rare case of acute Budd Chiari syndrome, which was found to be homozygous for factor-V leiden mutation in the postpartum period.
