Öğe Allergic granulomatosis and angiitis in the absence of asthma and blood eosinophilia: a rare presentation of limited Churg-Strauss syndrome(Springer Heidelberg, 2004) Sevinc, A; Hasanoglu, HC; Gokirmak, M; Yildirim, Z; Baysal, T; Mizrak, BAllergic granulomatosis and angiitis, also known as Churg-Strauss syndrome (CSS), is an uncommon vasculitis of unknown etiology. We report a 21-year-old male patient with fatigue, dry cough, and progressive dyspnea. He had no history of asthma or eosinophilia. Thorax computed tomography showed bullous/cystic areas with thin walls in varying sizes (5-15 mm). Histopathological examination of the open lung biopsy revealed granulomatous infiltration with histiocytes and eosinophilic leukocytes. We report spectroscopy analysis of a 26-year-old woman affected by right ovarian lesion diagnosed as mucinous cystadenoma. MRS was performed by the point-resolved spectroscopy technique with a long echo time (TE) (136 msec). MRS measurements were performed on the two distinct component of the right ovarian tumor. The classification of metabolite peaks area in this study was performed according to the technique described by Okada et al. The features of proton MRS studies are discussed. As well as strongly elevated lactate and N-acetyl-L-aspartate signals, the tumor spectrum showed lipid resonances. We present a 26-year-old woman with cervical cord compression due to massive calcification in the periodontoid area and discuss the X-ray and CT findings of the disease.Öğe Effect of caffeic acid phenethyl ester on cartilage in experimental osteoarthritis(Springer-Verlag, 2002) Elmali, N; Avan, I; Türköz, Y; Mizrak, B; Germen, B; Bora, AActivation of nuclear factor kappa B (NF-kappaB) in synovial cells is seen in RA and OA patients. Caffeic acid phenethyl ester (CAPE) is a specific and potent inhibitor of NF-kappaB. We aimed to determine the in vivo effects of intra-articular injections of CAPE on cartilage in an experimental rabbit osteoarthritis (OA) model. Two groups of six New Zealand white rabbits underwent unilateral anterior cruciate ligament transection (ACLT). Four weeks after ACLT, the test group was injected with 150 mug/kg CAPE in 0.5% ethanol once daily for 2 weeks and the control group was injected the same amount of 0.5% ethanol intra-articularly. All rabbits were killed 2 weeks after the last injection, and cartilage tissue was evaluated morphologically. A histological score totaling 7 points was determined for each knee. The CAPE group showed significantly decreased cartilage destruction and reduced loss of matrix proteoglycans. The histological score for cartilage tissue was significantly better in the CAPE group than in the control group (3.0 +/- 0.25 vs 5.3 +/- 0.55, P=0.005). This study suggests that intraarticular injection of CAPE may protect cartilage against the development of experimentally induced OA.Öğe Effect of melatonin and atenolol on carbon monoxide cardiotoxicity(Blackwell Publishing, 2006) Mizrak, B; Celbis, O; Parlakpinar, H; Ölmez, EThe aim of this study was to define the characteristics of heart rates and myocardial changes in rats exposed to carbon monoxide (CO), and the effects of reoxygenation, atenolol (a beta-blocker) and melatonin after sublethal CO intoxication. Widespread use of beta-blockers in cardiology practice and growing literature on the positive effect of melatonin in ischaemia reperfusion lead us to question their effects in case of CO intoxication. Rats were exposed to CO. After sublethal intoxication the rats were reoxygenated with ambient air. Subsequently blood values, electrocardiographic recordings and pathological changes were examined for each groups. Five rats died after CO intoxication in the control group: no myocardial changes were seen in light microscopy However, myocardium of seven reoxygenated rats presented contraction bands. Seven reoxygenated rats pretreated with atenotol had a higher number of contraction bands of myocardial cells. Seven reoxygenated rats pretreated with melatonin had more contraction bands than reoxygenated rats. and heart rate recordings of these animals revealed a profund and sustained bradycardia. Thus, melatonin and atenolol appear to have some adverse effects in CO intoxication on the myocardial cells.Öğe Effect of resveratrol in experimental osteoarthritis in rabbits(Springer Basel Ag, 2005) Elmali, N; Esenkaya, I; Harma, A; Ertem, K; Turkoz, Y; Mizrak, BObjective: Resveratrol (trans-3,4',5-trihydroxystilbene) is a phytoalexin found in high concentration in the skins of grapes and red wines which has been shown to have antiinflammatory, anticancerogen and antioxidant properties. Resveratrol is a potent and specific inhibitor of nuclear factor kappa B ( NF-kappa B). Resveratrol also inhibits COX-2 gene expression and enzyme activity. We aimed to determine the in vivo effects of intra-articular injections of resveratrol on cartilage and synovium in an experimental osteoarthritis (OA) model in rabbits. Methods: As OA model, rabbits underwent unilateral anterior cruciate ligament transection (ACLT). Five weeks after test group was injected with 10 mu Mol/kg resveratrol in dimethylsulphoxide ( DMSO) in the knees once daily for two weeks and as the control group at the same time DMSO was injected into the knees. All rabbits were killed one week after the last injection. Cartilage tissue and synovium were evaluated with a histological scoring system. Results: Histological evaluation of cartilage tissue by H&E staining revealed a significantly reduced average cartilage tissue destruction score of 1.7 in the resveratrol group versus 2.8 in the control group ( p = 0.016). Loss of matrix proteoglycan content in cartilage was also much lower, as determined by safranin O staining. Scores of synovial inflammation didn't show difference between groups ( 1,3 vs 2,2; p = 0.057). Conclusion: A characteristic parameter in arthritis is the progressive loss of articular cartilage. This study suggests that intraarticular injections of resveratrol starting at the onset of disease may protect cartilage against the development of experimentally induced OA.Öğe The effect of sustained and local administration of epidermal growth factor on improving bilateral testicular tissue after torsion(Springer, 2004) Uguralp, S; Karabulut, AB; Mizrak, B; Kaymaz, F; Kiziltay, A; Hasirci, NEpidermal growth factor (EGF) modulates Leydig cell proliferation, steroidogenesis, spermiogenesis, and Sertoli cell activity. It plays an important role in repairing ischemia-reperfusion injury in different tissues. The aim of this study was to evaluate the effects of sustained and local administration of EGF on improving bilateral testicular tissue after torsion. A total of 57 Wistar albino rats were used. For the EGF transport system, 1x2 cm gelatin films containing 2 mug EGF were used. Torsion was created by rotating the right testis 720degrees in a clockwise direction for 4 h in all groups except the control group. Then, in the torsion group, bilateral orchiectomy was performed. After returning the torsioned ipsilateral testes to their normal state, the bilateral testes were wrapped by 1x2 cm unloaded gelatin films in the gelatin (G7 and G21) groups and, by 2 mug EGF loaded gelatin films in the EGF 7 and EGF 21 groups. The testes were removed on the seventh and 21st days, respectively, for biochemical and histological examination. Histologically, Johnsen's spermatogenesis criteria and mean seminiferous tubule diameter (MSTD) measurements were used. The EGF7 group did not show significant loss of Sertoli cells, while in the G7 group the number of these cells decreased. The ipsilateral ischemic testis of the EGF21 group showed Leydig cell hyperplasia, and the contralateral non-ischemic testes in this group were similar to the control group. In the G21 group, the bilateral testes showed Sertoli cell only syndrome in some sections, and most of the cells were undergoing apoptosis. The mean spermatogenesis scores and MSTD in the EGF7 and EGF21 groups were higher than in the G7 and G21 groups (P<0.05). Malondialdehyde levels were significantly lower in the EGF groups than in the G groups (P<0.05). Glutathione peroxidase (GSH-Px) levels in the G21 group were significantly higher than in the EGF21 group. Our study shows that local and sustained EGF release after testicular torsion improves bilateral testicular injury. EGF administration may be a new treatment choice for bilaterally injured testis after detorsion without removing the twisted testis.Öğe Effects of pentoxifylline and vitamin E on the bilateral ovary after experimental ovarian ischemia(Georg Thieme Verlag Kg, 2005) Uguralp, S; Karabulut, AB; Mizrak, BPentoxifylline (PTX) and vitamin E inhibit the release of superoxide and hydroxyl radicals, and PTX improves capillary flow and tissue oxygenation. This experimental study was designed to determine the effects of PTX and vitamin E in the ovary after unilateral ovarian ischemia reperfusion (I-R) in albino Wistar rats. A vascular clamp was placed on the left ovary for 4 hours in all groups except for the control group. Following this, in the ischemia (1) group bilateral ovariectomy was performed. Saline, PTX, vitamin E, and PTX plus vitamin E were infused 30 min before reperfusion in the reperfusion (R), pentoxifylline (P), vitamin E (E), and pentoxifylline plus vitamin E (PE) groups, respectively. After 4 hours of reperfusion, the ovaries were removed for biochemical and histologic examination. MDA levels of bilateral ovaries in the PE group were significantly lower than in the E group (p < 0.0033). NO levels of bilateral ovaries in the PE group were significantly lower than in the P and E groups (p < 0.0033). Massive hemorrhage was determined in the ipsilateral ovaries of the R group. Hemorrhage was minimal or moderate in the ipsilateral ovaries of other groups. The contralateral ovaries showed congestion in different degrees. The contralateral ovaries of the group PE and the bilateral ovaries of the control group showed no pathological changes. PTX and vitamin E given together seems to be more effective in reducing I-R injury in ovarian tissue compared to administration of PTX, or vitamin E alone. However, further studies are required to evaluate the effective dose and duration of PTX and vitamin E on bilateral ovaries.Öğe Effects of resveratrol, raloxifene, tibolone and conjugated equine estrogen on vaginal squamous cell maturation of ovariectomized rats(Karger, 2005) Hascalik, S; Celik, O; Tamser, M; Mizrak, BObjective: The effects of estrogen replacement therapy, selective estrogen receptor modulators, or tibolone on vaginal squamous cell maturation in postmenopausal women are not well established. Resveratrol ( 3,5,4'- trans - trihydroxystilbene) has been shown to bind the estrogen receptor in rat uteri. The aim of this study was to cytologically evaluate vaginal smears from ovariectomized rats treated with resveratrol, raloxifene, tibolone and conjugated equine estrogen, and to compare each drug with regard to vaginal epithelial maturation. Material and Methods: Forty-two bilaterally ovariectomized Wistar albino rats were equally randomized into 6 groups: ( 1) control sham-operated rats; ( 2) ovariectomized rats administered 0.1% ethanol; ( 3) ovariectomized rats administered resveratrol at a dose of 5 mg/kg/day p. o.; ( 4) ovariectomized rats administered conjugated equine estrogen (CEE) at a dose of 0.1 mg/kg/day p. o.; ( 5) ovariectomized rats administered tibolone at a dose of 0.25 mg/kg/ day p.o., and ( 6) ovariectomized rats administered raloxifen at a dose of 1 mg/kg/day p. o. Administration of drugs started 5 days after bilateral ovariectomy and continued for 35 days. After 35 days of treatment a vaginal smear was obtained from each rat. Smears were stained with the usual Papanicolaou method, and observed with a light microscope by an experienced cytopathologist. Cytological grading was made according to the extent of parabasal, intermediate, superficial and anuclear squamous cells. Results: Ovariectomized rats had lower scores for superficial and anuclear cells when compared to sham-operated rats ( p < 0.05). The CEE group had higher scores for superficial and anuclear cells than those of the ovariectomized, raloxifene and tibolone groups ( p < 0.05). The resveratrol-treated rats had higher scores for superficial cells but lower scores for parabasal cells than ovariectomized rats ( p < 0.05). The raloxifene and tibolone groups had the same scores for intermediate, superficial and anuclear cells but lower scores for parabasal cells compared to ovariectomized rats. Conclusion: The results of our study suggest that resveratrol offsets the reduction in vaginal stratification generally observed after oophorectomy. Copyright (C) 2005 S. This tumor became calcified within 2 weeks. It was completely removed surgically, after which the patient's symptoms were relieved and neurological deficits regressed.Öğe Endothelial nitric oxide synthase expression in leiomyoma and parental myometrium(Karger, 2000) Gokdeniz, R; Mizrak, B; Ozen, S; Bazoglu, NThe aim of this study was to determine the expression of endothelial nitric oxide synthase (eNOS) in leiomyomatous tissue as a possible mediator of the growth process. Nine patients had myoma enucleation during the follicular phase. The myomata and adjacent myometrial tissues were fixed in formol until study. Immunohistochemical localization of leiomyomatous and myometrial tissue eNOS expression was performed using specific monoclonal antibodies to eNOS. Statistical comparisons were made using the Mann-Whitney Wilcoxon rank-sum test for the expression of eNOS. The test was considered significant when p values were <0.05. Immunostaining for eNOS was seen in the cytoplasm of myometrial endothelial and smooth muscle cells in both tissue sections. eNOS expression was significantly higher in the smooth muscle cells of leiomyomata, compared to parental myometrium (p < 0.0005). The expression of eNOS in vascular endothelial cells was not different in the leiomyoma and myometrium (p > 0.05). To our knowledge, this is the first study to document marked expression of eNOS in leiomyomatous tissue, compared to parental myometrium. We also conclude that the mechanism(s) of the growth-promoting effect of estrogen on leiomyomata is mediated by more synthesis of NO. Copyright (C) 2000 S. Although changes similar to linear scleroderma have been reported, the outcome is usually good. In this report, a 10-year-old boy who developed eosinophilic fasciitis without a good response to steroids is presented. He progressed to linear scleroderma within months. Our case reinforces the hypothesis that eosinophilic fasciitis may be an early manifestation or a variant of localized scleroderma similar to the other cases in the literature.Öğe Follicular thyroid cancer presenting initially with soft tissue metastasis(Oxford Univ Press, 2000) Sevinc, A; Buyukberber, S; Sari, R; Baysal, T; Mizrak, BFollicular thyroid cancer rarely manifests itself as a distant metastatic lesion. We report a case of an otherwise asymptomatic 58-year-old woman with follicular thyroid cancer who initially presented with a soft tissue mass on the right scapular region, An incisional biopsy specimen of soft tissue metastasis showed thyroid follicular neoplasm. Upon this diagnosis, the thyroid gland was re-evaluated by ultrasound, which demonstrated a solitary, hypoechoic nodule in the right lobe, Ultrasonography guided fine-needle aspiration biopsy of the thyroid nodule confirmed follicular neoplasm and the diagnosis of metastatic follicular thyroid cancer was established. The patient refused any type of treatment and left hospital against medical advice. 2.5 years later the patient was admitted to the hospital with giant, sarcoma-like multiple soft tissue masses. On this admission, the serum thyroglobulin level was extremely elevated (3500 ng/ml) and she only accepted to receive chemotherapy, Epirubicin and cyclophosphamide were administered, She received three courses of chemotherapy and is alive with a stable disease after 3 months of follow-up, This case of follicular thyroid cancer is reported because of its uncommon initial presentation with soft tissue metastasis which spread to multiple areas as giant soft tissue masses during follow-up.Öğe Giant solitary fibrous tumor of the liver with metastasis to the skeletal system successfully treated with trisegmentectomy(Springer, 2000) Yilmaz, S; Kirimlioglu, V; Ertas, E; Hilmioglu, F; Yildirim, B; Katz, D; Mizrak, B[Abstract Not Available]Öğe GnRH agonist decreases endothelial nitric oxide synthase (eNOS) expression in leiomyoma(Elsevier Sci Ireland Ltd, 2000) Gokdeniz, R; Ozen, S; Mizrak, B; Bazoglu, NObjective: To define the effect of GnRH agonist (GnRHa) treatment on endothelial nitric oxide synthase (eNOS) expression in leiomyoma. As eNOS expression is more pronounced in leiomyoma compared to parental myometrium, we hypothesized that the mechanism(s) of tumor shrinkage by GnRHa may be due to decreased nitric oxide (NO) production in leiomyoma. Methods: Eleven patients with leiomyoma ware operated for myoma enucleation by laparatomy. Six of them were treated with GnRHa every 28 days, three times before the operation. The remaining five patients who had no treatment prior to operation formed the control group. flood was drawn from the patients before treatment and on the day of operation for the assay of serum estradiol (E-2). Immunohistochemical localization of eNOS expression in leiomyoma and myometrium in treated patients, and in leiomyoma in the control group, was performed using monoclonal antibodies specific to eNOS. Results: All treated subjects showed a significant reduction of fibroid volume at the end of therapy. eNOS-positive cells were localized primarily within the vascular endothelium and smooth muscle cells, but had weak expression in fibroid and myometrial muscle cells in the treated group. The immunoreactivity was similar for both the leiomyoma and myometrium (P > 0.05). In contrast to this, the control group had shown strong expression in leiomyoma muscle cells (P < 0.005) in addition to the vascular endothelium and smooth muscle cells. Conclusion: GnRHa-induced tumor shrinkage should be due to diminished eNOS expression, most probably by lowering estrogen secretion. (C) 2000 International Federation of Gynecology and Obstetrics.Öğe Histopathologic comparison of conventional radial keratotomy and minimally invasive radial keratotomy in rabbits(Elsevier Science Inc, 1997) Er, H; Mizrak, BThe aim of the present study was to compare conventional radial keratotomy (RK) with minimally invasive RK (mini-RK) in terms of achieved incisional depth as well as the histopathologic changes in the rabbit corneal structures. Four conventional RK incisions were performed on the right eye and four mini-RK incisions were performed on the left eye of 12 Island rabbits using a centripetal cutting technique. The corneas were excised 20 days after the procedure and examined by light microscopy. Histopathologic examination showed that the mean achieved incisional depth (73.47%) in conventional RK was consistent with the intended incisional depth (80%), However, the mean achieved incisional depth (47.28%) was far from the intended incisional depth (80%) in eyes receiving mini-RK. The difference be tween achieved incisional depth of the two surgical techniques was statistically significant (t = 10.70, P < 0.05). Corneal structural changes and epithelial plug formations were less in eyes in mini-RK than in conventional RK. These findings suggested that the refractive results in mini-RK may be less effective than conventional RK. On the other hand, in the mini-RK group, less epithelial plug formation and limited histopathologic structural alterations may have an important role in preventing long-term overcorrection and corneal rupture after ocular trauma demonstrated in conventional RK technique.
