Öğe Abnormal eating attitudes and the level of impulsiveness in morbid obese patients who are candidates for bariatric surgery(2018) Erbay, Lale Gönenir; Bozkurt, PelinObesity is a chronic disease needs to be investigated not from the biological point of view alone, as it also has medical and psychological outcomes. While evaluating obese patients, we come across many psychopathological conditions. The present study aimed to investigate abnormal eating attitudes and the level of impulsiveness in morbid obese patients undergoing bariatric surgery. The present study comprised a total of 60 subjects, of whom 30 were morbid obese patients presented to the Inonu University School of Medicine, Department of General Surgery for bariatric surgery and remaining 30 were healthy controls. The participant’s abnormal eating attitudes were measured by “Eating Attitudes Test”, and the level of impulsiveness was measured by “Barratt Impulsiveness Scale (BIS-11)”. Comparing morbid obese patients with the control group, no significant difference was determined in terms of age (p=0.712). The score of abnormal eating attitudes test was significantly higher in the morbid obese patient group as compared to the control group (p=0.00). However, there was no difference between the groups in terms of the scores of Barratt impulsiveness scale (p=0.242). Individuals’ eating habit is one of the factors considered to be important for the start and maintenance of obesity. Moreover, high level of impulsiveness as well might be an important factor for the development of morbid obesity. However, in the present study, no statistically significant difference was determined between the groups despite higher scores of the patient group vs. the control group indicating the level of overall impulsiveness. Psychotherapeutic interventions focusing on impulsive characteristics and abnormal eating attitudes of the morbid obese patients might be helpful in medical and surgical treatments of these patients.Öğe Acute Urinary Retention Cuased by Biperiden in a Female Patient: A Case Report(Turgut Özal Tıp Merkezi Dergisi, 2015) Erbay, Lale Gönenir; Aydın, Yusuf; Yeşil, Bahar; Karlıdağ, RıfatÖz: Biperiden, psikiyatride çoğunlukla antipsikotik ilaçlara bağlı ortaya çıkan extrapiramidal sistem yan etkileri gidermek için kullanılan antikolinerjik bir ilaçtır. Genitoüriner sistem yan etkisi ön planda olan oxybutynin, flavoxate ve dicyclomine gibi antikolinerjiklerden farklı olarak, antiparkinson etkileri ön planda olan benztropin, procyclidine ve trihexyphenidyl ile birlikte gruplandırılır. Yan etki olarak üriner retansiyona sebeb olabileceği bilinmektedir. Ancak bu yan etkinin özellikle erkeklerde ortaya çıktığı görülmüştür. Prostat büyümesi olan erkeklerin bu konuda daha fazla risk altında olduğu raporlanmıştır. Biz, literatür taramamızda kadınlarda biperiden kullanımına bağlı üriner retansiyon vakasına rastlamadık. It is grouped together with benztropin, procyclidin, and trihexyphenidyl, which have prominent antiparkinson effects, rather than anticholinergic drugs like oxybutynin, flavoxate, and dicyclomine, which have prominent genitourinary system side effects. It is well known that it can cause urinary retention as a side effect though this effect is known to be more dominant in males. It is reported that males with prostate hypertrophy have more risk for urinary retention. In the literature, we could not find any female patients developing urinary retention due to use of biperiden. The aim of this study was to determine the frequency of sexual dysfunction in patients diagnosed with morbid obesity.Methods: The patient group included in the study consisted of 78 morbidly obese patients with BMI?40 kg/m2 and 68 healthy individuals with normal BMI. The data were obtained by using the sociodemographic information form filled by the participants, Beck anxiety scale, Beck depression scale and Golombok Rust Sexual Satisfaction Inventory.Results: Obese individuals were found to have sexual dysfunction compared to individuals with normal body BMI (p<0.05). When the Golombok Rust subscale scores of obese men were compared to those of normal-weight individuals, a significant deterioration was found in all subscale scores, including frequency, communication, satisfaction, avoidance, touch, impotence and premature ejaculation. In addition, the anxiety and depression scores of obese individuals were shown to be higher than those of normal-weight individuals (p<0.05).Conclusion: Morbidly obese individuals should be evaluated in terms of sexual functions. We aimed both to discuss the relationship between psychiatric disease and inflammatory factor over this case and to make contribution to the literature. A 57-year-old male patient. He had experienced depressive symptoms nearly 5 months ago after 15 days of adalimumab therapy started for ankylosing spondylitis. Meanwhile, the symptoms of the patient, who continued adalimumab therapy once every two weeks, progressed and accompanied by psychotic symptoms such as disorganized speaking and behavior. The patient was admitted to the psychiatry clinic; however, his complaints did not significantly improve with treatment but rapidly regressed after discontinuation of adalimumab therapy. To our knowledge, there is limited data on the mood episodes triggered by adalimumab. In the present case, presence of a connection between symptom onset and the date of starting the medication, as well as rapid symptom regression with discontinuation of the medication, was considered as “potential adverse drug effect” according to the Naranjo Algorithm. This suggests that alteration in proinflammatory processes such as Tumor Necrosis Factor (TNF) might be associated with the onset of mood episodes. Further studies would be important in understanding the role of immune system in the pathophysiology of depression, particularly the sub-types such as psychotic depression.Öğe Remission of obsessive-compulsive symptoms in hypomanic period in a patient with comorbid bipolar affective disorder and obsessive compulsive disorder: a case report(Turgut Özal Tıp Merkezi Dergisi, 2016) Erbay, Lale Gönenir; Şahin, Serhat; Öztaş, Gülşen; Kartalcı, ŞükrüAbstract Given the prevalence rates of obsessive compulsive disorder and the bipolar affective disorders in the general population, one would expect the co-occurrence of these syndromes to be rare. Yet, findings have revealed extremely high rates of comorbidity in obsessive compulsive disorder with both depressive disorders (50%) and bipolar disorder (10%) and shown that obsessive compulsive disorder is the most common anxiety disorder in patients with bipolar disorder. This situation makes it difficult to recognize the changing clinical disease and prognosis while response to treatment can also be adversely affected. Clarifying the phenomenological features of obsessive compulsive disorder-bipolar affective disorder comorbidity has important etiological and treatment implications. In this article, we discuss the pathophysiological importance of the improvement of obsessive compulsive symptoms during hypomanic episodes in a bipolar disorder patient, who had comorbid obsessive-compulsive disorder.
