Öğe Clinical results of endoprosthetic reconstructions in metastatic and primary bone tumors(2022) Sevimli, Reşit; Karaman, SerdarIn this study, we aimed to compare the functional outcomes and survival of 38 patients with primary bone and metastatic bone tumors who were reconstructed with endoprosthesis. Thirty-eight patients who underwent endoprosthesis for malign bone tumors 2012- 2015 were included in the study. We retrospectively reviewed 38 patients, eight of whom underwent limb salvage with tumor resection prosthesis of tibia, 21 femur, and 9 humerus. When comparing functional outcomes, the revised Musculoskeletal Tumor Society (MSTS) scoring system was used for all surviving patients. The patients were evaluated in two groups as primary bone tumor and metastatic bone tumor. The mean follow-up period of 32 healthy and alive patients was 38 (12 to 98) months. While 12 patients were primary malign bone tumors, the other 26 patients had bone metastasis. All six patients who died during follow-up had a diagnosis of metastatic bone tumor. Periprosthetic infection was observed in two patients, recurrence tumor in one and prosthetic dislocation in one. It was found that the MSTS score in patients with primary bone tumors (22, 6) was statistically significantly higher than in patients with metastases (17, 6) (p<0.05). The score of the patients who had upper extremity (%22) reconstruction was found to be significantly higher than the lower extremity (%78) (p<0.05). It was determined that better functional results were obtained for patients with primary bone tumors than patients with bone metastasis. The low complication rates obtained because of this study, which had an average follow-up of 38 months, showed us that reconstruction with endoprosthesis is promising for tumors of the femur, tibia and humerus.Öğe Spontaneous extensor pollicis longus (EPL) tendon rupture after ironing(Bayrakol Medical Publisher, 2021) Karaman, Serdar; Koroglu, Muhammed; Ozdemir, Zeynep Maras; Ertem, KadirSpontaneous tendon ruptures can occur rarely. Distal radius fractures, steroid applications and systemic diseases are mostly attributed to its etiology. In the treatment of spontaneous tendon ruptures, primary repair is not possible and tendon transfer is often required. The EPL tendon is the main extensor of the thumb. This muscle originates from the interosseous membrane, especially the dorsal part of the ulna. The EPL passes from the third dorsal extensor compartment ulnar to the Lister's tubercle before returning to the thumb. Since spontaneous tendon ruptures often occur due to degeneration, the defect is large in the area of the rupture. Therefore, spontaneous tendon ruptures are treated with tendon transfer rather than end-to-end repair. In this case report, we aimed to present a case of extensor indicis proprius (EIP) tendon transfer to our 64-year-old female housewife who was admitted to our clinic with a spontaneous rupture of the EPL tendon a resulting from excessive ironing in a short term. As a result of the examination and radiological evaluation of the patient who applied to our clinic with the complaint of inability to extend the thumb extension, a spontaneous EPL tendon rupture was diagnosed and surgical treatment was decided. While making a diagnosis of spontaneous tendon rupture, it was learned that the patient had no trauma and rheumatologic disease in her anamnesis. Ultrasound and magnetic resonance imaging (MRI) were used to confirm the diagnosis radiologically. The spontaneous EPL tendon rupture treatment was successfully provided by the transfer of the extensor indicis proprius (EIP) tendon. At the six month of follow-up after the surgical treatment, it was observed that the finger regained its former function. In elderly patients, repetitive, forceful finger movements for a long time may cause spontaneous tendon rupture. Long-term coercive thumb movement is a risk factor for EPL spontaneous tendon rupture.Öğe Which is more dangerous, earthquake, or the panic? Evaluation of the 24 January 2020 Elazig/T?rkiye earthquake related musculoskeletal injuries(Turkish Assoc Trauma Emergency Surgery, 2022) Ergen, Emre; Kaya, Oguz; Yilmaz, Ozgur; Ozdes, FlUseyin Utku; Batur, Omer Cihan; Karaman, Serdar; Guzel, IsmailBACKGROUND: The aim of this study is to evaluate the musculoskeletal injuries related with 24 January 2020 Elazig/Turkiye earthquake and their treatment protocols. METHODS: Data of patients applied to Inonu University Medical Faculty Hospital, Elazig Training and Research Hospital and Malatya Training and Research Hospital emergency departments within 48 h after the earthquake, were evaluated retrospectively. Age, gender, soft tissue injuries and sites, fracture sites and types, fracture etiology, and treatment methods were evaluated.RESULTS: 247 patients were evaluated. 118 were women and 139 were men. There were 24 (9.7%) pediatric patients. Mean age was 37.3 (1-92) years. Waist majority of injuries were simple soft-tissue injuries. There were 103 fractures in 86 patients. Thirty-eight patients' fractures were treated surgically.CONCLUSION: Every major disaster warrants retrospective studies so we can learn how to improve all levels of Emergency Medical Services. Great proportion of Elazig earthquake victims had only simple soft tissue injuries such as sprain, laceration, or contusion. Many patients were injured due to reasons indirectly related to the destruction brought by the earthquake. Panic caused by the earthquake caused more injury than the destruction it brought.
