Öğe Apart From Surgical Procedures, Another Important Point to Note in Lacrimal Canalicular Lacerations: Traumatic Retinal Tears(Lippincott Williams & Wilkins, 2019) Demirel, Soner; Dikci, Seyhan; Firat, Penpe Gul; Genc, Oguzhan; Mutlu, KayhanBackground: To evaluate fundus examinations in terms of traumatic retinal tears or dialysis that may be related to canalicular lacerations (CLs). Materials and Methods: This retrospective study was conducted on patients with CL repair from June 1, 2009 to May 30, 2018. The authors collected the data from the patients' medical records, including their demographic details, mechanism and setting of injury, associated ocular injuries, type of stent used, setting of repairs, duration of follow-up, complications, pre- or intraoperative findings of biomicroscopic and fundus examinations, refractive errors, postoperative follow-up of fundus examination, and management. Results: Sixty patients (43 males and 17 females) were included in the study. The mean age of all the patients was 27.50 +/- 21.72 (min: 1-max: 94) years. Isolated CL was found in 38 patients. Nonisolated CL, which means with some additional pathologies, were found in 22 patients (37%, approximately 1/3). Retinal follow-up records were available in 49 patients with a mean follow-up period of 7.48 +/- 9.82 months. Retinal tears were seen in 3 patients (0.05%) with no predisposing risk factors. Two of these tears were found in the follow-up period after the traumas. Conclusion: Consequent retinal tears after canalicular lacerations were not strongly addressed in previous works. In this study, the authors aimed to view CL from a different perspective. Canalicular laceration is also an ocular trauma that may threaten vision. The authors emphasized that performing detailed fundus examinations and follow-up in all canalicular traumas is necessary.Öğe Assessment of ganglion cell complex, macular thickness, and optic disc parameters in keratoconus patients(Wolters Kluwer Medknow Publications, 2020) Orman, Gozde; Firat, Penpe Gul; Doganay, Selim; Doganay, DeryaPURPOSE: Keratoconus (KC) is bilateral noninflammatory corneal disorder characterized by progressive corneal thinning, protrusion, and scarring. The purpose of this study was to evaluate ganglion cell complex(GCC), macula thickness(MT) and optic head disc parameters in keratoconus patients. METHODS: A hospital based prospective clinical case series was performed in Inonu University School of Medicine. 52 eyes of 52 keratoconus patients and 50 eyes of 50 normal patients were enrolled. RESULTS: There is no statistically significant in MT between groups. GCC in nasal superior, temporal superior and temporal inferior 9 mm from macula were found statistically significant decrease in keratoconus group (p<0,05). In optic disc analysis fifth and the eleventh clock-hour quadrants of peripapiller retina nerve fiber layer and cup area ratio were found statistically significant decrease in keratoconic eyes (p<0,05). Retinal vasculitis is a group of vision-threatening disorders, in which autoimmunity is thought to play a role in pathogenesis. We present the case of a patient who was diagnosed with retinal vasculitis and who was followed up for mucosal LP. LP has not been reported as a cause of retinal vasculitis in the literature. We believe that the retinal vasculitis in this case was related to LP because cellular immunity plays a role in the pathogenesis of both entities.Öğe Canalicular Incisional Approach for Recanalization of Punctum: A Modified Surgical Treatment for Acquired Complete Upper and Lower Punctum Occlusions as an Alternative to Bypass Methods(Lippincott Williams & Wilkins, 2020) Demirel, Soner; Ozturk, Emrah; Dikci, Seyhan; Firat, Penpe GulPurpose: To define the success of surgical technique used in the treatment of epiphora that is caused by bi-canalicular acquired complete punctum occlusion. Materials and Methods: The study is a retrospective consecutive case series, who underwent canalicular incisional approach for recanalization (CIAFOR) for acquired bi-canalicular punctum occlusions. The authors have used this technique in the cases not possible to perform a punctum dilatation and not possible to determine the punctum location exactly due to severe fibrosis. Functional and anatomical success was evaluated by the absence of epiphora and with lavage at post-operative 6th months. Results: Seven eyes of 5 patients (3 males and 2 females) were included in the study. The ages of the patients were 70, 65, 45, 64, and 70 years, respectively. Current follow-up periods vary between 6 and 23 months. Although functional and anatomical success achieved in all eyes, complication such as accessory punctum observed. Conclusions: CIAFOR seems to be a successful and simple surgical technique to treat acquired total punctum occlusion.Öğe Comparison of ganglion cell and retinal nerve fiber layer thickness in primary open-angle glaucoma and normal tension glaucoma with spectral-domain OCT(Springer, 2013) Firat, Penpe Gul; Doganay, Selim; Demirel, Ersan Ersin; Colak, CemilThe aim of this study was to evaluate the macular thickness (MT), ganglion cell complex (GCC), and circum-papillary retinal nerve fiber layer (RNFL) thickness in primary open-angle glaucoma (POAG) and normal tension glaucoma (NTG) with spectral domain optical coherence tomography (SD-OCT). A total of 169 subjects were enrolled: 52 normal subjects, 61 with POAG, and 56 with NTG. Spectral-domain optical coherence tomography (SD-OCT) was used to analyze MT, GCC, and RNFL thickness. To compare the discrimination capabilities between the MT, GCC, and RNFL thickness measurements, we analyzed the areas under the receiver operating characteristic (ROC) curves (AUCs). The relationships between GCC and RNFL measurement and also the relationships of the groups, with age, gender, GCC, and RNFL thickness were assessed. Normal subjects showed the thickest superior and inferior GCC, followed by in order NTG and POAG (p < 0.05). While there was a statistically difference in MT value of the normal subjects and the glaucoma patients (p < 0.05), MT value did not differ between POAG and NTG (p < 0.05). RNFL thickness parameters were significantly greater in normal subjects, followed in order by the NTG, and POAG (p < 0.05). Between the normal and entire glaucoma groups, all GCC and RNFL parameters showed the similar discrimination power. RNFL thickness parameters correlated significantly with all GCC thickness (p < 0.05). Superior RNFL thickness was the only independent variable between the POAG and NTG patients (odds ratio (OR) 0.942, p = 0.004, 95 %CI 0.905-0.981). SD-OCT evaluation results suggest higher GCC and RNFL parameters for NTG than POAG.Öğe Comparison of Nd:YAG laser (532 nm green) vs diode laser (810 nm) photocoagulation in the treatment of retinopathy of prematurity: an evaluation in terms of complications(Springer London Ltd, 2020) Dikci, Seyhan; Demirel, Soner; Firat, Penpe Gul; Yilmaz, Turgut; Ceylan, Osman Melih; Bag, Harika Gozde GozukaraPurpose To compare the anterior and posterior segment complications of diode (810 nm) laser photocoagulation (LPC) and Nd:YAG (532 nm green) LPC in the treatment of retinopathy of prematurity (ROP). Patients and methods The 84 eyes of 43 patients treated with diode LPC (group 1) and 58 eyes of 31 patients treated with Nd:YAG LPC (group 2) for ROP in our clinic were enrolled in the study. Medical records of all patients were investigated retrospectively. The patients in each group were examined in terms of birth weights, gestational weeks, stage of retinopathy, number of lasers pots, laser parameters, and anterior and posterior complications of LPC. Results The mean birth weeks of group 1 patients were postmenstrual 27.7 +/- 2.5 (23-33), while the mean birth weights were 1006.0 +/- 334.5 (540-1980) grams. The mean birth weeks of group 2 patients were postmenstrual 27.4 +/- 2.6 (23-33), while the mean birth weights were 1073.8 +/- 329.2 (480-1720) grams. The mean numbers of laser spots were 1036.0 +/- 515.2 (430-2410) in group 1 per eye, while the mean numbers of laser spots were 1085.4 +/- 526.0 (445-2530) in group 2 per eye (p >= 0.05). Additional laser application was performed in four eyes (4.8%) in group 1 and four eyes (6.9%) in group 2. Four eyes (4.8%) treated with diode LPC and one eye (1.7%) treated with Nd:YAG laser developed retinal detachment. Two eyes of a patient (3.4%) applied Nd:YAG LPC developed cataract. Conclusion Cataract may develop when Nd:YAG laser is used; however, posterior segment complications may be more likely to appear with the use of diode laser in these cases.Öğe Computed Tomography Diagnostic Abilities for Open-Globe Injuries in Pediatric Versus Adult Patients(Lippincott Williams & Wilkins, 2021) Dikci, Seyhan; Yildirim, Ismail Okan; Firat, Murat; Firat, Penpe Gul; Demirel, Soner; Yilmaz, Turgut; Tuncer, IlknurObjectives The aim of this study was to compare the role of computed tomography (CT) in the diagnosis of open-globe trauma and intraocular foreign body (IOFB) in pediatric and adult age groups. Methods Medical records of cases with open-globe trauma at Inonu University Hospital's Ophthalmology Emergency Service were retrospectively evaluated. Preoperative orbital CT images of the cases obtained at emergency services and their clinical and/or surgical findings were compared in pediatric and adult groups. Results We included 47 eyes of 47 cases aged 18 years and below (pediatric group) and 85 eyes of 82 cases over 18 years (adult group). The mean +/- SD age was 10.80 +/- 5.11 years (range, 2-18 years) in the pediatric group and 46.34 +/- 19.01 years (range, 19-82 years) in the adult group. Computed tomography images revealed 21.7% of the cases with corneal lacerations, 55.5% with scleral lacerations, and 91.6% with corneoscleral lacerations in the pediatric group, whereas the respective numbers were 48.4%, 66.6%, and 61.9% in the adult group. The detection rates of corneal penetrations and vitreous hemorrhage with CT were significantly lower in the pediatric group than in the adult group (P < 0.05). The CT scans diagnosed 66.6% of the pediatric cases and 90% of the adult cases with an IOFB. Conclusions Corneal lacerations and IOFBs can be missed, especially in the pediatric group, because the eye is smaller in adults. Pediatric patients with a history of ocular trauma should undergo an examination under general anesthesia followed by surgical exploration if necessary.Öğe Correlation between intraocular pressure obtained with water drinking test versus modified diurnal tension curve measurement in pseudoexfoliation glaucoma(Springer, 2021) Firat, Penpe Gul; Dikci, Seyhan; Firat, Ilknur Tuncer; Demirel, Soner; Firat, Murat; Ozturk, Emrah; Gok, Zarife EkiciPurpose The aim of this paper was to study the correlation and agreement between the intraocular pressure (IOP) peak value and fluctuations detected with the modified diurnal tension curve (mDTC) and the water drinking test (WDT) in pseudoexfoliation glaucoma (XFG) patients. Methods This prospective observational study enrolled 42 eyes of 42 XFG patients. The IOPs were measured at 2-h intervals from 8 am to 4 pm with a Goldmann applanation tonometer by a single observer to establish the mDTC. The WDT was then performed between 4 and 5 pm on the same day and the IOP was measured 4 times at 15-min intervals after water ingestion. The IOPpeak, IOPmean, IO (min), and IOPfluctuation were measured with both the mDTC and WDT. The paired sample t test, Spearman's correlation coefficient and Bland-Altman plots were used for statistical analysis. Results The mean age of the 42 patients consisting of 24 females and 18 males was 66.9 +/- 6.8 years, and the mean central corneal thickness was 517.7 +/- 29.1 mu m. The mean values with the mDTC and WDT measurements were 15.05 +/- 2.75 mmHg and 17.17 +/- 3.25 mmHg (p <= 0.0001, r = 0.884) for IOPmean, 16.76 +/- 3.45 mmHg and 18.92 +/- 3.94 mmHg (p <= 0.0001, r=0.787) for IOPpeak, and 13.61 +/- 2.56 mmHg and 15.11 +/- 2.84 mmHg (p <= 0.0001, r=0.824) for IOPmin, respectively, and a positive correlation was present between these values. Conclusion There was a positive correlation between the peak, minimum, and mean IOP values determined using the mDTC and WDT in treated XFG patients. WDT can be used as an alternative in the assessment of the IOP in these patients as a more practical method.Öğe The effect of apricots on the experimental cataract model formed by sodium selenite(Pergamon-Elsevier Science Ltd, 2013) Doganay, Selim; Duz, Cem; Firat, Penpe Gul; Cankaya, Cem; Kutukde, Derya; Cigremis, YilmazThis study was designed in order to investigate whether sun dried apricots have a preventive effect on the experimental cataract model formed by sodium selenite in rats. Fifty-nine Spraque-Dawley rat pups were divided into three groups. Group I (control group) consisted of twenty rat pups, born from the rats nourished ad libitum. Group 2 consisted of 18 newborn rats, born from the rats nourished ad libitum with 10% sun dried natural apricots. Group 3 consisted of 21 newborn rats, born from the rats nourished ad libitum. Subcutaneous (30 nmol/gr) sodium selenite injection was applied to all the newborn rats except the control group (Group 1) on postpartum day 10. Cataract development was graded by slit-lamp examination and photography. Encapsulated lenses were analyzed for reduced glutathione (GSH) and malondialdehyde (MDA), a marker of lipid per oxidation. Lenses were also analyzed for total nitrite (TN). The presence of oxidative stress in selenite cataract development and its prevention by sun dried apricots. (C) 2013 Elsevier Ltd. Iron deficiency anemia is an important public health problem. Also it is considered to be a risk factor for many diseases. The study demonstrates the iron deficiency anemia frequency in glaucoma patients and compares with the normal subjects. We aimed to determine the iron deficiency anemia frequency in glaucoma patients. Methods. Prospective, controlled study in a single university hospital setting. A total of 130 normal subjects (Group 1) and 131 glaucoma patients (Group 2) were included. The erythrocytes parameters, hemoglobin, red blood cell, hematocrit, mean corpuscular volume, mean corpuscular hemoglobin concentration, mean corpuscular hemoglobin, and red blood cell distribution width, and iron status indicators, Fe (iron), total iron binding capacity, and ferritin of the cases, in normal subjects and glaucoma patients were compared. Results. There was no statistically significant difference for the erythrocyte parameters between the groups (p >= 0.05). The number of the patients with iron deficiency anemia in both groups was similar. No statistically significant difference was found in the comparison of erythrocyte parameters and iron status indicators values according to the number of antiglaucomatous agents and visual field changes according to the presence of anemia in Group 2 (p >= 0.05). A statistically significant difference was found only in MCH when the erythrocyte parameters and iron status indicators values of the cases in glaucoma patients were compared with the glaucoma duration (p<0.05). Conclusion. The iron deficiency anemia frequency was like the normal population in glaucoma patients.Öğe Evaluation of metabolite changes in visual cortex in diabetic retinopathy by MR-Spectroscopy(Elsevier Science Inc, 2012) Ozsoy, Ercan; Doganay, Selim; Dogan, Metin; Alkan, Alpay; Firat, Penpe GulPurpose: To evaluate metabolite changes in the visual cortex of diabetic patients with nonproliferative or proliferative diabetic retinopathy by Magnetic Resonance Spectroscopy (MRS). Materials and Methods: 15 normal subjects (group 1), 15 patients with diabetes who did not have diabetic retinopathy (group 2), 15 patients with nonproliferative diabetic retinopathy (NPDR) (group 3), and 15 patients with proliferative diabetic retinopathy (PDR) (group 4) were included in the study. Furthermore, diabetic patients were divided into two groups according to HbA1c levels (Group A: 20 patients, HbA1c <8%; Group B: 20 patients, HbA1c >8%). In all cases' left visual cortex, amounts of N-acetyl-aspartate (NAA), choline (Cho), and creatine (Cr) were measured by MRS. NAA/Cr, Cho/Cr, and NAA/Cho ratios were calculated. Furthermore, all cases' complete blood count (CBC) and biochemical parameters were evaluated. Results: There was no statistically significant difference for NAA/Cr, Cho/Cr, and NAA/Cho ratios between groups 1. 2, 3, and 4 (P>0.05). However there was a statistically significant difference for NAA/Cr and NAA/Cho ratios between groups A and B (P<0.05). There was no statistically significant difference for Cho/Cr ratio between groups A and B (P>0.05). Conclusion: Although NAA/Cr and NAA/Cho ratios decrease in the visual cortex while diabetic retinopathy progresses, these decreases are not statistically significant. While HbA1c levels increase, the NAA concentration decreases in the visual cortex which indicates neuronal loss. The metabolite changes in the visual cortex are associated with acute events rather than chronic. (c) 2012 Elsevier Inc. Methods: Eighty eyes of 53 patients with PXS (Group 1), 80 eyes of 57 patients with PXG (Group 2) and 80 eyes of 45 control cases (Group 3) were included in the study. Anterior chamber depth, volume (ACV), angle (ACA) width, central corneal thickness (CCT), central 3.0, 5.0 and 7.0 mm corneal volumes and pupil diameters were compared between groups. Results: Although anterior chamber depths in the PXG group were significantly lower than the control group (p < 0.05), there was no statistically significant difference between the PXS group and the control group in means of anterior chamber depth values (p > 0.05). There were no statistically significant differences among PXS, PXG and the control group in mean values of ACV, ACA width, CCT, pupil diameters and central 3.0, 5.0 and 7.0 mm corneal volume (p > 0.05). Conclusions: The anterior chamber depths of PXG patients were lower than those of healthy individuals. Although the difference in anterior chamber depth between PXG patients and normal patients was statistically significant, this finding is unlikely to be of clinical significance. The anterior chamber depths of PXS patients were similar to those of healthy individuals. There were no significant differences in the means of ACV, ACA width, CCT, pupil diameter and corneal volume values on central 3.0, 5.0 and 7.0 mm among patients with PXS, PXG and healthy individuals.Öğe Evaluation of peripapillary retinal nerve fiber layer, macula and ganglion cell thickness in amblyopia using spectral optical coherence tomography(Ijo Press, 2013) Firat, Penpe Gul; Ozsoy, Ercan; Demirel, Soner; Cumurcu, Tongabay; Gunduz, AbuzerAIM: To investigate peripapillary retinal nerve fiber layer (RNFL), macula and ganglion cell layer thicknesses (GCC) in amblyopic eyes with spectral domain optical coherence tomography (SD-OCT). METHODS: Thirty six patients with a history of unilateral amblyopia and thirty two children who had emmetropia without amblyopia were included in this study. In this institutional study, 36 eyes of 36 patients with amblyopia (AE), 36 fellow eyes without amblyopia (FE), and 32 eyes of 32 normal subjects (NE) were included. RNFL, GCC and macular thickness measurements were performed with RS-3000 OCT Retina Scan (Nidek Inc CA. USA). RESULTS: The mean global thicknesses of the RNFL were 113.22 +/- 121.47, 111.57 +/- 18.25, 109.96 +/- 11.31 mu m in the AE, FE, and NE, respectively. There was no statistically significant difference for mean global RNFL thickness among the eyes (P=0.13). The mean thicknesses of the macula were 258.25 +/- 18.31, 258.75 +/- 19.54, 248.62 +/- 10.57 mu m in the AE, FE and NE, respectively. There was no statistically significant difference for thickness of macula among the eyes (P=0.06). The GCC was investigated into two parts: superior and inferior. The mean thicknesses of superior GCC were 102.57 +/- 113.32, 103.32 +/- 10.64, 100.52 +/- 15.88 mu m in the AE, FE, and NE, respectively. The mean thicknesses of inferior GCC were 103.82 +/- 112.60, 107.82 +/- 12.33, 105.86 +/- 10.79 mu m in the AE, FE and NE, respectively. There was no statistically significant difference for thickness of superior and inferior GCC between the eyes (P=0.63, P=0.46). CONCLUSION: The macular thicknesses of AE and FE were greater than the NE, although it was not statistically significant. Amblyopia does not seem to have a profound effect on the RNFL, macula and GCC.Öğe Evaluation of the effects of resveratrol and bevacizumab on experimental corneal alkali burn(Elsevier Sci Ltd, 2013) Doganay, Selim; Firat, Penpe Gul; Cankaya, Cem; Kirimlioglu, HalePurpose: To evaluate the effects of resveratrol and bevacizumab on experimental corneal neovascularization. Method: A corneal alkali burn was performed in 62 eyes of 31 male white Vienna rabbits. Resveratrol (group 1), dimethyl sulfoxide (group 2), bevacizumab (group 3) and 0.9% NaCl (group 4) were administered to both eyes of the rabbits by subconjunctival injection for 7 days. Corneal photos were taken at 15 days after alkali injury. Inflammatory index scores and neovascularization areas were calculated. Results: In bevacizumab group both inflammatory index scores and the calculation of the corneal neovascularization area was significantly less than the groups. Conclusion: The subconjunctival administration of bevacizumab inhibits corneal neovascularization effectively in the rabbit corneal alkali burn model. No effect of resveratrol to the corneal neovascularization on experimental model of the corneal alkali burn was seen at the doses of usage. (C) 2012 Elsevier Ltd and ISBI. Plasma and aqueous humor ADMA, SDMA and L-Arg levels of the participants were measured by using liquid chromatography-tandem mass spectrometry.Results: A significant increase in aqueous humor SDMA level was detected in POAG patients compared with controls (p =0.0115). No significant difference was detected in plasma and aqueous humor ADMA, L-Arg levels.Conclusion: The aqueous humor levels of SDMA are found to be associated with POAG. The result of this current study supports the role of nitric oxide pathway in glaucoma.Öğe Influence of corneal parameters in keratoconus on IOP readings obtained with different tonometers(Taylor & Francis Ltd, 2013) Firat, Penpe Gul; Orman, Gozde; Doganay, Selim; Demirel, SonerBackground Accurate intraocular pressure (IOP) measurement is important but of unsure reliability in patients with keratoconus. Different types and models of tonometers are available. This study investigated the influence of corneal parameters on IOP readings obtained by a Goldmann applanation tonometer, a non-contact tonometer and a dynamic contour tonometer. Methods IOP readings with the Goldmann applanation, non-contact and dynamic contour tonometers were obtained from 52 patients with keratoconus and from 50 normal subjects and their corneal parameters were measured using a Pentacam. Results The mean IOP measurements in keratoconus obtained with the Goldmann applanation, non-contact and dynamic contour tonometers were statistically significantly different from the mean IOP measurements in the normal subjects (p < 0.0001; p < 0.005; p < 0.0001, respectively). In the keratoconus group, the thinnest corneal thickness (TCT), steepest keratometry, the corneal curvature (CC), central corneal thickness (CCT) and the posterior corneal curvature (PCC) had a significant effect on the Goldmann applanation and non-contact tonometers but not on the dynamic contour tonometer. In the control group, thinnest and central corneal thicknesses had a significant effect on findings with the Goldmann and non-contact tonometers but not on the dynamic contour tonometer. The corneal volume (CV) had no significant effect on the three tonometers in both groups. Conclusions The corneal parameters affecting the IOP readings of the Goldmann applanation tonometers, non-contact tonometers and the dynamic contour tonometers are not the same. While the Goldmann applanation and non-contact tonometers were significantly affected by the corneal parameters that were measured, the dynamic contour tonometer was not affected by any of these corneal parameters.Öğe Modified Temporary Eyelid Margin Suture for Correction of Congenital Horizontal Tarsal Kink: A Novel Surgical Technique(Lippincott Williams & Wilkins, 2012) Demirel, Soner; Firat, Cemal; Firat, Penpe GulPurpose: Tarsal kink is a rare congenital eyelid anomaly that may lead to corneal opacity and amblyopia secondary to severe entropion. The authors aimed to describe a modified temporary lid margin suture method for tarsal kink correction. Method: In this novel surgical technique, a double-armed 6/0 prolene suture was passed through the upper eyelid and the lower eyelid margins to correct the tarsal kink under topical anesthesia. Result: The authors found that this procedure was very effective and successful for correction of tarsal kink with no incision of the tarsus or eyelid skin, providing a low risk of amblyopia. Conclusion: The modified temporal lid margin suture is a simple, practical, and effective procedure for tarsal kink correction. (Ophthal Plast Reconstr Surg 2012;28:300-302)Öğe Posterior reversible encephalopathy syndrome secondary to preeclampsia-a case report(2019) Esener, Burak; Ulucan Atas, Pamuk Betul; Firat, Penpe GulIn this article, we aimed to present a 29 year-old preeclampsia patient with sudden loss of vision in both eyes after cesarean section. In her ophthalmoscopic examination her visual acuity was at the level of light perception on both of eyes. The anterior segment examination of the patient was normal with normal eye movement in all directions. The fundus had a tilted disc, which was evident on the left. Emergency orbital, cranial MRI, carotid Doppler were requested with arterial blood pressure control of the patient. Close follow-up was recommended. Patient was diagnosed as posterior reversible encephalopathy syndrome with cortical visual loss due to vasogenic edema according to diffusion MRI result. Patient’s visual acuity was 10/10 on the right side and 16/100 on the left side after 3 days with control of arterial blood pressure.
