We present a case of AAC associated with ischemia of the gallbladder caused by aortic dissection Bakey type III.Öğe Appendiceal Enterobius vermicularis infestation in adults(Int College Of Surgeons, 2007) Isik, Burak; Yilmaz, Mehmet; Karadag, Nese; Kahraman, Latif; Sogutlu, Gokhan; Yilmaz, Sezai; Kirimlioglu, VedatThe objective of this study is to evaluate the incidence of Enterobius vermicularis in the appendices of the adult population and a possible relationship between E. vermicularis and acute appendicitis. E. vermicularis was identified in 18 (2%) of 890 patients. Six hundred sixty-five operations were performed for presumptive diagnosis of acute appendicitis, and E. vermicularis was found in 12 (2%) patients. The histopathological examination revealed acute inflammatory cells in four cases (33%). Three of these four specimens included luminal ova and one E. vermicularis. Histopathological examination of six cases revealed E. vermicularis in 225 incidental appendectomies with no evidence of either acute or chronic inflammatory cells. This study suggests a relationship between the presence of E. vermicularis ova and acute inflammation, but the presence of the pinworm in the lumen of the appendix is coincidental. On the other hand E. vermicularis in the appendix lumen can cause symptoms of acute appendicitis.Öğe CARCINOMA ERYSIPELOIDES FROM BREAST CANCER MIMICKING AS RADIODERMATITIS: REPORT OF A CASE(Aves, 2009) Sogutlu, Gokhan; Aydin, Cemalettin; Karadag, Nese; Olmez, Aydemir; Ozgor, Dincer; Deniz, SumerCarcinoma erysipeloides (CE) is an uncommon metastatic pattern arising from visceral carcinoma. As a consequence of lymphatic blockage by the tumor cells, erysipel like appearance can occur. It may be confused with erysipelas and other infl ammatory lymphedemas. We report a case of CE in a woman previously treated by mastectomy for locally advanced breast carcinoma. Intestinal ischemia is the most life-threatining one among these complications. In this paper, two cases of colon perforation, one of which with a mortal course, following coronary artery bypass grafting are presented. MATERIALS AND METHODS: 167 patients those have been followed by our clinic between 1998-2004 were included in this study. The mean age was 44,8. The patients were evaluated by physical examination, diagnostic mammography (above 35 years old) and ultrasonography (all patients) in the fi rst admittance. Biopsy was applied to 66 patients (39.5%) totally. Patients were classifi ed according to breast imaging reporting and data system (BI-RADS) and a certain follow-up protocol was initiated. RESULTS: In the cases (10,8 %) in whom benign calcifations were detected in their fi rst admittance, no changes in the calcifations were observed in 91,6 %. In 0,6 %, calcifations disappeared. In 25,2 % of the cases in whom mass was detected initially; the masses resolved in 22 %, remained same in 65 % and increased in size in 13 % in the fi nal evaluation. After the fi rst admittance, it was seen that menopause, oral contraceptives and hormone replacement therapy didn't aff ect the BI_RADS progression statistically. Benign pathological fi ndings were obtained in 100 % of US guided fi ne needle aspiration biopsies (FNAB) applied to BI-RADS 2 (n=58) and 3 (n=6) groups. CONCLUSIONS: Although in the follow-ups of the most of the patients with benign nonpalpable breast lesions or with negative mammographic and/or ultrasonographic lesions, clinical fi ndings remain the same, nearly half of the patients are subjected to biopsy in any time of their follow-up period. We think US guided FNAB is safe and reliable. On the other hand, mammographic and ultrasonographic BI-RADS can be evaluated together in the routine follow-ups of the patients with negative or benign fi ndings without biopsy relaibly in this manner.Öğe The Histopathological Effect of Resveratrol in Thyroid Tissue of Rats Exposed to Cigarette Smoke(Ortadogu Ad Pres & Publ Co, 2009) Kurus, Meltem; Sogutlu, Gokhan; Firat, Yezdan; Esrefoglu, Mukaddes; Yologlu, Saim; Ozturk, Feral; Otlu, AliObjective: The aim of this study was to assess the histopathological effect of resveratrol in thyroid tissue of rats exposed to cigarette smoke. Material and Methods: Forty adult, male Wistar Albino rats were divided into four groups for an experiment of 6 weeks duration. Group I was the control group. Rats in group 2 were exposed to cigarette smoke only and rats in group 3 received daily intraperitoneal injections of resveratrol (10 mg/kg/d). Animals in group 4 were exposed to both cigarette smoke and intraperitoneal injections of resveratrol. Rats of all groups were sacrificed; their thyroid glands were removed and were examined histopathologically. Results: While the control group and the resveratrol group had normal thyroid tissue, in the group exposed to cigarette smoke there was a significant decrease in follicles and differentiation of epithelial cells from cubic to flat cells. There was intracytoplasmic vacuolization in some epithelial cells, irregularity in follicular cells decreasing area and cell infiltrations. On the other hand, we observed significant improvement in these histopathological differences in the group that was exposed to both cigarette smoke and resveratrol. Conclusion: Resveratrol has healing effects on the damage of thyroid tissue of rats that are exposed to cigarette smoke at a dose and duration tested in this study.Öğe Hydatid cyst of the thyroid gland: report of three cases(Springer, 2013) Yilmaz, Mehmet; Akbulut, Sami; Sogutlu, Gokhan; Arabaci, Ebru; Kayaalp, CuneytCystic Echinococcosis is a parasitic infestation that is distributed world-wide. It may be found in nearly any part of the body, most often in the liver and the lungs, but occasionally in other structures such as the thyroid gland. The present study reports three cases of hydatid cysts of the thyroid gland, in patient ranging from 18 to 25 years of age. Two patients had concomitant hydatid disease involving organs other than the thyroid gland (secondary disease), and one had, sole, involvement of the thyroid gland itself (primary disease). Moreover, an occult papillary thyroid carcinoma was detected incidentally in one case, involving the unilateral thyroid lobe as the hydatid cyst. While several surgical procedures including left lobectomy and isthmectomy were undertaken in one patient, two patients underwent total thyroidectomy. No disease recurrence was observed in any of the three patients during the postoperative follow-up period. No study reporting the concomitance of hydatid cyst and neoplasia of the thyroid gland has been previously published. This concomitance indicates the importance of the differential diagnosis of lesions characterized by calcifications in the thyroid gland, especially in endemic regions.Öğe Kaposi's sarcoma after liver transplantation from a donor with a history of ventriculoperitoneal shunt and craniotomy for primary central nervous system lymphoma: Report of a case(Springer, 2008) Isik, Burak; Yilmaz, Sezai; Kirimlioglu, Vedat; Kirimlioglu, Hale; Yilmaz, Mehmet; Sogutlu, Gokhan; Ara, CengizThe transplantation of organs from donors who have undergone shunt surgery or craniotomy for a malignant central nervous system (CNS) tumor is controversial. We report a case of Kaposi's sarcoma (KS) developing as a result of immunosuppression in the recipient of a liver transplant from a donor who underwent craniotomy and ventriculoperitoneal shunt surgery for primary CNS lymphoma. Polymerase chain reaction assay did not isolate human herpes virus-8 in the Kaposi lesions. To our knowledge, this is the only case ever reported of KS developing after liver transplantation from a donor with lymphoma. Thus, with appropriate screening to exclude possible dissemination, patients with a history of high-grade primary CNS lymphoma treated by ventriculoperitoneal shunt and craniotomy may be accepted as donors.Öğe Melatonin Prevents Cyclosporine-Induced Hepatotoxicity in Rats(Karger, 2009) Kurus, Meltem; Esrefoglu, Mukaddes; Sogutlu, Gokhan; Atasever, AlperObjectives: Cyclosporine A (CsA) is a widely used immunosuppressive agent that is implicated in the formation of free oxygen radicals. Melatonin is known to be a free radical scavenger and an antioxidant agent. This study was designed to investigate the effects of melatonin on CsA-induced liver damage by histopathological examination. Materials and Methods: Thirty-two male rats of Sprague-Dawley origin were divided into 4 groups of 8 and treated for 28 days as follows: group 1 received daily doses of 0.1 ml/kg olive oil s.c.; group 2 received 4 mg/kg of melatonin; group 3 received 10 mg/kg CsA diluted in 0.1 ml/kg olive oil; group 4 was treated with 4 mg/kg melatonin i.p. and 10 mg/kg CsA s.c. Finally, the rats were sacrificed by terminal anesthesia, and liver tissue specimens were processed for light microscopy, stained with HE and examined under a light microscope. Results: Specimens of the control group showed normal liver histology, whereas group 3 showed major histopathological changes, such as cytoplasmic vacuolization, dilatation of the sinusoids, apoptosis and many mitotic figures. In group 4, the normal histology of the liver was preserved, although apoptosis, mitotic figures and cytoplasmic vacuolization were still infrequently observed. Nevertheless, there were significant differences between group 2 (melatonin) and group 3 (CsA) and between group 3 (CsA) and group 4 (CsA + melatonin) concerning these 3 parameters (vacuolization, sinusoidal dilatation and apoptosis). Conclusion: The results of this study suggest that CsA-related liver toxicity in rats could be significantly reduced by melatonin administration. Copyright (C) 2009 S. Karger AG, BaselÖğe The Morphological and Numeral Changes of Goblet Cells in the Jejunum of Ischemia-Reperfusion Applied Rats(Ortadogu Ad Pres & Publ Co, 2010) Dag, Meral; Kurus, Meltem; Sogutlu, Gokhan; Estegoglu, Mukaddes; Ozturk, Feral; Yologlu, Saim; Otlu, AliObjective: In this study, we aimed to investigate the numeric and morphological changes of goblet cells after experimental ischemia and reperfusion damage. Material and Methods: In our study, male Sprague Dawley rats were randomly divided into four groups being eight rats in each group. Groups were: control, sham, ischemia (15 minutes) and ischemia-reperfusion groups (I/R, 15 min. I/15 min.R). After the experiment, all rats were sacrificed and their jejunums were taken out. All the necessary procedures were carried out for morphological evaluation. Results: In the ischemia group, villi morphological changes from splitting up to atrophy, and were shorter; goblet cells were found to show reduction both in number and size. On the other hand, it was found that goblet cells in the crypts showed no change in number but in size. Findings in the I/R group were milder than the ischemia group, but the sizes of villi were found smaller in this group. While the number of goblet cells in the villi was normal, it was decreased in the crypts and the sizes of goblets both in the villi and in the crypts were reduced. Conclusion: We thought that during the ischemia, the number of goblets were decreased since they fall into the lumen. The rest were small because they came from the basis of the crypts; during the I/R procedure, the number of cells turned to normal because of the arrival of the goblets from the crypts to the villi. However, in this way the number in them in the crypts decreased and they looked small due to incomplete maturation.Öğe Oral L-arginine protects against cyclosporine-induced hepatotoxicity in rats(Elsevier Gmbh, 2008) Kurus, Meltem; Esrefoglu, Mukaddes; Karabulut, Aysun Bay; Sogutlu, Gokhan; Kaya, Mine; Otlu, AliCyclosporine A (CyA) leads to liver injury, probably by causing the production of free radicals and resulting in nitric oxide (NO) deficiency. We evaluated CyA-mediated liver damage histopathologically to determine the possible beneficial effects Of L-arginine (L-Arg). In this study, 7 groups of Sprague-Dawley rats; (1) Control group; (2) 0.9% NaCl group; (3) CyA group: 7.5 mg/kg/day; (4) L-Arg group: 2 g/It/day; (5) L-NAME (N-nitro-L-arginine methyl ester) group: 5mg/100ml/day; (6) CyA+L-Arg group: L-Arg (2 g/It/day) + CyA (7.5mg/kg/day); and (7) CyA + L-NAME group: CyA (7.5mg/kg/day) + L-NAME (5mg/100ml/day) were included. At the end of the treatments, animals were killed and hepatic tissues were treated for morphological (hematoxylin and eosin) and biochemical (NO and malondialdehyde, NIDA) analyses, and serum was processed for biochemical (alanine transaminase (ALT), aspartate transaminase (AST), bilirubin, alkaline phosphatase (ALP) and total protein) study. The results indicated that CyA-induced hepatotoxicity was characterized by sinusoidal dilatation, hepatocellular vacuolization, neutrophilic infiltration and hepatocellular necrosis. These findings were less pronounced in the CyA + L-Arg group than CyA alone group. L-NAME group showed moderate changes. The CyA + L-NAME (Group 7) had more severe changes. We found changes in tissue NO and NIDA levels. We think that the tissue damage caused by CyA is mild and reversible at the period when biochemical parameters are just starting to become abnormal and that L-Arg may have a protective effect against CyA damage on liver. All rights reserved.Öğe Postoperative Hypoparathyroidism in Total Thyroidectomy: Incidence and Predictive Factors(Galenos Yayincilik, 2007) Sogutlu, Gokhan; Cikim, Ayfle Sertkaya; Olmez, Aydemir; Sahin, Brahim; Cikim, Kerim; IsiK, Burak; Cinpolat, OzgurObjective: Total thyroidectomy is increasingly accepted as a choice of treatment not only for malignant but also for benign thyroid disorders. Nowadays, postoperative hypocalcaemia is the major concern in total thyroidectomy. The aim of this study is to quantify risk factors contributing to postoperative hypocalcaemia rates. Material and methods: Medical records of 88 patients who underwent total thyroidectomy from 2000 to 2004 were reviewed. In addition to demographic information, postoperative hypocalcaemia and related risk factors were identified. Results: The most common indication for total thyroidectomy was multinodular goiter (53.4%). One patient with anaplastic thyroid carcinoma died because of respiratory failure (1.1%). Temporary and permanent hypoparathyroidism rates were 26.1% and 3.4 %, respectively. Parathyroid reimplantation was performed to 7 patients (7.9%). Indication of surgery was statistically associated with an increased incidence of hypocalcemia (P=0.019 for thyroid carcinoma and P=0.005 for completion thyroidectomy), whereas, sex, age, neck dissection and parathyroid reimplantation were not. Conclusions: Postoperative hypocalcaemia is the major concern for thyroid surgeons. Completion and total thyroidectomy for thyroid malignancy increased postoperative hypoparathyroidism. With meticulous attention to operative technique and anatomical detail, surgeons can achieve low morbidity rates.Öğe Tuberculosis of the Thyroid Gland: An Uncommon Cause of Thyroid Nodules(Galenos Yayincilik, 2006) Sogutlu, Gokhan; Isik, Burak; Yilmaz, Mehmet; Karadag, Nese; Olmez, Aydemir; Kebebew, Electron; Kirimlioglu, VedatWe present the case of a 33-year-old woman with tuberculosis of the thyroid. She presented with a thyroid mass, weakness and palpitation for two years. The diagnosis was established on histological examination after thyroid lobectomy. She had an uneventful recovery and received antituberculous medications.
