Öğe Dermatofibrosarcoma protuberans: An unusual case of breast(2020) Gemici, Kazim; Gundogdu, RamazanDermatofibrosarcoma protuberans (DFSP) is a rare tumor of the breast. It is locally aggressive tumor with high rate of local recurrences. A 64 years-old female patient had recurrent large two DFSP lesions in left breast 14 months after the segmental mastectomy. No distant metastasis was detected. The patient underwent mastectomy. She refused adjuvant treatment. Despite wide resection was performed in the second surgery, the disease recurred again. As a conclusion, unless effective adjuvant treatments and medications are not present, surgical excision is crucial to prevent recurrence.Öğe Incidental cancer in colectomy specimens from patients with familial adenomatous polyposis: single centre experience and literature review(Springer, 2023) Uylas, Ufuk; Gundogdu, Ramazan; Sumer, Fatih; Samdanci, Emine; Kayaalp, CuneytBackgroundSince cancer development is inevitable in patients with familial adenomatous polyposis (FAP), we aimed to determine the incidence of incidental malignancy in prophylactic colectomy specimens.MethodsThe files of patients who underwent prophylactic surgery for FAP between 2010 and 2020 were retrospectively reviewed. The incidence of incidental malignancy in histopathological specimens was examined and a comprehensive literature review was made.ResultsFifty-five patients were included in the study, of whom 30 patients had a diagnosis of primary malignancy. Prophylactic colectomy was performed on 25 patients. The pathology results indicated that the specimens were benign in 12 patients (48%) and revealed carcinoma in situ in 11 patients (44%). Incidental malignancy was detected in 2 patients (8%). In the literature review, there were 243 patients who underwent prophylactic colectomy and incidental cancer was detected in 25 patients (10.3%) with the stages of 1 (7.4%), 2 (2.1%), and 3 (0.8%), respectively.ConclusionsIncidental cancer is not rare in patients who have undergone prophylactic colectomy for FAP. Hopefully. they are usually at early stages and unexpected advanced cancers are seen rarely.Öğe Is lobectomy an effective treatment method in giant hepatocellular carcinomas?(2021) Gundogdu, Ramazan; Uylas, Ufuk; Caliskan, Kenan; Erkan, Serkan; Kus, MuratAim: Hepatocellular carcinomas (HCC) are diagnosed at a high frequency worldwide. The most effective treatment method is surgery. Surgical treatment of giant HCCs (HCCs ≥10 cm in diameter) remains controversial due to its advanced stage. In this study, we discuss the surgical treatment of patients with HCC with a tumor size ≥10 cm, as well as early complications and effect on survival. Materials and Methods: Patients who underwent surgical treatment for HCC in our clinic between 2011 and 2021 were retrospectively reviewed. Patients older than ≥18 years of age who were treated with anatomical lobectomy and whose files were not missing any data were included in the study. Demographic characteristics, surgery and pathology reports, and survival data of the patients were analyzed. Results: A total of seven patients, five of whom were men, were included in the study, and the median age was 66 years (range 28–76). The median tumor diameter was 20 cm (range 10.5–24). Anatomical lobectomy was performed in all patients. The median duration of hospitalization was 4 days (range 3–7). Perioperative morbidity and mortality were not observed. At least one liver recurrence was observed in all patients during follow-up. Pulmonary metastasis was detected in two patients during follow-up. The median survival was 24 months (range 7–60). Conclusion: We think that lobectomy can be applied with low perioperative morbidity and mortality in patients with selected giant HCCs in experienced centers such as our clinic.Öğe Is routine pathological examination of hernia sac necessary?(2021) Gundogdu, Ramazan; Uylas, UfukAim: Minimally invasive hernia repair methods in which the hernia sac is not excised are applied frequently, bringing into question the necessity of routine pathological examination when the hernia sac is excised. Here, we aim to discuss the pathology results and unexpected histopathological findings of 437 sequential patients who underwent abdominal wall and inguinal hernia operation. Materials and Metods: Patients who underwent hernia surgeries and had the hernia sac sent for histopathological examinations at our hospital were retrospectively screened. Emergency and elective operated patients were included; hernia repairs without pathologic examination were excluded from the study. Pathology results were categorized into two groups as expected findings and unexpected findings. Results: A total of 437 patients were included in the study. Of these patients, 259 (59.3%) were male and the mean age was 51±16 years. Expected pathological findings were 98.9% and unexpected pathological findings were 1.1%. Unexpected pathological findings results were ranked according to frequency as follows: accessory adrenal cortex in two cases (0.4%), endometriosis in one (0.2%), skin pseudo epithelial hyperplasia in one (0.2%), and colon wall in one (0.2%). Conclusion: Histologically, unusual findings in hernia sacs were seen only in 1.1% and they were all benign. . For this reason, we think that microscopic examination of adult hernia sac specimens, other than those that cannot be clearly demonstrated to be benign macroscopically, unnecessarily increase the workload and cost.Öğe Laparoscopic cholecystectomy in pregnant women can be performed safely with cesarean section(2021) Gemici, Kazim; Uylas, Ufuk; Gundogdu, Ramazan; Alptekin, HusnuAim: The aim of this study is to demonstrate the efficacy of laparoscopic cholecystectomy performed during cesarean section in patients with symptomatic or complicated gallstones during pregnancy to prevent complications associated with gallstones in the early postpartum period.Materials and Methods: Patients were selected among the patients who were followed up in the obstetrics clinic between 2010 and 2015, who had gallbladder stones and showed symptoms such as acute cholecystitis, biliary pancreatitis, choledocholithiasis and biliary colic. Fifteen patients accepted our recommendation of laparoscopic cholecystectomy with cesarean section. Patients were evaluated together with general surgery clinic. Patients with biliary colic due to bile sludge and who underwent cholecystectomy during pregnancy were excluded from the study. Informed consent forms were obtained from the patients. The study was approved by the “Aksaray University Rectorate Human Research Ethics Committee” with the protocol number 2020/08-03.Results: Laparoscopic cholecystectomy was performed in the second trimester of pregnancy (two patients due to acute cholecystitis and one patient due to biliary pancreatitis) in three patients who did not respond to medical treatment. The other 12 patients underwent laparoscopic cholecystectomy simultaneously with cesarean section. Eight patients were discharged on postoperative first day and four patients were discharged on postoperative second day.Conclusion: We believe that laparoscopic cholecystectomy can be performed safely with caesarean section in pregnant women with symptomatic and complicated gallstones.Öğe A new approach on the regulation of NF-?B and Bax protein signaling pathway activation by royal jelly in fluoride-induced pancreas damage in rats(Churchill Livingstone, 2022) Aslan, Abdullah; Can, Muhammed Ismail; Beyaz, Seda; Gok, Ozlem; Parlak, Gozde; Gundogdu, Ramazan; Ozercan, Ibrahim HanifiForty-two healthy adult male rats (Wistar albino, n = 42, 8 weeks old, starting weights 200-250 g) employed in this study were subdivided into six groups randomly with seven rats per group as follows: (i) Control group: received standard diet; (ii) RJ group: received standard diet supplemented with royal jelly; (iii) F50 group: received standard diet supplemented with fluoride (50 mg/kg BW); (iv) F100 group: received standard diet supplemented with fluoride (100 mg/kg BW); (v) F50 +RJ group: received standard diet supplemented with fluoride (50 mg/kg BW) and royal jelly; (iv) F100 +RJ group: received standard diet supplemented with fluoride (100 mg/kg BW) and royal jelly. The study continued for a total of eight weeks. Western blot analysis was conducted to determine the post-translational expression levels of NF-kappa B, Bax, Bcl-2, TNF-alpha, Caspase-3 and Caspase-6 proteins in pancreas tissue. The pancreatic tissue was subjected to histopathological evaluation. Furthermore, MDA, GSH and CAT activities were examined by spectrophotometric analyzes. Our findings demonstrate that, compared to the control and RJ groups, Bcl-2 protein expression was augmented and, conversely, Caspase-6, Caspase-3 and Bax protein levels were decreased upon fluoride treatment. A statistically significant increase in TNF-alpha and NF-kappa B protein expressions was observed in the groups with fluoride-induced damage compared to the control and RJ groups. The MDA levels were increased in all fluoride-treated rats compared to those in the control and RJ groups, whereas the CAT and GSH activities were reduced in all rats with fluoride-induced damage. Although there was not a great difference between the groups regarding histopath-ological findings, there was a tendency to decrease in the rate of damage upon royal jelly treatment.Öğe Operative and conservative treatment of right colon diverticulitis(2021) Uylas, Ufuk; Gundogdu, Ramazan; Cetin, Durmus Ali; Oztop, Burak; Demirli Atici, Semra; Calik, BulentAim: This study assessed the results of conservative and surgical treatment of right colon diverticulitis. Right colon diverticula contain all layers of the colon and are called true diverticula. Perforation of these diverticula is observed less frequently due to their full thickness.Materials and Methods: 150 patients treated for colonic diverticulitis between 2015-2020 were retrospectively screened. Following exclusions, a total of 26 patients data were assessed. The patients were divided into two groups as conservative or surgical treatment. The outcomes for these two groups were compared and analyzed.Results: Most patients in the sample were female (n=18, 69%). The average age was 47 (22-83). Data from the conservative treatment group (n=17, 65%) and the surgical treatment group (n=9, 35%) were examined. Comorbidity, recurrence, and white blood cell count were all greater following conservative treatment (p> 0.05). In the surgical group, two (22% of the group) patients developed wound infections. Patients with fever at admission were in the surgical group (p 0.01). All patients in the conservative group had a Hinchey classification of 1a, while those in the surgical group were Hinchey 1a, 1b, and 3 (p 0.05). The median hospital stay was higher for the surgical group (p 0.05). No mortality was observed.Conclusion: Right colon diverticulitis has low complications and it can be treated conservatively. The differential diagnosis of rightsided colonic diverticulitis should be kept in mind when relevant symptoms present to prevent unnecessary surgeries. Surgical treatment is inevitable in instances of recurrent diverticulitis, generalized peritonitis, and suspected malignancy.Öğe Protective effect of royal jelly on fluoride-induced nephrotoxicity in rats via the some protein biomarkers signalling pathways: a new approach for kidney damage(Taylor & Francis Ltd, 2022) Aslan, Abdullah; Beyaz, Seda; Gok, Ozlem; Can, Muhammed Ismail; Parlak, Gozde; Gundogdu, Ramazan; Ozercan, Ibrahim HanifiIntroduction Protective effect of royal jelly (RJ) on fluoride-induced nephrotoxicity was investigated in this study. Methods 42 healthy male Wistar rats (n = 42, 8 weeks of age) were divided equally into 6 groups with 7 rats in each; (1) Group-1: Controls fed with standard diet; (2) Group-2: RJ [100 mg/kg] bw (body weight), by oral gavage; (3) Group-3: Fluoride [50 mg/kg] bw, in drinking water; (4) Group-4: Fluoride [100 mg/kg] bw, in drinking water; (5) Group-5: RJ [100 mg/kg] bw, by oral gavage + Fluoride [50 mg/kg] bw, in drinking water; (6) Group-6: RJ [100 mg/kg] bw, by oral gavage + Fluoride [100 mg/kg] bw, in drinking water. After 8 weeks, all rats were decapitated and their kidney tissues were removed for further analysis. The protein expression levels of caspase-3, caspase-6, caspase-9, Bcl-2, Bax, VEGF, GSK-3, BDNF, COX-2 and TNF-alpha proteins in kidney tissue were analysed by western blotting technique Results RJ increased Bcl-2, COX-2, GSK-3, TNF-alpha and VEGF protein levels and a decreased caspase-3, caspase -6, caspase-9, Bax and BDNF protein levels in fluoride-treated rats. Conclusion RJ application may have a promising therapeutical potential in the treatment of many diseases in the future by reducing kidney damage.Öğe Royal jelly abrogates flouride-induced oxidative damage in rat heart tissue by activating of the nrf-2/NF-?B and bcl-2/bax pathway(Taylor & Francis Ltd, 2021) Aslan, Abdullah; Beyaz, Seda; Gok, Ozlem; Can, Muhammed Ismail; Parlak, Gozde; Ozercan, Ibrahim Hanifi; Gundogdu, RamazanRoyal jelly is known to strengthen memory, provide antioxidative, antidiabetic, antitumor, anticancer, antibacterial, antiinflammatory, antihypertensive. In this study, 42 rats (n = 42) were used, and these rats were divided into 6 groups of 7 rats each. Groups: (i) Control Group: Group fed with standard diet; (ii) Royal Jelly (RJ) Group: RJ (100 mg/kg bw, gavage); (iii) F50 Group: Fluoride (50 mg/kg bw, drinking water); (iv) F100 Group: F (100 mg/kg bw, drinking water); (v) F50 + RJ Group: F (50 mg/kg bw, drinking water) + RJ (100 mg/kg bw, gavage); (vi) F100 + RJ Group: F (100 mg/kg bw, drinking water) + RJ (100 mg/kg bw, gavage). The rats were decapitated after 8 weeks, and their heart tissues were taken and examined. Lipid peroxidation by MDA (malondialdehyde) analyzes, GSH (glutathione) level and catalase activity were determined by spectrophotometer. Protein expression levels of caspase-3, caspase-6, caspase-9, Bcl-2, Bax, BDNF, Gsk-3, Nrf-2 and NF-kappa B proteins in heart tissue were determined by western blotting technique and hearth tissue evaluated by histopathologically. As a result, MDA levels, Bcl-2, Gsk-3 and NF-kappa B protein expression levels were reduced, whereas GSH levels, caspase-3, caspase-9, caspase-6, Bax, BDNF and Nrf-2 protein levels were increased in the F50 + RJ and F100 + RJ groups compared to the F50 and F100 groups. Material and methods: Our study continued for 8 weeks. 42 Wistar albino (8 weeks old) male rats were used in the study. The study included 6 groups; Group 1: Control group (fed with standard diet), Group 2: RJ (100 mg/kg, bw), Group 3: F-50 (50 mg/kg, bw), group 4: F-100 (100 mg/kg, bw) group 5: F-50 (50 mg/kg, bw) + RJ (100 mg/kg, bw) Group 6: F-100 (100 mg/kg, bw) + RJ (100 mg/kg, bw). Malondialdehyde (MDA), catalase (CAT) and glutathione (GSH) activities in liver tissue were determined by spectrophotometer. Liver tissue samples were examined histopathologically and various protein levels were determined by Western blotting technique. Results: RJ caused a significant decrease in MDA level, Bcl-2, GSK3 and NF-kappa B protein expression levels, whereas induced a significant increase in GSH level, CAT activities and Bax, BDNF, caspase-6, caspase-3, Nrf-2 protein expression levels. Conclusion: Our findings suggest RJ to be used as a hepatoprotective agent in the clinic to modulate the toxic effects of fluoride and other chemicals in the future.Öğe Royal jelly regulates the caspase, Bax and COX-2, TNF-? protein pathways in the fluoride exposed lung damage in rats(Churchill Livingstone, 2022) Aslan, Abdullah; Gok, Ozlem; Beyaz, Seda; Can, Muhammed Ismail; Parlak, Gozde; Gundogdu, Ramazan; Ozercan, Ibrahim HanifiThe study was carried out on 42 male rats divided into six groups with 7 rats in each group: two control groups, two injury groups and two treatment groups. One of the control groups received a basal diet while the other one was fed a basal diet supplemented with royal jelly (RJ) (100 mg/kg). The two injury groups were given 50 mg/kg and 100 mg/kg fluoride, respectively. The two treatment groups exposed to 50 mg/kg and 100 mg/kg fluoride were both fed basal diets with RJ (100 mg/kg). Lungs were taken for histopathological examination. Spectrophotometric analysis was utilized to determine Malondialdehyde (MDA), catalase (CAT) and glutathione (GSH) activities, and Western blotting technique was used to evaluate the levels of specific proteins. On one hand, our experiments revealed that RJ caused decreased MDA levels, and downregulation of COX-2, Bcl-2, GSK3 and TNF?? protein expressions. On the other hand, rolay jelly caused augmented GSH and CAT activities, as well as upregulated Bax, BDNF, caspase-3, caspase-6, caspase-9 protein expressions in rats injuried by the fluoride exposure. The results suggest that the application of RJ was very likely to have a healing effect on the degenerative changes seen in the examined tissue.Öğe Von Wahl Sign in Sigmoid Volvulus Patient with Cerebrovascular Disease(Gazi Univ, Fac Med, 2021) Uylas, Ufuk; Gundogdu, Ramazan; Turgut, EmreIn addition to radiological imaging, physical examination is also of great importance in the diagnosis of sigmoid volvulus. The presence of a palpable sigmoid colon on abdominal examination is known as the von Wahl sign. A 77-year-old female patient with a history of paraplegia caused by CVD was admitted to the emergency service with abdominal pain and obstipation. Abdominal examination of the patient revealed asymmetric distension in the abdomen in inspection and von Wahl sign in palpation. The coffee bean appearance on the plain radiograph supported the sigmoid volvulus. Successful colonoscopic detorsion was applied to the patient. Six months later, sigmoid volvulus recurrence occurred in the patient who refused elective surgery. Sigmoid colon resection and Hartmann's procedure were performed. The patient, who developed respiratory failure, died on the second postoperative day. In addition to imaging in patients with neurodegenerative disease, findings such as von Wahl sign in the physical examination are supportive in the diagnosis of sigmoid volvulus.
