Öğe Arthroscopy-aided fixation for metacarpophalangeal joint collateral ligament injury accompanied by osteochondral avulsion injury: Our results and surgical technique(2018) Gulec, Ali; Yildirim, AhmetAim: Though metacarpophalangeal joint collateral ligament injuries are commonly observed, the presence of accompanying avulsed osteochondral fragment or aponeurosis injury in adult patients is a situation that may require definite surgical treatment. Our study investigated patients attending our clinic after acute hand trauma with radial or ulnar collateral ligament injury who underwent arthroscopy-aided surgical treatment and who regularly attended check-ups to examine the efficacy and results of arthroscopyaided treatment for collateral ligament injury of the metacarpophalangeal joint. Material and Methods: The study assessed fracture healing, joint movement angles and complications of 11 patients who attended our hand surgery department from December 2015 to April 2018 who had ulnar or radial collateral ligament injury of the metacarpophalangeal joint with accompanying osteochondral avulsion fracture repaired with the aid of arthroscopy with at least 1 year of continuous attendance at check-ups. Results: Four patients had 1st proximal finger avulsion fracture accompanying ulnar collateral ligament injury, four patients had 2nd proximal finger avulsion fracture accompanying ulnar collateral ligament injury, 1 patient had 4th finger and 2 patients had 5th finger proximal phalanx fractures accompanying radial collateral ligament injury. The patients’ avulsion fractures and ligament injuries healed without problem, 2 patients had limited joint movement angles and 1 patient developed complex regional pain syndrome. Conclusion: It is considered that the results of arthroscopy-aided collateral ligament injury repair and avulsed fragment fixation are good and perfect healing.Öğe The effect of patient position to dexa measurement coronary artery disease(2018) Ataoglu, Muhammet Baybars; Ozer, Mustafa; Ayanoglu, Tacettin; Yildirim, Ahmet; Hatipoglu, M. Yasin; Cetinkaya, MehmetAim: The aim was to compare and contrast the lumbal area DEXA measurements made in lateral and front-back positions in the clinically diagnosed female patients with osteoporosis. Material and methods: From the female patients that were referred to our clinic due to low energy fractures that occurred in areas other than the vertabrae, whom were thought to have osteoporotic fractures, were postmenaposal with no known history of diseases, and medication that can lead to osteoporosis and fracture, with no known previous diagnosis of osteoporosis, 39 patients have been included in the study. Anteroposterior and lateral standart lumbar DEXA measurements were performed on all patients Results: The results of our study revealed that the the laterally performed lumbar Dual Energy X-ray Absorbsiometer (DEXA) measurements have diagnostic advantage in osteoporosis over anteroposteriorly performed measurements . The antiosteoporotic treatment would have beeen prescribed to 82% of the patients if the anteroposteriorly performed lumbar region Dual Energy X-ray Absorbsiometer (DEXA) measurements were taken into account, while when laterally performed measurements were taken into account the percentage was 97.5%. Conclusion: If lateral lumbar area Dual Energy X-ray Absorbsiometer (DEXA) measurements are used in diagnosing osteoporosis more patients would receive the required treatment and the risk of the osteoporotic low energy fractures could be lowered.Öğe Multiple Cervical Spinous Process Fracture (Clay Shoveler Fracture): Case Report(2017) Ayanoglu, Tacettin; Ataoglu, Muhammet Baybars; Ozer, Mustafa; Yildirim, Ahmet; Tokgoz, Mehmet AliClay-Shoveler’s is fracture of one or more spinous processes to include lower cervical or upper thoracic vertebrae. In this article, it was aimed to investigate possible mechanisms of Clay-Shoveler’s fracture and share radiological results of q case of 11 years (2003) follow-up after the traffic accident. A 25-year-old male patient complained of neck pain spreading to both shoulders, and there was an in-car traffic accident two weeks ago. Patient’s motion restriction was 50% for active flexion, lateral flexion and rotation, and 20% for active extension movement. Imaging revealed a minimal inferior displaced avulsion fracture in C6, C7 and T1 vertebra spinous processes. Patient returned to daily activities in 3rd month after immobilization with medical treatment and neck collar. Clay-Shoveler’s fracture is most commonly seen in T1, then C7, T2, T3 and C6. The aim of our study was to determine the effect of paricalcitol on experimentally induced arthritis. Materials and methods: Type II collagen combined with Freund's adjuvant was applied to induce arthritis in Wistar albino female rats. Paricalcitol (0.3 mu g/kg daily) was subcutaneously injected starting 1 day after collagen applications (prophylactic group) or 1 day after the onset of arthritis (therapeutic group), until day 29. Results: The 29th day arthritis scores were lower compared to the 13th day scores in the paricalcitol groups (P < 0.05), while they were higher in the arthritis group (P < 0.05). Marked cartilage-bone destruction and extensive perisynovial inflammation were detected in the arthritis group. Decreased cartilage-bone destruction and perisynovial inflammation in the paws were observed in the paricalcitol groups. The tissue mRNA levels of DKK1, Wnt5a, and axin-2 were higher in the arthritis group than in the control group. In the paricalcitol groups, mRNA expressions were lower than in the arthritis group. Conclusion: The present study shows that the Wnt/beta-catenin signaling pathway is active in arthritis. Moreover, paricalcitol ameliorates arthritis via inhibiting the Wnt/beta B-catenin pathway. The purpose of this study is to research the therapeutic effect of resveratrol in collagen-induced arthritis (CIA) model in rats and whether resveratrol affects the activities of signaling pathways those are potent pathogenic actors of rheumatoid arthritis. Arthritis was induced by intradermal injection of chicken type II collagen combined with incomplete Freund's adjuvant in Wistar albino rats. One day after the onset of arthritis (day 14), resveratrol (20 mg/kg/day) was given via oral gavage, until day 29. The paws of the rats were obtained for further analysis. Tissue Wnt5a, mitogen-activated protein kinase (MAPK), Src tyrosine kinase and signal transducer, and activator of transcription-3 (STAT3) mRNA expressions were determined by real-time polymerase chain reaction. Resveratrol ameliorated the clinical and histopathological (perisynovial inflammation and cartilage-bone destruction) findings of inflammatory arthritis. The tissue mRNA expressions of Wnt5a, MAPK3, Src kinase, and STAT3 were increased in the sham group compared to the control group. Resveratrol supplement decreased their expressions. The present study shows that Src kinase, STAT3, and Wnt signaling pathway are active in the CIA model. Resveratrol inhibits these signaling pathways and ameliorates inflammatory arthritis.
