Öğe Evaluation of endothelial function in subclinical hypothyroidism and subclinical hyperthyroidism(Mary Ann Liebert, Inc, 2004) Cikim, AS; Oflaz, H; Ozbey, N; Cikim, K; Umman, S; Meric, M; Sencer, ESubclinical hypothyroidism and subclinical hyperthyroidism are two frequently occurring conditions for which exact therapeutic approaches have not yet been established. The aim of this study was to compare the endothelial function and carotid artery intimae-media thickness (IMT) of these two groups of patients to euthyroid subjects and to assess the effects of these conditions on endothelial function. Study groups comprised of 25 subclinical hypothyroid patients (mean age, 32.28 +/- 9.67 years), 13 subclinical hyperthyroid patients (mean age, 35.69 +/- 9.67 years), and 23 euthyroid subjects (mean age, 35.87 +/- 7.93 years). They were evaluated for flow-mediated dilatation (FMD), and carotid artery IMT. The groups were matched strictly for atherosclerotic risk factors. The subclinical hypothyroid group was found to have significantly lower FMD values. No significant differences were observed between the groups with respect to other vascular parameters. The only discriminative factor between the groups was the state of their thyroid function. Therefore, subclinical hypothyroidism may have adverse effects on endothelial function independent from other well-known atherosclerotic risk factors.Öğe Hypopituitarism in older adults - The report of five cases with different presentations(Advanstar Communications, 2006) Cikim, A; Dikilitas, M; Cikim, KPatients with hypopituitarism may present with many different signs and symptoms which result from reduced secretion of one or more of pituitary hormones. Severe hyponatremia and hypoglycemia are rare but potentially life threatening presenting clinical features of hypopituitarism. As these abnormalities are completely restored with replacement therapy and patients are often undiagnosed until admission, a high clinical suspicion Is necessary to diagnose a patient 1 with hypopituitarism. We present five cases exhibiting rare symptoms of hypopituitarism-severe hyponatremia, hypoglycemia and osteoporosis-as the only presenting feature; these cases illustrate that hypopituitarism must be considered In the differential diagnosis of these abnormalities, especially in older patients.Öğe Serum proinflammatory mediators at different periods of therapy in patients with multiple myeloma(Hindawi Publishing Corporation, 2005) Kuku, I; Bayraktar, MR; Kaya, E; Erkurt, MA; Bayraktar, N; Cikim, K; Aydogdu, IMultiple myeloma (MM) is a malignant disease characterized by the clonal proliferation of plasma cells within the bone marrow. Several cytokines have been demonstrated to be involved in the control of growth, progression, and dissemination of MM. We determined serum levels of interleukin-1 beta (IL- 1 beta), Soluble interleukin-2 receptor (sIL-2R), interleukin-6 (IL-6), interleukin-8 (IL-8), tumor necrosis factor-alpha (TNF-alpha), and C-reactive protein (CRP) in 14 newly diagnosed MM patients. The median age of the patients was 63.4 +/- 10.8 years and all of the patients were stage III (classified according to the Durie-Salmon classification). The same parameters were measured in 15 healthy controls. In addition, we also examined the effects of vincristine-adriamycin-dexamethasone (VAD) therapy on the same parameters and mediators as well as the relationship among the parameters in the same patient groups. The serum concentrations of TNF-alpha, IL-1 beta, sIL-2R, I L-6, IL-8, and CRP (18.6 +/- 3.7 pg/mL, 10.1 +/- 2.8 pg/mL, 730 +/- 220 U/mL, 11.4 +/- 3.3 pg/mL, 23.9 +/- 8.3 pg/mL, and 49.9 +/- 19.5 mg/dL, resp) were significantly higher in newly diagnosed MM patients than ill healthy controls (P <.0001). All of the parameters were found to be significantly reduced after chemotherapy. In conclusion, we found that after the VAD therapy, the level of these cytokines which are thought to play an important role in the pathogenesis of MM was significantly suppressed. This is the first study demonstrating strong impact of VAD treatment on circulating mediators of sIL-2R and IL-8 levels parameters.
