Öğe The importance of curettage in cases of sexual violence(2017) Celbiş, Osman; Börk, Turgay; Karaer, Abdullah; Ozel Ozcanlı, Özlem; Porgali Zayman, EsraAbstract: Sexual assault is among the gravest crime against the physical and moral integrity of an individual. When sexual assault results in pregnancy it causes further psychological and social damage. This study aims to examine the reasons for delayed reporting in cases of sexual assault that result in pregnancy, and the importance of abortion in such cases. Seven cases referred by the judicial authorities to Inonu University’s Turgut Ozal Medical Center for uterine evacuation are included in the study. The cases were examined in terms of age, whether the parents were still alive, place of residence, assaulter characteristics, mental levels of victims, psychiatric examination results, and gestational week upon referral to the hospital. The mean age of the cases was 17.42±2.76 years. Six of the cases were referred from Malatya and one was referred from a neighboring province. Four of the cases were past their 10th gestational week. Abortions were performed in six of the cases. One of the women, in the 19th week of pregnancy, decided not to have the abortion upon consultation with her family. The women in the cases were, on average, 76 days pregnant upon referral to the center. Their medical histories indicated that five of the cases hid their pregnancy because they were concerned about their family’s reaction. None of the cases knew that they were able to end the pregnancy until week 20. Pregnancy is one of the unwanted and preventable consequences of sexual assault. The family’s attitude toward the victim plays an important role in earlier detection of the cases. Bu çalışmanın amacı nükleer faktör-?B (NF-kB) aktivasyonu, myeloperoksidaz (MPO), antiinflamatuar sitokin interlökin-4 (IL-4) ve regulatuar sitokin transforming growth faktör-? (TGF-?) gibi bazı immün faktörleri şizofreni hastalarında ve yaş ve cinsiyet olarak eşleştirilmiş kontrol grubunda karşılaştırmaktır. Yöntem: Tedaviye dirençli 20 şizofreni hastalığı olan kişi ve yaş ve cin-siyet olarak uyumlu 20 sağlıklı kontrolde interlökin-4 (IL-4), transfor-ming growth faktör-? (TGF-?), myeloperoksidaz (MPO) ve nükleer faktör-?B aktivasyonunun (NF-?B) plazma düzeyleri analiz edildi. Hastalık şiddeti Kısa Psikiyatrik Değerlendirme Ölçeği kullanılarak de-ğerlendirildi (BPRS).Bulgular: Bu çalışmada antipsikotik tedaviye dirençli şizofreni hasta-larında serum TGF-? düzeyleri kontrollere göre anlamlı derecede daha düşük, NF-?B düzeyleri ise anlamlı derecede daha yüksek saptandı. Se-rum IL-4 ve MPO düzeylerinde ise hasta ve kontrol grupları arasında anlamlı fark bulunmadı.Sonuç: Bu çalışmada dirençli şizofreni hastalarında, psikotik belirtilerin alevli olduğu dönemde TGF-? seviyesinin düşük saptanması, Th1/Th2 dengesini sağlamakta yetersiz kaldığını göstermektedir. Bu durumun şi-zofrenide dirençten sorumlu olup olmadığının araştırılması için geniş ölçekli çalışmalara ihtiyaç vardır. Önceki çalışmalarda ilk atak tedavisiz şizofreni hastalarında gösterilen NF-?B deki yükselmenin, bu çalışmada tedaviye dirençli şizofreni hastalarında da gösterilmiş olması NF-?B'nin hastalığın fizyopatolojisine baştan beri dahil olduğunu düşündürmek-tedir.Öğe The Use of Handwriting Changes for the Follow-up of Patients with Bipolar Disorder(Turkish Neuropsychiatry Assoc-Turk Noropsikiyatri Dernegi, 2022) Ayaz, Nusret; Celbis, Osman; Porgali Zayman, Esra; Karlidag, Rifat; Oner, Bedirhan SezerIntroduction: Patients with bipolar disorder have motor skill deficiency and cognitive dysfunction. It is suggested that these deficiencies are related to the neurodevelopmental course of the disease. The aim of this study was to examine whether or not there was a change in the handwriting of patients with bipolar disorder in periods of mania and remission. Method: Two samples of handwriting and signature were taken from the cases, one on admission before treatment and one just after discharge. The first sample was taken in the period when the patients hospitalised with manic episode before medications and the second sample was taken in remission when the decision for discharge was made. The handwriting changes betwen the manic episode and the remission period were examined. Results: In the examination of the handwriting between the two periods. there was an increase in the size of the writing, pen pressure, the length of words, the spaces between words, and missing and incorrect punctuation marks in the manic episode. There were no differences in any handwriting measure on function of the specific diagnosis or the antipsychotic dose but tremor count was higher in patients using antipsychotics (p<0.05). Most of the parameters showing a change were found to be related to the Young Mania Rating Scale (YMRS) scores. Conclusion: The results of this study determined severe changes in the handwriting of patients with bipolar disorder in a period of mania. Handwriting features can be used to screen patients for discharge and can be used for prediction when a patient might switch into manic episode. Also intake YMRS scores were found as the most important feature that affect handwriting parameters in manic patients.
