Öğe The effects of the posttraumatic stress and general health status on paediatric burns and their sleep habits(Elsevier Sci Ltd, 2023) Duken, Mehmet Emin; Yayan, Emriye Hilal; Zengin, Muerside; Sevgi, TekcanBackground: Burns are serious injuries that can require hospitalization and have physical and emotional sequelae. Sleep disturbance can occur after trauma as evidenced by posttraumatic stress symptoms such as re-experiencing of a trauma with repetitive dreams, memories and flashbacks. This area has been minimally examined with pediatric burn survivors; therefore, the aim of this study was to investigate the effects of posttraumatic stress and general health on sleep habits of pediatric burn survivors from. Method: This is a correlational descriptive study. Its sample included 7-18-year-old inpatients at the burn clinic of a university hospital from January to November 2019. This study used the Posttraumatic Stress Disorder Reaction Index, the General Health Questionnaire (12) and the Children ' s Sleep Habits Questionnaire for data collection. Results: Of the participants, 58.6% were male, 46.5% were primary school students, and 62.6% had no diseases other than their burns. 20.2% of the children had epilepsy, 14.2% had diabetes and 3% had pneumonia. Of the child burn victims, 45.5% were brought to the hospital because of contact with fire. All of them had first-degree burns, 59.6% had seconddegree burns, and 80.8% had third-degree burns. After their burns, infections developed in 20.2% during trauma and in 43.4% during burn treatment. A structural equation model indicated that the child burn victims ' general health perceptions did not significantly affect their sleep habits (p > 0.05). A significant correlation was found between the children's posttraumatic stress disorder and their sleep habits (p < 0.05). Posttraumatic stress disorder explained 7.2% of their sleep habits. A positive and significant correlation was found between the children ' s general health perceptions and posttraumatic stress disorder (r = 0.74, p < 0.05). Conclusion: Burns of any kind can cause posttraumatic stress, sleep disorders and impairment of health status in children.Öğe Hemofilili çocuğa sahip ebeveynlerin bakım yükünün algılanan sosyal destek ve yaşam kalitelerine etkisinin incelenmesi(İnönü Üniversitesi, 2021) Sevgi, TekcanHemofilili Çocuğa Sahip Ebeveynlerin Bakım Yükünün Algılanan Sosyal Destek ve Yaşam Kalitelerine Etkisinin İncelenmesi Amaç: Araştırma, hemofilili çocuğa sahip ebeveynlerin bakım yükünün algılanan sosyal destek ve yaşam kalitesine etkisini değerlendirmek amacıyla yapıldı. Materyal ve Metot: Araştırma, Turgut Özal Tıp Merkezi Pediatrik Onkoloji/Hematoloji Polikliniğine Temmuz 2019 – Haziran 2020 tarihleri arasında başvuran hemofilili çocuğa sahip 44 ebeveynle tanımlayıcı olarak yapıldı. Veriler ''Ebeveyn Tanıtıcı Bilgi Formu'', ''WHOQOL-8.TR.Dünya Sağlık Örgütü Yaşam Kalitesi Anketi'', ''Algılanan Çok Boyutlu Sosyal Destek Ölçeği'', ''Zarit Bakım Yükü Ölçeği'' kullanılarak toplandı. Araştırmacı tarafından veri toplama araçlarıyla toplanan veriler SPSS 25.0 paket programında hesaplanmıştır. Ebeveynlerin tanımlayıcı özelliklerinin belirlenmesi için sayı ve yüzde, bakım yükü, algılanan sosyal destek ve yaşam kalitesi arasındaki ilişkinin belirlenmesinde korelasyon analizi, bağımlı ve bağımsız değişkenler arasındaki ilişkinin yönü ve şiddetinin belirlenmesinde path analizi kullanılmıştır. Bulgular: Hemofilili çocukların %18.2'sinde ağrı, %13.6'sında kanama olduğu, %43.2'sinin ayda bir hastaneye başvurduğu görüldü. Bakım veren bireylerin %68.2'sinin kadın olduğu, %65.9' nun çalışmadığı ve %75'inin ekonomik durumlarının orta olduğu bulundu. Bakım verme zorluğu arttıkça yaşam kalitesi puan ortalamasının azaldığı gözlendi. Bakım verenlerin %86.4'ünün herhangi bir destek almadığı, %70.5'in hastalığın maddi yük getirdiği tespit edildi. Sosyal destek puan ortalamaları artan ebeveynlerin yaşam kalitesinin arttığı gözlendi. Sonuç: Araştırmada ebeveynlerin bakım yükünün artması ve sosyal desteklerinin olmaması yaşam kalitesini etkileyen faktörler arasında bulundu. Hemşirelerin ebeveynleri gereksinimleri doğrultusunda bilgilendirmeleri, ebeveynlere profesyonel destek sağlamaları ve danışmanlık yapmaları önerilebilir. Material and Methods: This is a correlational descriptive study. The study was conducted with parents of 52 children who were admitted and treated in a paediatric burn clinic at a university hospital between January and December, 2019. The Zarit Care Burden Scale, the Multidimensional Scale of Perceived Social Support, and the State Anxiety Scale were used in the study. Path analysis was performed with observed variables, and structural equation model (SEM) was constructed using Amos programme. Results: The mean age of the parents was 30.94 +/- 7.33 years. In the study, the cause of burns in 55.8 % of the children was contact with flame (oil, thinner, and gas) and it was found that 82.7 % of the children had third degree burns and 65.4 % had second degree burns. A positive significant correlation was found between anxiety and caregiving burden, but a negative correlation was found between anxiety and parental social support. Conclusions: The study found that the level of anxiety and caregiving burden of parents of children with burns increased as their social support decreased. In line with the results of our study, it is seen that health professionals (doctors, nurses, psychologists, social workers) working in paediatrics can involve parents in care interventions by assessing their social support, caregiving burden, and anxiety. Practice Implications: It has been observed that the care burden and anxiety levels of burned children increase as the social support they receive from their parents decreases. Healthcare professionals (doctors, nurses, psychologists, social workers) working in this clinic should provide social, emotional, and psychological support to parents and share the burden of care. It has been shown that as social support increases, caregiving and anxiety levels can improve significantly.
