Öğe Corticosteroid-induced avascular necrosis of the femoral head is increased in the treatment of COVID-19 pandemic(2023) Oklu, Yunus; Ertem, Kadir; Aslantürk, OkanIschemia of an appendage caused by dressing, tourniquet or hair is a rare, but devastating clinical entity. A six-year-old girl presented to the emergency department with necrosis of her right thumb caused by compressive dressing for four days. The patient was treated with an immediate surgical decompression and the thumb was saved. The patient had a full range of the thumb at 10 months of follow-up. In conclusion, instead of waiting for full demarcation, timely surgical interventions may protect the fingers from being amputated.Öğe Distal humerus eklem içi AO/ASIF tip C kırığı nedeniyle ameliyat edilen hastaların fonksiyonel ve radyolojik sonuçlarının değerlendirilmesi(İnönü Üniversitesi, 2021) Oklu, Yunus; Karakaplan, MustafaAmaç: Bu çalışmanın hedefi distal humerus AO/ASIF 13-C1, 13-C2, 13-C3 tipi eklem içi kırık nedeniyle ameliyat edilen hastaların fonksiyonel sonuçlarını ve bu sonuçları etkileyen faktörleri belirleyip sunmaktır. Materyal metot: Ocak 2009–Mayıs 2020 yılları arasında İnönü Üniversitesi Tıp Fakültesi Turgut Özal Tıp Merkezi Ortopedi ve Travmatoloji Anabilim Dalında humerus alt uç kırık nedeniyle ameliyat edilen ve en az 6 ay takibi olan 47 hasta çalışmaya dahil edildi. AO/ASIF tip A ve tip B kırıklar, 18 yaş altı hastalar, telefonla ulaşılamayan ve takipten çıkmış hastalar çalışma dışı bırakıldı. Yaralanma mekanizması, kırık tipi, kullanılan cerrahi yöntem, plak yerleşim şekli, postoperatif harekete başlama süresi retrospektif olarak incelendi. Hastaların son kontrollerinde de grafi çekilerek osteoartrit ve heterotopik ossifikasyon varlığı değerlendirildi. Eklem hareket açıklığı hesaplandı. Kavrama ve sıkma güçleri dinamometre ile ölçüldü. Sonuçları değerlendirmek için hızlı DASH (Disabilitiy of Arm, Shoulder and Hand) ve MAYO (Mayo Elbow Performannce Score) anketleri kullanıldı. Ek olarak yaş, cinsiyet, plak tipi, ameliyat öncesi bekleme zamanı ve yaralanma şekli, kırık alt tipleri, kırık ekstremitenin dominant olup olmamasının ve komplikasyon varlığının fonksiyonel sonuçlar üzerindeki etkileri araştırıldı. Bulgular: Yaş ortalaması 43,7 (18-92) olan hastaların 29'u erkek, 18'i kadındı. Ortalama takip süresi 73,2 aydı. Hastaneye başvuru zamanı ile ameliyata kadar geçen süre ortalama 6,1 gündü. Hastaların 15'inde (%31,9) açık kırık mevcut olup 18 hastanın (38,3) dirsek kırığına ek olarak başka kırıkları mevcut idi. Kırık vakalarından 21'i (%44,7) ek hastalığa sahipti. Postoperatif dönemde atel ile immobilizasyon süresi ortalama 5,1 hafta iken hastaların fizik tedavi ile rehabilitasyona başlama zamanı ise ortalama 7,2 haftaydı. Son kontrolde sağlam tarafla kıyaslandığında ortalama ekstansiyon kaybı 17,2° iken, ortalama fleksiyon kaybının ise 16° olduğu tespit edildi. Ortalama eklem hareket açıklığı 120° olarak ölçüldü. Hastaların ortalama hızlı DASH skoru 15,1 puan ve ortalama MAYO skoru 88,4 puandı. Hastaların %91,5'inde iyi ve çok iyi sonuç elde edildi. Sonuç: Nadir olarak görülen distal humerus kırıklarında radyolojik görüntüden bağımsız olarak fonksiyonel sonuçlar genelde iyidir. Kırık dirsekte eklem hareket açıklığı anlamlı ölçüde azalmaktadır. Kırık tipi, yaş, preoperatif bekleme süresi, immobilizasyon süresi, eşlik eden yaralanma varlığı hızlı DASH ve MAYO puanını etkilemektedir. Cerrahi süre arttıkça komplikasyon oranı artmaktadır. Material and Method: A total of 113 patients who were admitted to our clinic for meniscal tear between 2009 and 2017 were included in our study. Sixty one patients who underwent arthroscopic meniscal repair and 52 underwent partial meniscectomy. All inside meniscal repair technique was used for all meniscal repairs. Clinical assessments included physical examination findings, Lysholm score, the IKDC score and visual analog score(VAS). Barret criteria were used for the clinical assessment of healing status. Results: The patients who underwent meniscal repair, 54 (88.5%) were male 7 (11.5%) were female. The mean age was 32 (19- 45) years. Our mean follow-up period was 47 (range; 24-109) months. Number of patients underwent only meniscal repair was 29 and concurrent anterior cruciate ligament reconstruction was 32. According to the Barret’s criteria’s patients had 18% surgical failure. The patients who underwent partial meniscectomy33 were male 19 were female. Our mean follow-up period was 29.5 (range; 24- 101) months. Eight patients (15.4%) had surgical failure. IKDC, Lysholm and VAS scores were statistically significantly improved postoperatively both in repair and partial meniscectomy group Conclusion: Meniscus repair should be performed whenever possible independent from age of patient. We aimed to present our clinical experience with HTS of toes and attract physicians' attention to this rare entity.Methods: Between January 2012 and September 2022; 26 patients (25 pediatric and one adult case) were treated for HTS. All pediatric cases were treated surgically under loop magnification. The adult patient was treated nonsurgically. The patient's age, gender, affected appendage and side, duration of symptoms, and postoperative complications were recorded.Results: Thirty-six toes of 25 patients (13 boys, 11 girls, and a male adult) were included in the study. The mean age of pediatric patients was 126.6 days. The third toe was the most affected (n:16), followed by the fourth (n:8). In seven patients more than one was affected.Conclusion: HTS should be treated as soon as possible when diagnosed to prevent further complications including appendage loss.& COPY; 2023 European Foot and Ankle Society. All rights reserved.Öğe Percutaneous needle aponeurotomy for the treatment of Dupuytren’s contracture(Eklem Hastalıkları ve Cerrahisi Dergisi (Eski Adı: Artroplasti Artroskopik Cerrahi Derg.), 2019) Karakaplan, Mustafa; Ertem, Kadir; Polat, Hacı; Sakçı, Mehmet Şah; Oklu, YunusObjectives: This study aims to present the early-period outcomes of patients with Dupuytren’s contracture (DC) treated with percutaneous needle aponeurotomy technique. Patients and methods: This retrospective study included 28 patients (20 males, 8 females; mean age 63 years; range, 44 to 88 years) (75 fingers) treated with percutaneous aponeurotomy due to DC between November 2011 and December 2015. Patients were evaluated according to their demographic characteristics, presence of additional disease, family history, history of drug use, complication rate, recurrence development, postoperative patient satisfaction, reoperational request, postoperative Disability of Arm Shoulder and Hand Questionnaire and visual analog scale scores. Results: Mean follow-up duration was 29 months (range, 12 to 60 months). In the postoperative satisfaction questionnaire, 92.9% (n=26) of patients stated that they were satisfied. In the questionnaire of reoperation request, 82.1% (n=23) of patients accepted the reoperation. While the complication rate was 39.3%, recurrence rate was 35.7%. Conclusion: Percutaneous needle aponeurotomy technique may be an effective, simple, and safe method for the treatment of DC. Materials and Methods: A hundred (56 female, 44 male) first year medical students were included in our study. A vitamin D screening form, which included 16 questions, was used to obtain data regarding medical students’ genetic, nutrition and cultural life. Results: Out of 100 medical students, 25 hydroxyl vit- D3 levels was below 5 ng/ml in 10%, 5-10 ng/ml in 10%, 10-20 ng/ml in 30%, 20-30 ng/ml in 40% and above 30 ng/ml in 10% medical students. While it has been found out that urbanization causes vitamin D deficiency and insufficiency in both females and males by negatively affecting sunlight exposure and nutrition, females have less sunlight exposure and their vitamin D levels are lesser when compared with males. Conclusions: In conclusion, urbanization decreases exposure to sunlight and changes nutritional habits and forms a basis for vitamin D deficiency.
