    Comparison of high-intensity laser therapy and combination of transcutaneous nerve stimulation and ultrasound treatment in patients with chronic lumbar radiculopathy: A randomized single-blind study

    Objective: To compare the effects of high-intensity laser therapy (HILT) and a combination of transcutaneous nerve stimulation (TENS) with ultrasound (US) therapy on pain and functionality in patients with chronic lumbar radiculopathy. Methods: This prospective randomized comparative study was conducted in Department of physical medicine and rehabilitation, Turgut Ozal Medicine Center, Malatya, Turkey from April 2016 to September 2016. A total of 54 patients with chronic lumbar radiculopathy were enrolled in this study. The patients were randomly divided into two groups: Group 1 (n:27) received 10 sessions of a combination of hot pack, TENS, US and exercise, and Group 2 (n:27) received hot pack, HILT and exercise. The outcomes measured were low back with unilateral leg pain level measured by visual analog scale (VAS) and functionality measured with the Oswestry Disability Index (ODI) at the end of the therapy and four weeks later. p-value less than 0.05 considered statistically significant. Results: In two groups, VAS (low back with unilateral leg pain) and ODI scores showed significant changes. At the end of the 2 weeks intervention, participants in Group-1 showed a significantly greater decrease in pain than participants in Group-2. Statistically significant differences in pain variation and functionality (VAS and ODI) were observed four weeks after treatment sessions for participants in the TENS+US therapy group compared with participants in the HILT group. Conclusion: HILT and TENS+US combined with exercise were effective treatment modalities in decreasing the VAS and ODI scores. TENS+US combined with exercises were more effective than HILT combined with exercise.

    Lomber radikülopatili hastalarda yüksek yoğunluklu lazer tedavisi (hilterapi) etkinliğinin tens ve us kombinasyonu ile karşılaştırılması

    Amaç: Lomber radikülopatili hastalarda yüksek yoğunluklu lazer tedavisi (hilterapi) ile TENS ve US kombine tedavisinin ağrı, fonksiyonellik üzerine olan etkilerinin karşılaştırılması amaçlanmıştır. Gereç ve Yöntem: İnönü Üniversitesi Tıp Fakültesi Fiziksel Tıp ve Rehabilitasyon AD polikliniğine başvuran, en az 4 haftadır mekanik bel ağrısı ve/veya herhangi bir bacağa yayılan ağrısı olan ve radyolojik olarak lomber MRG ile disk herniasyonu doğrulanmış 54 hasta çalışmaya dâhil edildi. Çalışma prospektif, randomize, tek kör olarak tasarlandı. Hastalar randomize olarak 2 gruba ayrıldı. İlk gruba 2 hafta (haftada 5 gün toplam 10 seans) süre ile hot pack, TENS, ultrason tedavisi, ikinci gruba 2 hafta (haftada 5 gün toplam 10 seans) süre ile 20 dakika süre ile hot pack ve 10 dk süre ile yüksek yoğunluklu lazer tedavisi uygulandı. Her iki gruba fizyoterapist eşliğinde haftada 5 gün olacak şekilde toplam 10 seans, 3 set halinde 10 tekrarlı egzersiz programı verildi. Egzersiz programı abdominal kasları güçlendirme egzersizleri ve lomber stabilizasyon egzersizlerini içermekteydi. Hastalar tedavi sonrası ve 1. ayda kontrole çağrıldı. Ağrı için Visuel Analog Skala (VAS) ve fonksiyonel değerlendirme için Modifiye Oswestry Skalası kullanıldı. Bulgular: Çalışmaya toplam 54 hasta (36 kadın ve 28 erkek, 19-64 yaş arası, ortalama yaş 51.7±11.5) dâhil edildi. Her iki grupta tedavi sonrası ölçümlerde tüm parametrelerde istatiksel olarak anlamlı değişiklik tespit edildi. Gruplar arası karşılaştırmada ise TENS ve US kombine tedavisinin yüksek yoğunluklu lazer tedavisine göre tedavi öncesi (TÖ) baz alındığında hem tedavi sonrası (TS) hem de 1. ay değerlendirmesinde Visuel Analog Skalası ve Oswestry değerlerinde anlamlı oranda daha fazla azalma sağladığı görüldü. Sonuç: Lomber radikülopatide TENS+US kombine tedavisi yüksek yoğunluklu lazer tedavisine kıyasla ağrı ve fonksiyonel skalalar üzerine istatistiksel açıdan anlamlı oranda etkili bir yöntemdir. Çalışmamızda mevcut tedavilerin kısa süreli etkinliği değerlendirildiği için yüksek yoğunluklu lazerin uzun dönem etkilerinin değerlendirildiği yeni çalışmalara ihtiyaç duyulmaktadır.
