Öğe Assessment of a trial electromechanical delay in patients with mitral valve prolapse(2011) Acikgoz N.; Yagmur J.; Ermis N.; Cansel M.; Atas H.; Tasolar H.; Pekdemir H.Atrial arrhythmias are seen frequently in symptomatic patients with mitral valve prolapse (MVP). The purpose of our study was to evaluate whether the atrial electromechanical delay (AEMD) measured by tissue Doppler imaging (TDI) is prolonged in patients with MVP. A total of 40 patients with MVP (16 males/24 females, 33.4 ± 6.1 years), and 40 controls (18 males/22 females, 34.2 ± 4.2 years) were included in the study. Inter-AEMD and intra- AEMD were measured by TDI. P-wave dispersion (PWD) was calculated from the 12-lead electrocardiogram. Inter-AEMD and intra-AEMD were significantly longer in patients with MVP than in the controls (31.6 ± 12.1 vs 24.7 ± 5.4 ms, p = 0.001 and 8.1 ± 5.3 vs. 5.7 ± 1.9 ms, p = 0.008; respectively). PWD was significantly longer in patients with MVP than in the controls (41.3 ± 7.1 vs 34.7 ± 4.3 ms, p<0.0001). The left atrial (LA) diameter, anterior and posterior mitral leaflet thicknesses were significantly higher in patients with MVP than in the controls. (35.4 ± 3.0 vs 31.9 ± 3.0 mm, p<0.0001 and 3.6 ± 0.9 vs 2.8 ± 0.7 mm, p<0.0001 and 2.9 ± 0.7 vs 2.2 ± 0.4 mm p<0.0001; respectively). Inter-AEMD was positively correlated with PWD, mitral leaflet thickening and LA diameter. We showed that AEMD is significantly prolonged in patients with MVP, and speculated that this prolongation may reflect the increase of the risk of atrial arrhythmias in MVP subjects. © 2011 Academic Journals.Öğe Assessment of atrial conduction time in patients with behçet's disease(Sociedade Portuguesa de Reumatologia, 2014) Cansel M.; Yagmur J.; Taşolar H.; Karincaoglu Y.; Ermis N.; Acikgoz N.; Bayramoglu A.Objective: Behçet's disease is characterized by increased inflammatory activity, and there there might be an increased risk of atrial arrhythmia in patients with this disease. Our study is aimed to evaluate a novel method of measuring atrial electromechanical features expressed as interatrial and intraatrial electromechani - cal delay by tissue Doppler echocardiography in patients with Behçet's disease. Methods: We evaluated 57 patients (mean age: 36.3±12.1 years) with Behçet's disease and 34 sex and age matched healthy volunteers (mean age: 38.4±8.6 years) as control group. P-wave dispersion (PWD) was calculated from the 12-lead surface ECG, interatrial and intraatrial electromechanical delay were measured by tissue Doppler imaging and conventional echocardiography. Results: Interatrial electromechanical delay and intraatrial electromechanical delay were prolonged in patients with active Behçet's disease compared with the patients with inactive disease and the controls (p<0.0001, p<0.0001, p=0.013 and p=0.001, respectively). Erythrocyte sedimentation rate and high-sensitivity C-reactive protein values of of patients with active Beh - çet's were significantly higher than those with inactive Behçet's disease and the controls (p<0.0001 and p<0.0001, respectively). High-sensitivity C-reactive protein and erythrocyte sedimentation rate were correlated with interatrial electromechanical delay in patients with Behçet's disease (r=0.44, p=0.001 and r=0.64, p<0.0001, respectively). We report a giant left atrial ball thrombus which was removed under surgery uneventfully, in a 48-year-old male patient with the presence of sinus rhythm and no valvular disease with previous off-pump coronary artery bypass surgery.Öğe Multivessel coronary thrombosis in a patient with idiopathic thrombocytopenic purpura(2012) Yagmur J.; Cansel M.; Acikgoz N.; Yagmur M.; Eyupkoca F.; Ermis N.; Akturk E.A 49-year-old woman who had idiopathic thrombocytopenic purpura was admitted to our hospital with severe chest pain. Electrocardiography revealed inferolateral myocardial infarction. The patient underwent immediate coronary angiography, which revealed thrombi in the left coronary system. Percutaneous intervention was not indicated, because the thrombi had occluded the distal segments of multiple coronary arteries. Administration of tirofiban satisfactorily dissolved the thrombi. © 2012 by the Texas Heart® Institute, Houston.
