Öğe Clinical results of anterior cruciate ligament reconstruction with bone-patellar tendon-bone autograft: A prospective study(2018) Aslanturk, Okan; Ergen, Emre; Gormeli, Ayse Cemile; Gormeli, Gokay; Yilmaz, OzgurAim: Anterior cruciate ligament(ACL) injury is the most common ligament injury of knee. Gold standard treatment of ACL rupture is reconstruction of ligament with autograft or allograft. There is still controversy in graft choice in literature. In this study, we aimed to present prospective follow-up results of patients who were reconstructed with bone-patellar tendon-bone autograft for ACL rupture. Material and Methods: Twenty-seven patients were operated for ACL rupture with bone-patellar tendon-bone(B-PT-B) autograft ACL reconstruction. Patients were followed for at least 2 years. Patients were evaluated preoperatively and postoperatively with Lysholm knee function score, IKDC objective score and Tegner activity level scale. Development of arthritis were evaluated radiologically. Results: All of operated patients were male. The mean age of the patients was 25.4 (range 19-35 years). The mean follow-up time was 27.9 (range 24-30 months). There was a statistically significant increase in all postoperative knee scores. Hematoma, requiring surgical drainage, developed in two patients. Six patients had anterior knee pain while kneeling at final follow up. Conclusion: ACL rupture causes knee instability that can lead early knee arthritis and meniscal injuries. Patients with ACL rupture showed increased knee stability and functional scores after reconstruction. Anterior cruciate ligament reconstruction with B-PT-B autograft incorporates and heals faster with low rerupture and complication rate.Öğe Complications after the percutaneous release of trigger thumb in adults(2020) Aslanturk, Okan; Yilmaz, Ozgur; Ari, Bunyamin; Canbay, Ali; Kavak, HüseyinAim: Trigger finger is a common orthopedic problem causing pain and could restrsict daily activities. Surgical intervention can be done via open or percutaneously. There is not much data about minor complications in literature. In this study, we reported our results and complications of percutaneous release (PR) of trigger thumb.Material and Methods: Retrospective data of patients treated for trigger thumb with PR between September 2017 and January 2019 were reviewed. Age, gender, affected side, preoperative Quinnell grade of triggering; previous history of steroid injection and history of medical diseases (e.g. diabetes mellitus, rheumatoid arthritis) were recorded.Results: Thirty thumb of 28 patients who met the inclusion criteria treated with PR for trigger thumb included in the study. Mean age of patients was 53.7 ±9.95 months (range 36-73). Twenty (71%) of patients were female, 8 (29%) were male. Right hand was dominant side in all patients. Right thumb was affected in 18 (64%) patients, left thumb in 8 (29%) and 2 (7%) were bilateral. Due to Quinnell grading system 9 (32%) patients were grade 2 and 19 (68%) were grade 3. Twenty (71%) of patients had previous history of steroid injection. There were 5 complications in our patients. One recurrence of triggering and 4 ecchymosis and edema have been reported.Conclusion: Percutaneous release of trigger thumb is safe and reliable technique with low complication rate. To prevent complication surgeon should be careful during the procedure and should well inform patient what to do after procedure.Öğe Exploiting ionisable nature of PEtOx-co-PEI to prepare pH sensitive, doxorubicin-loaded micelles(Taylor & Francis Ltd, 2020) Ozturk, Naile; Kara, Asli; Gulyuz, Sevgi; Ozkose, Umut Ugur; Tasdelen, Mehmet Atilla; Bozkir, Asuman; Yilmaz, OzgurAims This study was conducted to evaluate block copolymers containing two different poly(ethyleneimine) (PEI) amounts, as new pH-sensitive micellar delivery systems for doxorubicin. Methods Micelles were prepared with block copolymers consisting of poly(2-ethyl-2-oxazoline)-co-poly(ethyleneimine) (PEtOx-co-PEI) and poly(epsilon-caprolactone) (PCL) as hydrophilic and hydrophobic blocks, respectively. Doxorubicin loading, micelle size, pH-dependent drug release, and in vitro cytotoxicity on MCF-7 cells were investigated. Results The average size of drug-loaded micelles was under 100 nm and drug loading was between 10.7% and 48.3% (w/w). pH-sensitive drug release was more pronounced (84.7% and 68.9% (w/w) of drug was released at pH 5.0 and pH 7.4, respectively) for the micelles of the copolymer with the lowest PEI amount. The cell viability of doxorubicin-loaded micelles which were prepared by the copolymer with the lowest PEI amount was 28-33% at 72 h. Conclusions PEtOx-co-PEI-b-PCL micelles of this copolymer were found to be stable and effective pH-sensitive nano-sized carriers for doxorubicin delivery.Öğe Hair tourniquet syndrome of toe(Elsevier, 2023) Koroglu, Muhammed; Ozdes, Huseyin Utku; Ozbey, Rafet; Yilmaz, Ozgur; Ergen, Emre; Oklu, Yunus; Aslanturk, OkanIntroduction: Hair tourniquet syndrome (HTS) is a rare surgical emergency caused by a hair or thread wrapping around an appendage. We aimed to present our clinical experience with HTS of toes and attract physicians' attention to this rare entity.Methods: Between January 2012 and September 2022; 26 patients (25 pediatric and one adult case) were treated for HTS. All pediatric cases were treated surgically under loop magnification. The adult patient was treated nonsurgically. The patient's age, gender, affected appendage and side, duration of symptoms, and postoperative complications were recorded.Results: Thirty-six toes of 25 patients (13 boys, 11 girls, and a male adult) were included in the study. The mean age of pediatric patients was 126.6 days. The third toe was the most affected (n:16), followed by the fourth (n:8). In seven patients more than one was affected.Conclusion: HTS should be treated as soon as possible when diagnosed to prevent further complications including appendage loss.& COPY; 2023 European Foot and Ankle Society. All rights reserved.Öğe Hair tourniquet syndrome of toes and fingers in infants(Turkish Assoc Orthopaedics Traumatology, 2019) Aslanturk, Okan; Ozbey, Rafet; Yilmaz, Ozgur; Ergen, EmreObjective: Hair tourniquet syndrome is an uncommon condition characterized by strangulation appendages by a hair or thread. The aim of this study was to evaluate the results of hair removal and antibiotic therapy in our patients with hair tourniquet syndrome. Methods: Between January 2012 and August 2018, 16 patients (8 boys, 8 girls; mean age: 118.5 [range: 20 to 380] days) were treated surgically for hair tourniquet syndrome. All patients were treated surgically under local or general anesthesia in the pediatric emergency department or in the operating theater using magnifying loupes. The age, gender, affected fingers or toes and the affected sides of the patients and the duration of symptoms until presentation were recorded. Results: A total of 24 toes and fingers were treated for hair tourniquet syndrome. The right side was affected in 12 patients, the left side was affected in three, and both sides in one. The second toe was affected in three patients, the third toe in eleven, the fourth in six, and the fifth in two patients. Both the thumb and the second finger were affected in one patient. The average duration of the symptoms (excessive crying, swelling, redness) was 1.5 (range: 1 to 2) days. All patients healed without any complications. Conclusion: Hair tourniquet syndrome should be kept in mind as an etiology in infants with toe and finger strangulation. These patients should be examined undressed. Immediate removal of hair is an effective treatment method to save appendage. (C) 2019 Turkish Association of Orthopaedics and Traumatology. Publishing services by Elsevier B.V.Öğe Prevalence of metacarpophalangeal sesamoid bones of the hand in Turkish population(Turkish Joint Diseases Foundation, 2021) Ergen, Emre; Yilmaz, Ozgur; Ari, Bunyamin; Nacar, Erkay; Cimen, Ayla Ozaydogdu; Ertem, Kadir; Aslanturk, OkanObjectives: This study aims to investigate the prevalence and location of the metacarpophalangeal (MCP) sesamoid bones using computed tomography (CT) images. Patients and methods: A total of 767 hands of 735 patients (503 males, 232 females; mean age: 36.9 +/- 17.0 years; range, 18 to 105 years) obtained from picture archiving and communication system were retrospectively analyzed between January 2016 and December 2019. The sesamoid bones of MCP joints I, II, III, IV, and V were recorded. Data including age, sex, side, number, pathologies, and location of the sesamoid bones were recorded. Results: The prevalence of sesamoid bones was found to be 100%, 37.61%, 1.17%, 0.5%, and 80% in MCP I, II, III, IV, and V, respectively. There was no significant correlation between the sex of the patient and presence of sesamoid bone at MCP II or MCP V (p>0.970 and p=0.176, respectively). The presence of sesamoid bone at MCP II was statistically significantly correlated with the presence of sesamoid bone at MCP V (p<0.001). There was no statistically significant difference in the side and sesamoid prevalence in the remaining 703 patients (p>0.05). Conclusion: The prevalence of MCP V sesamoid bone is higher than previous studies from our country. The CT of hand can be used to determine sesamoid fractures and degenerative conditions of sesamoids.Öğe Results of surgical treatment with proximal femur nail in adult femur pertrochanteric fractures(Bayrakol Medical Publisher, 2021) Ergen, Emre; Karakaplan, Mustafa; Ari, Bunyamin; Aslanturk, Okan; Yilmaz, Ozgur; Nacar, ErkayAim: Proximal femoral nails are widely used in the treatment of pertrochanteric fractures, as they can be applied in a shorter time and with less bleeding, with their more stable biomechanical properties during axial loading. In this retrospective study, conducted in our institution, we aimed to determine the effective- ness of PFN, especially in various osteoporotic conditions, mental health conditions, anesthesia risk, functional outcomes and mortality Material and Methods: Functional and radiologic results of 109 consecutive patients with intertrochanteric femur fractures, who have undergone osteosynthesis with proximal femoral nail between October 2009 and January 2015, were examined retrospectively. In addition to demographic characteristics, the type of fracture, comorbidities, type of trauma, duration from the moment of fracture to surgery, surgery duration and complications, presence of union and position of the implant were evaluated clinically and radiologically. The American Society of Anesthesiologists (ASA) scale was used for surgical risk assessment. The Harris Hip Scoring System was used for functional evaluation. Results: Fifty-nine patients were male and 50 were female. The mean age was 77,69 (17-102) years. The mean follow-up time was 32,9 (12-61) months. The etiology was as follows: a simple fall in %89.9 of cases, a fall from height in %7,3 of cases and motor vehicle accidents in %1,8. Operative procedures were performed within an average of 3,54 days after admission to our hospital (range 0-17 days). The average length of hospital stay was 7,76 days (range 2-56 days). Nine patients had complications related to the fracture. The mean score on the Harris scale at the last follow-up was 79,6 (48-100). The mortality rate in the first year was %29,6. A high ASA score and age over 80 years were found to generate a statistically significant increase in the risk of mortality and poor functional outcome. Discussion: Pertrochanteric fractures are usually diagnosed in elderly patients due to low energy traumas. The first choice of treatment must be surgical for an early return to pre-fracture activity levels. The proximal femoral nail is a reliable, safe and effective treatment method for pertrochanteric fractures due to its advantages such as application with closed reduction, providing anatomic and biological fixation, short surgical time, low blood loss and complication rate, and the possibility of early weight-bearing.Öğe Synthesis a group of 5(6)-substituted benzimidazole Zn(II) and Co(II) complexes and investigation their cytotoxic and antimicrobial activities(Wiley, 2022) Yilmaz, Ulku; Apohan, Elif; Kucukbay, Hasan; Yilmaz, Ozgur; Tatlici, Eray; Yesilada, OzferA series of 5(6)-substituted benzimidazole ligand-zinc(II) and cobalt(II) complexes were synthesized and characterized by H-1, C-13 NMR, IR, CHN analysis techniques. In vitro, anticancer and antimicrobial activities of ligand and complexes were evaluated. The cytotoxic activities of compounds were investigated against the human lung adenocarcinoma (A549) cell lines and healthy human lung bronchial epithelium (BEAS2B) cells. Compounds 1, 6, and 8 were observed to exhibit higher cytotoxic activity than the others at the 72nd hour. Also, the compounds showed varying degrees of antimicrobial activity against the microorganisms used. Compounds 1, 5, and 8 were most effective against standard strains Pseudomonas aeruginosa ATCC 27853 and Staphylococcus aureus ATCC 29213. As for the antifungal activity, compounds 1, 5, 6, and 8 showed high antifungal activity against the yeast type fungi Candida albicans ATCC 90028 and Candida tropicalis.Öğe Synthesis, cytotoxic and antimicrobial activities of novel cobalt and zinc complexes of benzimidazole derivatives(Elsevier Science Sa, 2017) Apohan, Elif; Yilmaz, Ulku; Yilmaz, Ozgur; Serindag, Ayfer; Kucukbay, Hasan; Yesilada, Ozfer; Baran, YusufIn this study fourteen novel cobalt (II) or zinc (II) complexes of benzimidazoles were synthesized from the 1-(4-substitutedbenzyl)-1H-benzimidazoles and CoCl2.6H(2)O or ZnCl2. Cytotoxic activities of novel complexes were investigated against lung cancer cells (A549) and BEAS-2B. Three of the examined compounds (1, 4 and 5) showed high cytotoxic activity against A549. While the IC50 of the cisplatin was 2.56 pg/mL for A549 cells at 72 h, the IC50 values of compounds 1, 4 and 5 were 1.97, 1.87 and 1.9 mu g/mL, respectively. IC50 values of these compounds for BEAS-2B cells were higher than the IC50 values for A549. While the IC50 values for BEAS-2B cells were 59.8, 24.5 and 32.67 mu g/mL, respectively, the IC50 of the cisplatin was determined as 2.53 pgimL in the present work. All rights reserved.Öğe Turkish obstetricians' self-birth preferences, attitudes and practices towards caesarean section on maternal request and vaginal birth after caesarean section: a national online survey(Taylor & Francis Inc, 2022) Astepe, Bahar Sariibrahim; Ayaz, Reyhan; Koleli, Isil; Yucedag, Mehtap; Yilmaz, Ozgur; Uzel, KemineThe aim of the study was to investigate the reasons for Turkish obstetricians having self-caesarean section on maternal request (CSMR) and their attitudes and practices related to CSMR and vaginal birth after caesarean section (VBAC). The questionnaire form constructed by the authors was sent online to obstetricians working in different cities of Turkey. A total of 206 obstetricians participated and answered a self-administered questionnaire. The self-birth preferences were as follows: 17% had a vaginal delivery (VD), 61.2% had a caesarean section (CS), 4.9% had both VD and CS. Of the participants with CS, 56.3% had CSMR. The most common reason for their having self-CSMR was that it was 'safe for the baby'. The most common reason for performing CSMR with their patients was 'due to birth anxiety and phobia'. Fifty-five percent of the participants said that they first recommended VD. The most common concern related to VBAC was 'I'm afraid of legal sanctions about complications'. Although many Turkish obstetricians recommend VD to their patients, they are afraid of the complaints/trials related to unforeseen complications during VD. IMPACT STATEMENT What is already known on this subject? Caesarean section (CS) rates all over the world and in Turkey are rising. The exact frequency of CS on maternal request (CSMR) is not known, but it is estimated to be between 4% and 18%. What do the results of this study add? Although Turkish obstetricians had high self-CSMR rates, they thought that it was more correct to direct patients to vaginal delivery (VD). Obstetricians are afraid of unforeseen complications during VD and related complaints and legal trials. What are the implications of these findings for clinical practice and/or further research? Physicians expect improvements in the judgement and punishment issues related to unforeseen complications in the birth process. Besides the actual rates of CSMR need to be reported in the literature and why obstetricians perform CSMR needs to be investigated globally.Öğe Which is more dangerous, earthquake, or the panic? Evaluation of the 24 January 2020 Elazig/T?rkiye earthquake related musculoskeletal injuries(Turkish Assoc Trauma Emergency Surgery, 2022) Ergen, Emre; Kaya, Oguz; Yilmaz, Ozgur; Ozdes, FlUseyin Utku; Batur, Omer Cihan; Karaman, Serdar; Guzel, IsmailBACKGROUND: The aim of this study is to evaluate the musculoskeletal injuries related with 24 January 2020 Elazig/Turkiye earthquake and their treatment protocols. METHODS: Data of patients applied to Inonu University Medical Faculty Hospital, Elazig Training and Research Hospital and Malatya Training and Research Hospital emergency departments within 48 h after the earthquake, were evaluated retrospectively. Age, gender, soft tissue injuries and sites, fracture sites and types, fracture etiology, and treatment methods were evaluated.RESULTS: 247 patients were evaluated. 118 were women and 139 were men. There were 24 (9.7%) pediatric patients. Mean age was 37.3 (1-92) years. Waist majority of injuries were simple soft-tissue injuries. There were 103 fractures in 86 patients. Thirty-eight patients' fractures were treated surgically.CONCLUSION: Every major disaster warrants retrospective studies so we can learn how to improve all levels of Emergency Medical Services. Great proportion of Elazig earthquake victims had only simple soft tissue injuries such as sprain, laceration, or contusion. Many patients were injured due to reasons indirectly related to the destruction brought by the earthquake. Panic caused by the earthquake caused more injury than the destruction it brought.
