Effects of Illness Perception on Self-Care Agency and Hopelessness Levels in Liver Transplant Patients: A Descriptive Cross-Sectional Study
(Sage Publications Inc, 2022) Demir, Bilsev; Demir, Ilker
Individuals with liver transplantation (LT), hopelessness, and lack of self-care may occur and change in the illness perceptions; however, no study has examined the effects of the illness perceptions on self-care agency and hopelessness levels in individuals with LT. This study was conducted to examine the effects of the illness perceptions of patients who had received LT surgery on their self-care agency and hopelessness levels. A descriptive cross-sectional study with a convenience sample (N = 120) was conducted at a center in eastern Turkey. The data were collected by using the Brief Illness Perception Questionnaire (B-IPQ), Self-Care Agency Scale (SCAS), Beck Hopelessness Scale (BHS). In the study, the mean total B-IPQ, SCAS, BHS scores was found as 57.50 +/- 3.61, 83.83 +/- 9.43, 10.19 +/- 3.81, respectively. There was a positive and significant relationship between the B-IPQ and BHS total scores.

Intracerebroventricular salusin-? infusion to rats increases hypothalamus-pituitary-testicular axis hormones
(Academic Press Inc Elsevier Science, 2021) Demir, Ilker; Tekin, Suat; Vardi, Nigar; Sandal, Suleyman
Salusin-beta (Sal-beta), which originates from preprosalusin, is a multifunctional hormone with a peptide structure. Sal-beta exists in the hypothalamus and can stimulate the pituitary gland. The present study was conducted to determine the effects of Sal-beta on hormones that play roles in the male reproductive system. Forty male Wistar Albino rats were used in the study. No infusions were performed on the control group, and infusions were applied to the infusion groups (artificial cerebrospinal fluid to the sham group, 2 and 20 nM Sal-beta to the experimental group) through intracerebroventricular infusion for 7 days at 10 mu l/hour rate. The animals were decapitated after 7 days of infusion; and the hypothalamus, testicles, and blood tissue samples were collected. The gonadotropin-releasing hormone (GnRH) mRNA levels were determined from the hypothalamus tissues by using the Real Time-PCR Method, and the serum luteinizing hormone (LH), follicle-stimulating hormone (FSH) and testosterone levels were determined using the ELISA method. Also, Hematoxylin-Eosin Staining Method was used for histopathological evaluations in the testicle tissues. As a result, Sal-beta infusion increased GnRH mRNA levels in hypothalamus tissues (p < 0.05) besides, serum LH, FSH, and testosterone levels of the rats were higher at significant levels following Sal-beta infusion compared to the control and sham group (p < 0.05). In the histological examination of the testicle tissues, Sal-beta application was found to decrease the seminiferous tubule diameter and germinal epithelial thickness (p < 0.05). This evidence is the first, indicating that Sal-beta, which is administered to male rats with central infusion, stimulates hypothalamus and pituitary tissues, and causes increased secretion of male reproductive hormones.

Relationship Between Cognitive Function and Balance, Fatigue, and Physical Activity Levels in Patients with Liver Cirrhosis
(Mdpi, 2026) Demir, Ilker; Demir, Bilsev
Background: Liver cirrhosis is a chronic and progressive disease, and it affects liver parenchymal cells. Although it is known that during the course of this disease, cognitive function, balance, and physical activity levels decrease, and fatigue severity increases, the relationship between these variables remains unclear. This study is the first to examine the relationship between cognitive function level and balance, fatigue, and physical activity levels in patients with liver cirrhosis and was conducted to provide a new perspective on treatment. Aim: This study aims to investigate the relationship between cognitive function and balance, physical activity, and fatigue in patients with liver cirrhosis. Method: A total of 132 patients were included in the study. Cognitive function levels of the patients were measured with the Montreal Cognitive Assessment Scale, balance performance using the One-Legged Stance Test and timed up and go tests, physical activity levels with the International Physical Activity Scale-Short Form, and fatigue levels with the Fatigue Severity Scale. Results: Correlation analyses showed that cognitive function (MoCA) was significantly associated with static balance (r = 0.232, p = 0.007) and fatigue severity (r = -0.297, p = 0.001), whereas no statistically significant relationships were observed with dynamic balance (r = -0.068, p = 0.441) or physical activity (r = -0.011, p = 0.903). Multivariable regression analyses indicated that disease duration (beta = 0.02, p = 0.009) and exercise habits (beta = 0.65, p = 0.031) were independently associated with cognitive function (MoCA), while disease duration was also independently associated with static balance performance (beta = 0.08, p = 0.002). Conclusions: These findings indicate statistically significant associations between cognitive function, static balance, and fatigue severity, whereas no significant associations were observed with dynamic balance or physical activity. These relationships should be interpreted as associative rather than causal and suggest that cognitive status may be clinically relevant when evaluating balance performance and fatigue burden in patients with cirrhosis.
