Öğe Dermatofibrosarcoma protuberans: An unusual case of breast(2020) Gemici, Kazim; Gundogdu, RamazanDermatofibrosarcoma protuberans (DFSP) is a rare tumor of the breast. It is locally aggressive tumor with high rate of local recurrences. A 64 years-old female patient had recurrent large two DFSP lesions in left breast 14 months after the segmental mastectomy. No distant metastasis was detected. The patient underwent mastectomy. She refused adjuvant treatment. Despite wide resection was performed in the second surgery, the disease recurred again. As a conclusion, unless effective adjuvant treatments and medications are not present, surgical excision is crucial to prevent recurrence.Öğe Effectiveness of ursodeoxycholic acid for the treatment of gall bladder stones and sludge in pregnant women(2021) Okus, Ahmet; Tanrikulu, Yusuf; Gemici, Kazim; Alptekin, Husnu; Sevinc, BarisAim: The objective of this study is to evaluate the effectiveness of ursodeoxycholic acid (UDCA) for the treatment of symptomatic pregnant women with gall bladder stones and/or sludge. Material and Methods: Gynecology and obstetrics clinic patients who were symptomatic with gallbladder stones and/or microlithiasis were included in the study. Group 1 consisted of cases with conservative treatment and group 2 consisted of cases with UDCA treatment. After treatment, the groups were compared for symptomatic and radiological relief and hospitalization. Results: For group 2, the mean treatment time with UDCA was 7.6 weeks. After treatment, a full cure was achieved in 28% of cases and symptomatic relief was achieved in 36% of cases. In group one; there was response to treatment in only 12% of cases. In the UDCA group, none of the patients needed hospitalization; however, in the conservative treatment group, five cases needed hospitalization. Conclusion: UDCA is effective for the treatment of symptomatic gall bladder disease in pregnancy. It both relieves symptoms and decreases the need for hospitalization.Öğe Laparoscopic cholecystectomy in pregnant women can be performed safely with cesarean section(2021) Gemici, Kazim; Uylas, Ufuk; Gundogdu, Ramazan; Alptekin, HusnuAim: The aim of this study is to demonstrate the efficacy of laparoscopic cholecystectomy performed during cesarean section in patients with symptomatic or complicated gallstones during pregnancy to prevent complications associated with gallstones in the early postpartum period.Materials and Methods: Patients were selected among the patients who were followed up in the obstetrics clinic between 2010 and 2015, who had gallbladder stones and showed symptoms such as acute cholecystitis, biliary pancreatitis, choledocholithiasis and biliary colic. Fifteen patients accepted our recommendation of laparoscopic cholecystectomy with cesarean section. Patients were evaluated together with general surgery clinic. Patients with biliary colic due to bile sludge and who underwent cholecystectomy during pregnancy were excluded from the study. Informed consent forms were obtained from the patients. The study was approved by the “Aksaray University Rectorate Human Research Ethics Committee” with the protocol number 2020/08-03.Results: Laparoscopic cholecystectomy was performed in the second trimester of pregnancy (two patients due to acute cholecystitis and one patient due to biliary pancreatitis) in three patients who did not respond to medical treatment. The other 12 patients underwent laparoscopic cholecystectomy simultaneously with cesarean section. Eight patients were discharged on postoperative first day and four patients were discharged on postoperative second day.Conclusion: We believe that laparoscopic cholecystectomy can be performed safely with caesarean section in pregnant women with symptomatic and complicated gallstones.
