Endoftalmili Olgularda Medikal ve Cerrahi Tedavi Sonuçları

Özet
Kliniğimizde takip ettiğimiz endoftalmili olgularda medikal ve cerrahi tedavinin görme düzeylerine olan etkisini değerlendirmek.
Gereç ve Yöntem: Fırat Üniversitesi Hastanesinde Ocak 2005-Eylül 2009 tarihleri arasında endoftalmi tanısı ile tedavi uygulanmış 24 hastanın 24 gözü incelendi. Olguların tümünden intravitreal örnek alınıp intravitreal antibiyotik uygulandı. Bütün hastalara oral antibiyotik (siprofloksasin, seftriakson) ve topikal antibiyotik (vankomisin, kinolon, seftazidim) uygulandı.
Bulgular: Olguların ortalama yaşı 60.58 (7-79 yaş) idi. Ortalama 2.58 ay (1hf -7 ay) takip edildiler. Olguların 18’i erkek, 6’sı kadın idi. Olguların 14 tanesinde (%58.33) katarakt cerrahisi sonrası gelişen endoftalmi, 8 tanesinde (%33.33) travma sonrası gelişen endoftalmi, 1 tanesinde (% 4.16) keratoplasti sonrası gelişen endoftalmi ve 1 tanesinde (% 4.16) endojen endoftalmi mevcuttu. Olguların 12 (%50)’inde vitreus örneğinde üreme oldu. Olguların 9 tanesine (% 41.66) cerrahi prosedür, 1 tanesine ise enflamasyon ilerlediği için eviserasyon yapıldı. Olguların 6 tanesine ise (%25) cerrahi planlandığı halde kabul etmediği için uygulanamadı. Olguların 9 tanesinde (%37.5) ortalama 3 sıra görme artışı sağlandı. Olgulardan 12 (%50) tanesinde görme düzeyinde değişiklik saptanmadı.
Sonuç: Endoftalmili olgularda uygun olarak yapılan medikal ve cerrahi tedaviyle enflamasyon büyük oranda kontrol altına alınabilmekte ancak olguların yarısında görme kaybının önüne geçilememektedir.
To determine the effect of medical and surgical treatments on visual acuity of patients with endophthalmitis in our clinic. Methods: Twenty four eyes of 24 patients who were treated for endophthalmitis in our clinic between January 2005- September 2009 were analyzed. Intravitreal antibiotic injection had been performed in all patients before vitreus samples were obtained. We administered topical (vancomycin, quinolones, ceftazidime) and oral (ciprofloxacin, ceftriaxone) antibiotics to all patients. Results: Mean age was 60.58 (7-79 years); 18 were male and 6 female. Predisposing factors for infective endophthalmitis included cataract surgery in 14 patients (%58.33), post traumatic endophthalmitis in 8 patients (%33.33), keratoplasty surgery in 1 patient (%4.16) and endogenous endophthalmitis in one patient. Twelve patients (%50) had culture positive vitreus samples. Vitrectomy was performed in nine patients but evisseration was performed in one patient because of the progressive inflamation after vitrectomy. Nine patients had a mean visual improvement of approximately three lines and 12 patients had no change of visual acuity. Conclusion: Inflammation could be controlled with medical and surgical treatment but visual loss could not be prevented in fifty percent of patients with endopthalmitis.
