Öğe Ghrelin and lipid levels in panic disorder before and after treatment and their relationship with agoraphobia(Medicinska Naklada Zagreb, 2015) Gul I.G.; Cumurcu B.E.; Karlidag R.; Turkoz Y.Background: We aimed to evaluate serum ghrelin (GHR) levels and lipid profile in panic disorder (PD), with and without agoraphobia, and to compare these parameters before and after treatment. Subjects and methods: The GHR and lipid profiles were measured in blood samples taken from 31 PD patients with agoraphobia, 22 PD patients without agoraphobia, and 53 control group subjects. 23 of the 53 patients who were prescribed 20 to 40 mg/day paroxetine had continued treatment. The 23 patients who had continued treatment were measured again at the end of twelve weeks. Results: The GHR and triglyceride (TRG), total cholesterol (Total-C), low-density lipoproteins (LDL-C), and very low-density lipoproteins (VLDL-C) levels were higher in the PD with agoraphobia group than the PD without agoraphobia and control groups. The 23 patients that had continued their treatment were re-evaluated, and the serum GHR, Total-C levels, and BMI after treatment were significantly decreased, compared to the values before treatment. Conclusions: There may be a pathophysiological relationship between the GHR and lipid profiles that interact with each other in PD. R.Aim Sexual dysfunction has been found in many disorders that are chronic or disabling. The aim of this study was to evaluate the sexual satisfaction levels, sexual function and their relationship with the mental state in a group of patients being followed-up with a diagnosis of Behçet's disease (BD). Method A total of 50 BD patients and 50 control-group subjects were administered the Hamilton Depression Rating Scale (HDRS), Hamilton Anxiety Rating Scale (HARS), Golombok Rust Sexual Satisfaction Scale (GRISS) and Arizona Sexual Experiences Scale (ASEX). Results The ASEX, GRISS total, HDRS and HARS scores were significantly higher in the patient group than the control subjects (P = 0.0001, P = 0.007, P = 0.0001, P = 0.0001 respectively). Sexual dissatisfaction was seen in 40 (80%) of the patient-group and 16 (32%) of the control-group subjects according to the GRISS (P = 0.0001). Female study participants had higher mean scores than the control subjects for the ASEX, GRISS total scores and the GRISS satisfaction, avoidance, vaginismus and orgasm subscale scores (P = 0.0001, P = 0.002, P = 0.02, P = 0.001, P = 0.006, P = 0.03 respectively). Male study participants had different mean scores for the controls regarding the ASEX scores and the GRISS impotence, premature ejaculation, satisfaction and frequency subscale scores (P = 0.01, P = 0.01, P = 0.0001, P = 0.03, P = 0.007 respectively). Discussion The negative effect of the disorder on the biological and functional status and daily living activities in BD patients also influences the patients' sexual experiences and satisfaction. The negative effects of chronic diseases such as BD should therefore be defined and the disorder evaluated from a wide perspective during the treatment process. © 2012 The Authors. Journal of the European Academy of Dermatology and Venereology © 2012 European Academy of Dermatology and Venereology.Öğe Plasma iron, copper, zinc levels in patients experiencing delirium following coronary artery bypass grafting(2008) Cumurcu B.E.; Karlidag R.; Unal S.; Sezer O.H.; Battaloglu B.; Mendil D.; But K.Aim: To investigate the role of plasma iron (Fe), copper (Cu), and zinc (Zn) levels on delirium in patients following coronary artery bypass grafting (CABG). Methods: Twelve of the 50 patients included in the study showed signs of delirium during postoperative follow-up. We diagnosed delirium using DSM-IV-TR (Diagnostic and Statistical Manual of Mental Disorders) criteria. Delirium Rating Scale (DRS) was used to estimate delirium severity. In both groups pre-and postoperative plasma Fe, Cu, and Zn levels were analyzed with atomic absorption spectrometry (FAAS) method and compared statistically. Results: There was no significant difference between the groups for preoperative plasma Fe and Zn levels, while the Cu level was statistically significantly lower in the delirium group (p<0.05). The postoperative plasma Fe and Zn levels were statistically significantly higher in the delirium group (p=0.0001). The Cu level was statistically significantly higher in the delirium and non-delirium group (p=0.0001, p<0.05, respectively). The comparison of postoperative plasma Fe, Cu and Zn levels between the two groups showed a statistically significantly higher level in the delirium group compared to the non-delirium group (p=0.0001). The postoperative plasma Cu/Zn levels were statistically significantly higher in the delirium group (p<0.05). Conclusion: It may be suggested that the increased postoperative plasma levels of these trace elements may play a role in delirium seen after CABG. © Universitätsverlag Ulm GmbH 2009.Öğe The relationship between late onset bipolar affective disorder which responds na valproate and hypertension: A case report(2012) Cumurcu B.E.; Karlida? R.; Özdemir S.; Gül I.G.; Öztoprak E.Bipolar affective disorders beginning after the age 50 are named late onset bipolar disorder. It is indicated that the etiology of late onset bipolar disorders can be primary or secondary to another factor. Therapeutic agents, infections, metabolic instability, brain tumors and aneurysm, epilepsy, toxins, traumatic brain injury, hyperthyroidism, multiple sclerosis, Cushing syndrome, dementia and cerebrovascular diseases are known as some of these etiologic factors. In this report a 55 year old women, who has been hypertension diagnosis for 3 years is observed. White matter hyperintensity has been seen in her MRI and she has had late onset bipolar affective disorder diagnose. In the light of literature the etiology of late onset bipolar affective disorder will be discussed in this report.Öğe Smoking induced worsening of myoclonic dystonia due to haloperidol: A case report(Istanbul Universitesi, 2012) Kartalci S.; Unal S.; Cumurcu B.E.; Karlidag R.Acute dystonia is a side effect of antipsychotic medication, and appears shortly after beginning treatment. The movement disorder is characterized by sustained muscle contractions that are typically slow, but rapid dystonia referred to as myoclonic dystonia has also been described. Cranial, pharyngeal and cervical muscles are generally affected causing fixation of the jaw, retrocollis, torticollis or opisthotonic posturing. Acute laryngeal dystonia (laryngospasm) with dysphonia has rarely been reported. Although known for six decades, the mechanism underlying acute dystonia is not clearly understood. It has been reported to be associated with striatal dopaminergic and cholinergic dysfunction due to the D2 receptor blockade of antipsychotics in the basal ganglia. We discuss a patient with haloperidol-induced atypical myoclonic dystonia that got worse with smoking together with the role of the cholinergic system in acute dystonia in this case report.
