    Investigation of the relationship between hospitalization periods in patients with acute coronary syndrome

    Aim: The aim of this study was to investigate the relationship between high sensitivity C-reactive protein (HsCRP), cardiac markers, D-dimer and brain natriuretic peptide (BNP) measured at admission to the emergency department in patients with acute coronary syndrome (ACS).Material and Methods: In our clinic, patient records were scanned backwards. The records of the patients who presented to the emergency department with chest pain were evaluated. Patients with acute coronary syndrome were included in the study. Laboratory values, echocardiogram and electrocardiograms of these patients were examined.Results: 88 patients with acute coronary syndrome were included in our study. According to the complaints of the patients, 93.2% (n=82) had chest pain, 45.5% (n=40) sweating, and 42% (n=37) had left arm pain at their admission to the emergency unit. The mean stick blood glucose value of the patients who came to our emergency department was 157±75.38 mg / dl (37.00-524.00 mg / dl). Biochemical laboratory values measured from venous blood taken at the time of arrival of patients, mean HsCRP 11.67±15.4 mg / l, insulin 14.87±29.53 IU / ml, glucose level 139.11±74.3 mg / dl, CK 426.9±12x8.9 mg / dl and CK-MB were determined as 41.1±46.8 mg / dl.Conclusion: We conclude that there is no correlation between cardiac parameters, DDIM, glucose, insulin and HsCRP levels in the in-hospital prognosis and there is a relationship between the measured BNP levels and the length of hospital stay.Keywords: Emergency service; trauma; blood glucose; DDIM.

    Mid-term outcome of wide resection in musculoskeletal fibrosarcoma patients

    The aim in this study, musculoskeletal system fibrosarcomas which are rare malignant soft tissue tumors that originate from fibroblasts, was to evaluate the mid-term outcome of patients that were diagnosed, treated and followed-up at our clinics. Included in the study were 12 patients treated for fibrosaecoma at our clinics between 2014 and 2017. The patients were evaluated in terms of age, location of tumor and time of resection, and were followed-up for mid-term recurrence. A wide resection was performed on all patients. No recurrence was found in all but one patient during follow-up. Musculoskeletal system fibrosarcomas are rare but represent high mortality and morbidity risks since the diagnosis is commonly delayed. The most frequently seen symptom is a painless mass reaching large dimensions. Early diagnosis plays a major role in prognosis, as is the case with other malignant tumors. In conclusion, we suggest that malignancy should be considered in the presence of giant, fixed and painful tumors, and a wide resection should be applied.

    Our clinical outcomes in patients operated with the diagnosis of pigmented villonodular synovitis

    Abstract: The aim of this study was to evaluate the clinical and surgical results of 21 cases treated with pigmented Villonodular Synovitis in our clinic between 2010 and 2018 and to share these results with the literature. The 5-year follow-up results of 21 cases with pigmented Villonodular Synovitis were evaluated retrospectively. Only patients undergoing surgical treatment were included in the study. The patients were investigated in terms of age, gender, anatomical region, size of the tumor, a surgical technique applied and recurrence. The involved joints are in turn; seven knees, five ankles, four feet, two hands, two hips, and one shoulder joints. The patients’ ages ranged from 9 to 81 years (mean 45). 11 of them were male, and 10 were female. Clinically; Pain and swelling in the involved joints were the most common complaints, and the most common symptom was the compression effect of the mass. The most commonly involved anatomic region was the knee joint. The majority of patients had no history of trauma. Recurrence was found in 23.8% of patients who underwent surgical treatment. Pigmented Villonodular Synovitis is a middle-aged disease that causes severe morbidity and compromises the quality of life. Arthroscopic and open surgical synovectomy can be performed successfully in the treatment of this condition.

    Relation of vitamin D levels with sunlight, genetics and nutrition

    Abstract: Aim: We aimed to reveal the effects of reduced exposure to sunlight and insufficient nutrition due to urbanization and altered cultural life on Vitamin D levels in medical students. Materials and Methods: A hundred (56 female, 44 male) first year medical students were included in our study. A vitamin D screening form, which included 16 questions, was used to obtain data regarding medical students’ genetic, nutrition and cultural life. Results: Out of 100 medical students, 25 hydroxyl vit- D3 levels was below 5 ng/ml in 10%, 5-10 ng/ml in 10%, 10-20 ng/ml in 30%, 20-30 ng/ml in 40% and above 30 ng/ml in 10% medical students. While it has been found out that urbanization causes vitamin D deficiency and insufficiency in both females and males by negatively affecting sunlight exposure and nutrition, females have less sunlight exposure and their vitamin D levels are lesser when compared with males. Conclusions: In conclusion, urbanization decreases exposure to sunlight and changes nutritional habits and forms a basis for vitamin D deficiency. So, this shows that lack of vitamin D, which is a major cause of bone fractures in older ages, is actually mainly seen in younger ages.
