Knee osteoarthritis often causes knee flexion contracture and the flexion contracture may affect posture and lumbar lordosis.Material and Methods: We performed a retrospective study by evaluating the records of patients who have undergone bilateral knee arthroplasty in our clinic between 2012 and 2016 in patients due to degenerative bilateral knee osteoarthritis and minimum 12 months follow up. Patients with prior hip surgery, prior spine surgery, rheumatic disease and postoperative flexion contracture were excluded. Lumber lordosis, sacral slope, pelvic tilt and pelvic incidence were measured on lateral lumbosacral view in supine position pre-postoperatively and compared with paired T test. Extension deficit was also measured preoperatively.Results: This study includes 83 patients (77 females and 6 males) who underwent bilateral knee arthroplasty and matched the inclusion and exclusion criteria. The average age at the time of first total knee arthroplasty was 64.4 years. Mean follow up was 37.3(12-66) months. The mean lumber lordosis was 43.9° preoperatively and 49.6° postoperatively. The increase in lumbar lordosis angle was statistically significant (p: 0,001). These changes were only related to the degree of extension deficit but not to age and postoperative follow-up time. Sacral slope, pelvic incidence and pelvic tilt did not change significantly. Conclusion: Bilateral knee arthroplasty increased lumbar lordosis patients with primer degenerative osteoarthritis especially who had high extension deficit. Sacral slope and pelvic tilt were decreased postoperatively but not significantly changed.Öğe Innovation of Strategies and Challenges for Fungal Nanobiotechnology(Springer International Publishing Ag, 2016) Saglam, Necdet; Yesilada, Ozfer; Cabuk, Ahmet; Sam, Mesut; Saglam, Semran; Ilk, Sedef; Emul, EzgiNanotechnology involves the study and use of materials under the 100 nm scale, exploiting the different physiochemical properties exhibited by these materials at the nanoscale level. Microorganisms are the best model and role of action for the nano/biotechnological applications. This technology has become increasingly important for the biotechnology and the related sectors. Promising applications have been already employed in the areas of drug delivery systems using bioactive nanoencapsulation, biosensors to detect and quantify pathogens, chemical and organic compounds, alteration of food compositions, and high-performance sensors and film to preserve fruits and vegetables. Moreover, the taste of food and food safety can be improved by new nano-materials from the microbiological sources. The huge benefits from this technology have led to increases in the market investments in nanoscience and nanoproducts in several areas.Öğe Multi–functional intramedullary self-locking ulna nailing system: Proximal oblique locking without the need of fluoroscopic guidance, and it’s effects on olecranon joint surface(2020) Saglam, Necdet; Aytekin, Mahmut Nedim; Ozturk, Recep; Akpinar, FuatAim: Due to the necessity of using the small diameter nails in the ulna, negative outcomes such as failure in the self-locking, long term exposure to radiation, and increased operation time are expected. Several clinical trials propose that an ideal nail system for unlar fixation challenge has not been developed yet. Material and Methods: This study involved the use of a new multifunctional intramedullary self-locking ulna nailing system and 36 intact dry cadaveric ulna bones. The relationship between the oblique self-locking screw, which has made a crucial contribution to the nailing system, and the joint surface of olecranon was investigated because this screw does not necessitate the use of fluoroscopic guidance and is easy to apply.Results: In our study, during anterior or medial or posterior oblique locking, no olecranon joint surface damage occurred on any bone with a locking rate of 100%. Conclusion: The clinical trials demonstrated that the proximal oblique locking was achieved easily in a short period of time, indicating the device’s immense value for the patient and the surgeon.
