Öğe Calcium Deposits of the Metacarpophalangeal Joint on the Fourth and Fifth Finger(2022) Ekrem, Serhat; Köse, Harun; Maraş, Zeynep; Ertem, KadirAcute calcium deposition disease is characterized by calcium hydroxyapatite crystal deposition within tendons and surrounding tissue of joints. It can manifest acute or chronic. It’s most common symptom is pain in affected area. Two patients applied to our outpatient clinic with complaints of hand pain. Calcific masses were detected by radiological imaging. After radiological imaging and laboratory examinations the patients diagnosed acute calcium deposition disease. One of the patients had ongoing symptoms for 2 years, the other one had ongoing symptoms for 3 years. Both of the patients had tried conservative treatment multiple times. These patients were treated surgically because of long-standing symptoms. Although acute calcium deposition disease usually resolves spontaneously with conservative treatment, patients with recurrent or persistent lesions can be treated surgically.Öğe Endoscopy assisted carpal tunnel syndrome release surgery: Case series(2024) Balıkçı Çiçek, İpek; Ertem, Kadir; Sağır, Ahmet; Ekrem, SerhatObjective: Endoscopy-assisted surgical procedures for CTS has become increasingly prevalent since 1990s. In endoscopy- assisted carpal tunnel release surgery (EACTRS), it has been proposed that nerve damage likelihood is similar to other procedures. It has also been proposed that EACTRS is especially associated with smaller scar size, painless and faster recovery period. We aimed to determine our clinical and functional results in patients with EACTRS and compare them with the literature. Method: We evaluated 25 (39 hands ) patients who underwent endoscopy-assisted carpal tunnel release surgery between 2012 and 2017. In all patients, demographic characteristics, preoperative- postoperative functional scores, patient satisfaction levels and time required to return-to-work were recorded. Results: The endoscopy-assisted carpal tunnel release surgery had positive significant effect on patient satisfaction scores(p<0.05). A statistically significant difference was found between those with and without Double Crush syndrome (DCS) in favor of those without DCS in terms of pre- and postoperative BTCQ symptom severity score (p<0.05). We found only two scar tenderness and two revision Carpal Tunnel release. Conclusion: The EACTRS is an effective method in the treatment of carpal tunnel syndrome. Clinical outcomes are comparable to other methods. Those with DCS, those with comorbidities, and female gender were evaluated as negative prognostic factors in patients who underwent EACTRS.Öğe Flexion contracture in 2-5 digits following hypothenar flap(Bayrakol Medical Publisher, 2022) Kose, Harun; Ekrem, Serhat; Sagir, Ahmet; Turkmen, Ersen; Ertem, KadirWe aimed to discuss a case of severe flexion contractures affecting 2nd-5th fingers as a late complication of a hypothenar skin flap that we applied to a patient with a fingertip injury that ended with tissue loss in the tip of the fifth finger of the right hand. Soft tissue defects of the little finger are challenging, especially when a bone, tendon or vascular pedicle is exposed because of trauma. The hypothenar random flap is easy to harvest and has a good color and texture match to the little finger pulp. For this complication, which did not improve sufficiently with physical therapy methods, contractures were tried to be eliminated by open surgical release of the 2nd-5th fingers to the proximal interphalangeal joints.
