Mostly, they originate from the posterior area of the prostate and asymptomatic. Anterior location of the prostatic cyst is rarer than posterior. The prostatic cyst in a 41 year-old man presenting with acute or nary retansion. Transurethral resection FUR) of the cyst was performed, which revealed a benign cyst lined with columnar epithelium and proliferative urothelial lining on histopathological evaluation. Prostatic cysts particularly in young men with obstructive lower urinary tract symptoms are rarely seen. Management of the prostatic cyst with TUR seems to be a minimally invasive approach with successful outcomes.Öğe Does preoperative neutrophil-to-lymphocyte ratio have a value in predicting recurrence in bladder tumour after intravesical BCG treatment?(Scientific Publishers India, 2017) Camtosun, Ahmet; Celik, Huseyin; Altintas, Ramazan; Topcu, Ibrahim; Tasdemir, CemalThe purpose of this study was to evaluate the prognostic value of the ratio of preoperative neutrophil to lymphocyte after intravesical BCG treatment in non-muscle invasive bladder cancer (NMIBC). The data of 89 patients, operated for urothelial carcinoma and treated with BCG after the diagnosis of NMIBC between 2007 and 2014, were reviewed retrospectively. Preoperative complete blood counts and first cystoscopic examination findings of the patients were examined. Median follow-up was 28.7 months. Tumour recurrence was detected in 33 patients (Group 2) of 89 patients. There was no tumour recurrence in 56 patients (Group 1). Neutrophil/lymphocyte ratio, MPV and CIS incidence were significantly increased in Group 2. This study showed that, preoperative high neutrophil/lymphocyte ratio, MPV and CIS occurrence can be used as independent criteria to predict tumour recurrence after intravesical BCG treatment in NMIBC.Öğe Evaluating the concordance between Vesical Imaging Reporting and Data System scores and bladder tumor histopathology(Elsevier Singapore Pte Ltd, 2025) Gungor, Hasan; Camtosun, Ahmet; Topcu, Ibrahim; Karaca, LeylaObjective: This study aimed to assess the local staging of bladder tumors in patients utilizing preoperative multiparametric MRI (mpMRI) and to demonstrate the clinical efficacy of this method through a comparative analysis with corresponding histopathological findings. Methods: Between November 2020 and April 2022, 63 patients with a planned cystoscopy and a preliminary or previous diagnosis of bladder tumor were included. All participants underwent mpMRI, and Vesical Imaging Reporting and Data System (VI-RADS) criteria were applied to assess the recorded images. Subsequently, obtained biopsies were histopathologically examined and compared with radiological findings. Results: Of the 63 participants, 60 were male, and three were female. Categorizing tumors with a VI-RADS score of >3 as muscle invasive, 84% were radiologically classified as having an invasive bladder tumor. However, histopathological results indicated invasive bladder tumors in 52% of cases. Sensitivity of the VI-RADS score was 100%; specificity was 23%; the negative predictive value was 100%; and the positive predictive value was 62%. Conclusion: The scoring system obtained through mpMRI, VI-RADS, proves to be a successful method, particularly in determining the absence of muscle invasion in bladder cancer. Its efficacy in detecting muscle invasion in bladder tumors could be further enhanced with additional studies, suggesting potential for increased diagnostic efficiency through ongoing research. The VI-RADS could enhance the selection of patients eligible for accurate diagnosis and treatment.Öğe Evaluation of reproductive and renal toxicity of varenicline in male rats(Mashhad Univ Med Sciences, 2019) Oguz, Fatih; Beytur, Ali; Taslidere, Elif; Parlakpinar, Hakan; Oguz, Hilal Kurnaz; Polat, Alaaddin; Topcu, IbrahimObjective(s): Varenicline is a selective partial agonist for the nicotinic acetylcholine receptor a4b2 subtype, which is widely used to treat smoking addiction. However, there is still no data about its potential toxic effects on tissues. In this study, we aimed to determine the varenicline-induced toxicity on reproductive and renal tissues in rats. Materials and Methods: Rats were randomly divided into two groups: control (n=10) and varenicline (n=24). Then, rats in each group were sub-divided equally as acute and chronic groups. The control rats were orally given distilled water only. Varenicline was administrated orally as follows: 1st-3rd days 9 mu g/kg/day, 4th-7th days 9 mu g/kg twice daily, and 8th-90th days 18 mu g/kg twice daily. The rats of acute and chronic groups were sacrificed on the 45th and 90th days, respectively. Some tissue markers related to oxidative stress were measured, and sperm characteristics were observed. Results: In the acute group, varenicline led to a significant decrease in SOD activities in both kidney and testis tissues. In the chronic group, varenicline significantly increased MDA and MPO production, and reduced CAT and GPx levels in the kidneys and testes. Also, SOD and GSH levels significantly decreased in the testes. Moreover, sperm characteristics were negatively affected; histopathological deformation was observed in the testes and kidneys in all groups. Conclusion: This study showed that varenicline could detrimentally affect the kidneys and testes in both acute and chronic term usage. Further studies will provide more insights into the molecular dynamics of this damage.Öğe Forecasting Stone-Free Status Following Percutaneous Nephrolithotomy Utilizing Explainable Machine Learning(Mdpi, 2026) Cicek, Resul; Topcu, Ibrahim; Dural, Bulut; Balikci Cicek, Ipek; Yilmaz, Murat; Colak, CemilBackground: This study aimed to create and evaluate explainable machine learning models for forecasting postoperative stone-free status following percutaneous nephrolithotomy (PNL) utilizing a substantial clinical cohort. Methods: This retrospective single-center analysis encompassed 2144 adult patients who received PNL from 2010 to 2024. We employed clinical, radiographic, stone-related, and surgical data to train four supervised machine learning models: Extreme Gradient Boosting (XGBoost), Random Forest, Light Gradient Boosting Machine (LightGBM), and Adaptive Boosting (AdaBoost). We used the Synthetic Minority Oversampling Technique exclusively on the training set to fix the class imbalance. We assessed the model's accuracy, precision, recall, F1-score, and area under the receiver operating characteristic curve (ROC-AUC) to see how well it worked. SHapley Additive exPlanations (SHAP) were used to measure explainability. Results: The total stone-free rate was 84.8%. XGBoost had the best predictive performance of the models tested, with an accuracy of 0.916 and a ROC-AUC of 0.975. LightGBM was close behind. Random Forest and AdaBoost had relatively inferior performance. SHAP analysis identified anatomical anomalies as demonstrated the strongest association with stone-free outcomes. The size of the access sheath and the number of stones were next. Other parameters that were identified by SHAP as important contributors to model predictions were the placement of the stone, Guy's Stone Score, the length of the operation, and the density of the stone. These feature associations demonstrated clinical coherence with established knowledge in surgical practice. Conclusions: Explainable machine learning algorithms, especially XGBoost, can accurately predict stone-free outcomes following PNL in a way that makes sense to doctors.
